Adrenal Carcinoma is a rare neoplasm able to secret hormones from all layers of adrenal cortex. This is a report of adrenal carcinoma in a young woman with recently diagnosed diabetes mellitus and arterial hypertension, who presented with a cushingoid phenotype, virilization, and elevated lactic dehydrogenase. The patient was biochemically studied to corroborate hypersecretion of adrenocortical hormones, additionally infiltrative liver and bone lesions were documented. Surgical intervention is considered definitive therapy, but she was not a candidate for this procedure, due to tumor invasion in vessels of the portal area, she underwent for salvage chemotherapy and steroidogenesis inhibitor. El carcinoma adrenal es una neoplasia rara capaz de secretar hormonas de todas las capas de la corteza suprarrenal. Se presenta el informe de un caso de carcinoma suprarrenal en una mujer joven con diabetes mellitus e hipertensión arterial de reciente diagnóstico, que se presentó con fenotipo cushingoide, virilización y deshidrogenasa láctica elevada. La paciente se abordó bioquímicamente para corroborar hipersecreción de hormonas corticosuprarrenales, adicionalmente se documentó lesión infiltrativa hepática y ósea. Se considera la intervención quirúrgica como la terapia definitiva, pero al no ser candidata a dicho procedimiento debido a invasión tumoral en vasos del área portal, se optó por quimioterapia de rescate y un inhibidor de esteroidogénesis.
