Shoulder is one of the most important joints in human body. Many movements are related to shoulder joint in daily life. Shoulder discomfort is a common complaint in orthopedic clinics. In the process of diagnosis, patients with shoulder pain are accessed by shoulder physical examinations. However, physical examinations do not hold consistent results due to low sensitivity and specificity. For severer shoulder pain, medical images such as ultrasonography and magnetic resonance imaging (MRI) would be accompanied to confirm the diagnosis. Although with higher accuracy than physical examinations for identifying possible injured locations, medical images usually cost more and take longer and would be impractical for mild to moderate symptoms. Some studies use electromyography (EMG) to evaluate shoulder function, however, it is hard to measure deep muscles and move at the same time. The research purpose was to develop a method to evaluate shoulder disorder using musculoskeletal simulation in different movements. This study captured human movement using a motion capture system and EMG and input motion data to musculoskeletal simulation software (OpenSIM). The simulated shoulder model combined two currently models, and used 16 markers. Eleven healthy subjects and 2 patients were recruited. Subjects were asked to do the range of motion tests such as shoulder abduction, extension, forward flexion, external rotation, internal rotation, horizontal adduction and horizontal abduction. Then, subjects were asked to do physical examinations such as empty can test, lift-off test, Neer’s test, Hawkins’ test, apprehension relocation, O’Brien’s test and speed test. The movement data were used for the musculoskeletal simulation by our musculoskeletal model. The weight of each subjects and static motion should be input to model in order to calibration, and we simulated 15 motions. Our model has high correlation in simulation results and EMG. In total 231 parameters, the healthy group has 90.59% parameters belonging to normal distribution, and the Cronbach's alpha is 0.782 in the repeatability test. Two patients can be distinguished by six motions in this study, and Neer’s test and Hawkins’ test can be quantized in our method. To sum up, this study succeed to establish a method to evaluate shoulder disorder by using musculoskeletal simulation.
