    الوصف: Both a cylinder and a slab phantom have been recommended to be used as calibration phantoms for eye lens dosimetry in the International Atomic Energy Agency TECDOC. This study describes investigations on the influence of the type of phantom on the calibration of dosemeters. In order to fulfil the purpose, backscatter radiation from practically used water-filled phantoms was evaluated by calculations and experiments. For photons, the calculations showed that the cylinder phantom had 10 % lower backscattered effect at maximum than a slab phantom, and simulated well the backscattered effect of the human head or neck to within ±10 %. The irradiation results of non-filtered optically stimulated luminescence and radio-photoluminescence glass dosemeters indicated that the differences of the calibration factors between the two types of phantoms were up to 20 and 10 %, respectively, reflecting the response to backscattered photons. For electrons, no difference was found between the two types of phantoms.

    الوصف: Background In Japanese guidelines, target value of low-density lipoprotein cholesterol (LDL-C) <100mg/dL is recommended as standard management for secondary prevention of coronary artery disease. On the other hand, the guidelines also state that LDL-C targeting <70mg/dL should be considered in high-risk patients. However, the effectiveness of strict LDL-C management in the prevention of long-term coronary event recurrence in Japanese patients remains unclear. Purpose The purpose of the present study was to evaluate whether the strict management of LDL-C targeting <70 mg/dL was effective to prevent recurrence of acute coronary syndrome (ACS) than standard management in patients with previous percutaneous coronary intervention (PCI). Methods From January 2007 to August 2020, we performed coronary angiography in 359 patients with previous PCI who were suspected of having signs of recurrent cardiac ischemia. Patients were stratified into three groups according to achieved LDL-C value; <70mg/dL (n=57), 70 to <100mg/dL (n=135) and ≥100mg/dL (n=167). In addition, patients who had previous ACS and/or diabetes mellitus were defined as high-risk group, and sub-analysis by their achieved LDL-C values was performed in high-risk group and non-high-risk group. Endpoint was recurrence of ACS. Moreover, risk factors associated with recurrent-ACS were examined in patients with LDL-C <100 mg/dL. Results After follow-up (median 6.1 years), 99 patients (28%) had recurrent-ACS. Recurrent-ACS was significantly lower in patients with LDL-C <70mg/dL than LDL-C 70 to <100mg/dL and LDL-C ≥100mg/dL (p<0.01 and p<0.001, respectively). In sub-analysis, high-risk group with LDL-C <70 mg/dL had lower incidence of recurrent-ACS than LDL-C 70 to <100 mg/dL (p=0.03). Similar tendency was found in non-high-risk group (p=0.08).
