الوصف: Countries, undergoing intense social and economic transition, usually experience growing inequalities in mortality, which are caused by different health improvement trends. In some countries, these trends can be even diverging.1 Higher socioeconomic groups benefit more from improving general economic situation and prosperity. During such periods of transition, health inequalities are mainly determined by unequal access to health care resources, as well as different distribution of behavioral risk factors, related to social and economic stratification.2 Social disadvantage has cumulative effects on health disadvantages over time.3 Nevertheless, the nature of inequalities in health differs in various countries and is changing throughout time: when societies enter more stable period of development, other, ‘non-causal’ and ‘non-material’ factors start playing more important role. Social selection on the basis of health also contributes to health inequalities.4 This might be more obvious in the countries with universal and generous welfare arrangements, as well as sustainable development. Concerning measurement of health inequalities, absolute inequalities gradually gain more importance, reflecting the ‘real’ situation in more optimistic way; however, measuring relative inequalities should also be taken into account. Therefore, I suggest that while measuring and explaining causes of health inequalities, country economic and social context should be taken into consideration.
