Obesity has emerged as a strong risk factor for severe COVID-19 infection, making individuals with obesity more susceptible to hospitalisation, assisted ventilation and death. This article reflects how obesity and COVID-19 compose a pandemic overlap of syndemic effects whose synergies reinforce both biological and structural disadvantages reproducing and exacerbate persistent health disparities and inequalities over time. The case of obesity, and its ecology of health problems, constitutes a global health challenge that points to the need to look at health in a broader framework which takes into account how the central drivers of society, and the economic and social policy under neoliberalism, affect the distribution of health and disease, potentially conditioning other pandemic viral crises in the future. A obesidade surgiu como forte fator de risco para os quadros graves de infeção por COVID-19, tornando os indivíduos com obesidade mais suscetíveis à hospitalização, ventilação assistida e morte. O presente artigo reflete como obesidade e COVID-19 compõem uma sobreposição de pandemias de efeitos sindémicos, cujas sinergias reforçam quer desvantagens biológicas quer estruturais, reproduzindo e agravando disparidades e desigualdades de saúde persistentes ao longo do tempo. O caso da obesidade e a sua ecologia de problemas de saúde constitui um desafio de saúde global, que aponta a necessidade de olhar a saúde num quadro mais alargado que tenha em conta como os motores centrais da sociedade e a política económica e social sob o neoliberalismo afetam a distribuição da saúde e doença, condicionando potencialmente outras crises virais pandémicas no futuro.
