    الوصف: The World Health Organization (WHO) has recently introduced an international approach to standardize reporting of pancreaticobiliary cytopathology. The WHO Reporting system for Pancreaticobiliary Cytopathology (WHO system) introduces two distinct categories for non-invasive premalignant lesions of the ductal system, based on cytomorphological grading: “pancreatic neoplasm of low risk/grade” (PaN-low) and “pancreatic neoplasm of high risk/grade” (PaN-high). This reclassification aims to provide a more precise assessment of risks of malignancy (ROM) for different neoplastic categories. The WHO system focuses on the diagnostic categories, their associated ROM, and recommended management per category.Pancreatic neoplasms comprise various histological types, each displaying distinct background features that can assist in the diagnostic process. Recognizing background features like mucin background, necrotic background, desmoplastic stroma, cancer-associated fibroblasts, and stromal fragments presents notable challenges. Certain background features may overlap across histological types.In this review, we present a summary of the key updates in the WHO system and highlight diagnostic challenges associated with background features.

    الوصف: Aiming at the problem that the back module of the existing nursing bed chair is difficult to realize the mutual change between the postures of the nursing bed and wheelchair,a new type of back plate mechanism is proposed. A serial backplane spatial combination mechanism is formed by connecting multiple mechanisms with single function,and its spatial freedom is analyzed to meet the motion demand. The target size is parameterized,and the optimal position of the support connecting rod,backplane and support frame hinge is obtained using the minimum inclusive area method. According to the geometric relationship and statics analysis result of the backplane mechanism,the JC35D electric push rod with rated stroke and working stroke of 175 mm and 230 mm and self-locking force of 1 000 N is used as the driving mechanism. The backplane mechanism of the nursing bed chair is designed according to the optimized parameters and the drive device type,and the overall virtual prototype is assembled by SolidWorks. The simulation finite element analysis plug-in is used to analyze the stress of the backplane under the posture of the nursing bed chair,and the safety and reliability of the structural strength are verified. Under the posture of nursing bed,Adams View software is used to carry out the static rollover simulation test of the mechanism,and the stability requirement of the backplane under the rollover test load is verified.

    الوصف: Abstract Background The BRAFV600E mutation is valuable for the diagnosis, prognosis, and therapy of papillary thyroid cancer (PTC). However, studies related to this mutation have involved only a small number of patients. Therefore, we performed a large‐scale analysis from a single institute to evaluate the accuracy of combined fine‐needle aspiration (FNA) and BRAFV600E mutation tests for PTC diagnosis. Methods A total of 4600 patients with thyroid nodules who underwent both FNA cytology and BRAFV600E mutation analysis on FNA specimens were enrolled. The association between the BRAFV600E mutation and clinicopathological features was analyzed. A separate analysis was performed for the 311 patients who underwent repeated FNA for comparison of cytological evaluation and BRAFV600E mutation results. The diagnostic efficacy of the BRAFV600E mutation test and cytologic diagnoses was evaluated for 516 patients who underwent preoperative FNA tests in comparison with conclusive postoperative histopathologic results. Results The cytology results of all 4600 FNA samples were categorized according to The Bethesda System for Reporting Thyroid Cytology (TBSRTC) stages I–VI, which accounted for 11.76%, 60.02%, 6.46%, 3.61%, 6.71%, and 11.43% of the samples, respectively. The BRAFV600E mutation was detected in 762 (16.57%) FNA samples, with rates of 1.48%, 0.87%, 20.20%, 3.01%, 66.02%, and 87.81% for TBSRTC I–VI lesions, respectively. Among the 311 repeat FNA cases, 81.0% of the BRAFV600E‐positive and 4.3% of the BRAFV600E‐negative specimens with an initial indication of cytological non‐malignancy were ultimately diagnosed as malignant by repeat FNA (p

    الوصف: Beijing, the capital of China, is experiencing a serious lack of water, which is becoming a main factor in the restriction of the development of the social economy. Due to the low economic efficiency and high consumption proportion of agricultural water use, the relationship between economic growth and agricultural water use is worth investigating. The “decoupling” index is becoming increasingly popular for identifying the degree of non-synchronous variation between resource consumption and economic growth. However, few studies address the decoupling between the crop water consumption and agricultural economic growth. This paper involves the water footprint (WF) to assess the water consumption in the crop production process. After an evaluation of the crop WF in Beijing, this paper applies the decoupling indicators to examine the occurrence of non-synchronous variation between the agricultural gross domestic product (GDP) and crop WF in Beijing from 1981 to 2013. The results show that the WF of crop production in 2013 reduced by 62.1% compared to that in 1980 — in total, 1.81 × 109 m3. According to the decoupling states, the entire study period is divided into three periods. From 1981 to 2013, the decoupling states represented seventy-five percent of the years from 1981 to 1992 (Period I) with a moderate decoupling degree, more than ninety percent from 1993 to 2003 (Period II) with a very strong decoupling degree and moved from non-decoupling to strong decoupling from 2004 to 2013 (Period III). Adjusting plantation structure, technology innovation and raising awareness of water-saving, may promote the decoupling degree between WF and agricultural GDP in Beijing.

    الوصف: Abstract Background Intestinal metaplasia (IM) of the gastric mucosa is classified as complete (Type I) and incomplete IM (Type II and III) subtypes, which showed significantly different risk for developing to gastric adenocarcinoma (GAC). GCRG213, a variant of L1-endonuclease (L1-EN), first identified in our lab, was upregulated in GAC tissue. However, the relationship between GCRG213 and IM subtypes is not clear. Our study explored the association of GCRG213 protein (GCRG213p) with IM subtypes. Methods Gastric cancer and/or para-tumor tissue samples were collected from 123 patients who underwent gastrectomy for intestinal type gastric adenocarcinoma. The subtypes of IM were characterized with Alcian blue-periodic acid-Schiff and High Iron Diamine-Alcian blue staining methods. Immunohistochemistry of GCRG213p was performed, and its expression in gastric adenocarcinoma and para-tumor tissue including dysplasia, IM, and normal mucosa were analyzed. Results GCRG213p was expressed in 48.94% IM, 57.14% dysplasia and 55.32% GAC, respectively. GCRG213p expression was higher in well and moderately differentiated adenocarcinoma (P = 0.037). In IM glands, GCRG213p expressed mainly in the cytoplasm of absorptive enterocytes with defined brush borders, but not in goblet cells.     الوصف: Abstract Background Programmed death ligand 1 (PD-L1) is a ligand for the inhibitory programmed cell death protein 1 (PD-1), which are targeted by several anti-PD-1 and PD-L1 drugs for a variety of human cancers. However, only a few studies have evaluated PD-L1 expression in esophageal squamous cell carcinoma (ESCC) with a large Chinese cohort. Our present study is to evaluate the association of PD-L1 expression with clinicopathological features on ESCC. Methods Using tissue microarray and immunohistochemistry, PD-L1 expression on tumor cells and tumor-infiltrating immune cells was studied in 378 advanced ESCC patients without neoadjuvant chemoradiotherapy. Its correlation with clinicopathological parameters was analyzed. Results PD-L1 was expressed on 29.9% (113/378) ESCC tumor cells and 40.2% (152/378) tumor-infiltrating immune cells.     الوصف: Salmonella infection in humans can become chronic, which leads to low-grade persistent inflammation. These chronic infections increase the risk of several gastrointestinal diseases, including cancer. Salmonella AvrA is a multifunctional protein that influences eukaryotic cell pathways by regulating ubiquitination and acetylation. In an animal model, we have demonstrated that infection with AvrA-expressing Salmonella induces beta-catenin signals and enhances colonic tumorigenesis. Beta-catenin signaling is a key player in intestinal proliferation and tumorigenesis. The relative contributions of AvrA-induced proliferation and inflammation on tumorigenesis, however, are unknown. STAT3 is activated in chronically inflamed intestines in human inflammatory bowel diseases and in colitis-associated colon cancer. In the current study, mice were colonized with Salmonella AvrA-sufficient or AvrA-deficient bacterial strains. Then, inflammation-associated colon cancer was induced through the use of azoxymethane/dextran sulfate sodium. We determined that AvrA-expressing bacteria activated the STAT3 pathway, which is predicted to enhance proliferation and promote tumorigenesis. Transcriptional activity of STAT3 and its target genes were upregulated by Salmonella expressing AvrA, thus promoting proliferation and intestinal tumorigenesis. Our findings provide new insights regarding a STAT3-dependent mechanism by which the specific bacterial product AvrA enhances the development of infection-associated colon cancer. These insights might suggest future biomarkers to risk assessment and early detection of infection-related cancer.

    الوصف: Acinic cell carcinoma (AciCC) may pose a diagnostic challenge, particularly on small biopsies and fine needle aspiration (FNA) because of its variable histology including potential high-grade transformation and its mimickers. Immunoreactivity with circumferential membranous staining for DOG1 can support the diagnosis of AciCC but is not entirely specific. A novel rearrangement t(4;9)(q13;q31) leading to up-regulation of nuclear receptor subfamily 4 group A member 3 (NR4A3) has been described in AciCC, is potentially detectable by fluorescence in situ hybridization (FISH) and may be useful in the evaluation for AciCC. Using NR4A3 Dual Color Break Apart Probe (ZytoVision, Germany) FISH was performed on AciCCs from 3 large academic institutions. NR4A3 rearrangement was defined as positive signal patterns in 15% of tissue interphase nuclei. Fifty-two AciCCs including 47 resections and 5 FNAs (including 5 paired FNA/resections) were analyzed. Five non-AciCC salivary gland tumors and 2 sialadenitis cases were used as controls. Eight AciCCs (15%; 8/52) failed FISH testing. FISH was positive in 23 AciCCs (sensitivity 59%, 23/39) with 100% concordance between 5 matched resection/FNAs (3 were positive for FISH and 2 were negative). FISH was negative in all non-AciCCs (specificity: 100%, 0/7). NR4A3 FISH has a sensitivity of 59% and specificity of 100% in detecting AciCC, which suggests that NR4A3 rearrangement-driven up-regulation is a recurrent, specific oncogenic event in AciCC, consistent with prior results. Hundred percent concordance between matched FNA/resection samples validates its potential utility on cytology samples.
