الوصف: INTRODUCTION AND AIMS: Relapses of nephrotic syndrome often follow viral infections, commonly of the upper respiratory tract. Based on the assumption that corticosteroids inhibit cytokine production, we examined, in a prospective randomized controlled trial, whether daily administration of maintenance prednisolone during infections, reduces relapse rates in children with frequently relapsing nephrotic syndrome. METHODS: Patients with idiopathic, frequently relapsing nephrotic syndrome eligible for therapy with prolonged alternate day (ad) prednisolone (0.5-0.7 mg/kg) with/without levamisole (2 mg/kg ad) were eligible. Stratified randomization (based on baseline treatment) was done to groups I (Intervention) or C (Control). During viral infections, patients in Gr.I were advised to take the same dose of prednisolone every day for 7 consecutive days; patients in Gr.C continued to receive ad prednisolone. Relapse was defined as 3+ proteinuria on 3 consecutive days&treated with prednisolone [2 mg/kg/d until remission (trace/nil protein for 3 days);1.5 mg/kg ad for 4-wk,then tapered]. Relapse considered infection-associated if it occurred within 2-weeks of onset of viral illness. Patients were followed for 12 months; intention to treat principle was used for analysis. RESULTS: Of 142 patients screened, 100 were included (50 each in Gr.I&C). The mean age at inclusion was 78.5± 32 and 81.7± 33 months respectively in Gr. I and C. The respective mean relapse rates in preceding 12 months were 3.4±0.4 and 3.5±0.4. 86 children completed the trial. Five were lost to follow up (3&2 in gr.I&gr.C); 9 required alternative treatment [cyclophosphamide(5), mycophenolate&tacrolimus(2 each)]. Patients in Gr.I showed significantly lower relapse rates (Table; mean difference 0.9 episodes/patient/yr; 95% CI 0.4-1.4). Majority (92%) infections were upper respiratory (226 in Gr.I, 161 in Gr.C). The number of infection-associated relapses was significantly lower in Gr.I (Table; mean difference 0.7; 95% CI 0.3-1.1).
