Dietary carbohydrate intake and obesity are suggested as risk factors for postmenopausal breast cancer. The study was conducted to determine whether carbohydrate quality measured by glycemic index(GI) and glycemic load(GL), and plasma adipokine concentration account for breast cancer risk. Breast cancer patients (n=263) and hospital‐based control subjects (n=263) were recruited from National Cancer Center, Korea. GI and GL were calculated based on food frequency questionnaires, and plasma concentrations of adiponectin and leptin were determined. Study results indicated that the GI(p=0.037) and GL(p=0.045) values were negatively associated with plasma adiponectin concentration while GL (p=0.028) was positively associated with plasma leptin concentration. There was a significant association between GI (OR = 6.06, 95% CI=2.24–16.37, p=0.0029), and GL (OR=4.42, 95% CI=1.59–12.28, p=0.0191) with breast cancer risk in postmenopausal women. The associations between GI, GL and breast cancer were more evident in subjects with lower plasma adiponection.
