    الوصف: BackgroundMicrovascular decompression (MVD) remains the primary surgical treatment for trigeminal neuralgia due to its positive postoperative results. This study aims to evaluate the outcomes of patients with primary trigeminal neuralgia who underwent MVD. Additionally, the paper offers a detailed explanation of the surgical methodology of MVD employed at the neurosurgical hospital in Kazakhstan.MethodsThe study involved 165 medical records of patients with trigeminal neuralgia who underwent MVD between 2018 and 2020. Out of these 165 patients, 90 (54.55%) were included in the final analysis and were further evaluated using the Barrow Neurological Institute pain intensity score. Various variables were analyzed, including age, sex, affected side, dermatomes, offending vessel, and surgical intervention type. Moreover, the surgical technique employed at the hospital was described.ResultsThe average follow-up period after the MVD procedure was 32.78 ± 9.91 months. The results indicated that out of the 90 patients, 80 (88.89%) achieved a good outcome as evidenced by BNI scores I and II. It was observed that patients with affected maxillary dermatomas and those with affected ophthalmic + maxillary dermatomas were more likely to experience fair + poor postsurgery BNI scores. On the other hand, patients with neurovascular conflicts involving the maxillary + mandibular dermatomas demonstrated good BNI scores (p = 0.01).ConclusionsThe outcomes of MVD in patients with primary trigeminal neuralgia showed good BNI scores within this study population. The outcome depended on the affected dermatome of the trigeminal nerve with the vessel. Additionally, patient positioning, intraoperative management including small skin incisions, minimal craniotomy, and precise closure of the dura, as well as intraoperative neurolysis, may contribute to achieving good clinical and satisfactory post-surgery aesthetic outcomes.

    الوصف: Introduction: The transverse-sigmoid-sinus-transition constitutes an important landmark during a retrosigmoid craniotomy. Due to anatomical variations, the location is highly variable. Landmarks for identification of the anterior border of the sigmoid sinus have been described extensively, such as the mastoid notch, digastric point, external auditory meatus and crux of the helix curvature. There is a paucity of landmarks for the identification of the posterior border, however. Research question: We examined the relationship between the transverse-sigmoid-sinus-transition and the most-posterior-part-of-the-auricula. Material and methods: We performed a retrospective analysis of one-hundred patients (38 males and 62 females) who underwent cerebral MRI examinations at Antwerp University Hospital (Belgium). Using Brainlab®, the transverse-sigmoid-sinus-transition and most-posterior-part-of-the-auricula coordinates were calculated and compared. Left and right sides were compared in both the anteroposterior and craniocaudal axis. Results: Mean age was 56.4 ± 16.1 years. Mean MPPA-TSST-distance in the anteroposterior direction was −1.93 mm (right) and −1.96 mm (left). Mean MPPA-TSST-distance in the craniocaudal direction was −5.16 mm (right) and −5.04 mm (left). Discussion and conclusion: The transverse-sigmoid-sinus-transition seems to be located more anterior and caudal with respect to the most-posterior-part-of-the-auricula, meaning that it can be considered a save landmark. A correction of five mm needs to be applied in order to identify the inferior border of the transverse sinus. Left/right and gender had no significant influence. The most-posterior-part-of-the-auricula can be considered a fast and practical anatomical landmark for identification of the transverse-sigmoid-sinus-transition, without affecting operative fluency, especially during an emergency craniotomy.

    الوصف: Although the role of iron in bacterial infections has been well described for Staphylococcus (S.) aureus, iron acquisition in (bovine-associated) non-aureus staphylococci and mammaliicocci (NASM) remains insufficiently mapped. This study aimed at elucidating differences between four diverse bovine NASM field strains from two species, namely S. chromogenes and S. equorum, in regards to iron uptake (with ferritin and lactoferrin as an iron source) and siderophore production (staphyloferrin A and staphyloferrin B) by investigating the relationship between the genetic basis of iron acquisition through whole genome sequencing (WGS) with their observed phenotypic behavior. The four field strains were isolated in a previous study from composite cow milk (CCM) and bulk tank milk (BTM) in a Flemish dairy herd. Additionally, two well-studied S. chromogenes isolates originating from a persistent intramammary infection and from a teat apex were included for comparative purpose in all assays. Significant differences between species and strains were identified. In our phenotypical iron acquisition assay, while lactoferrin had no effect on growth recovery for all strains in iron deficient media, we found that ferritin served as an effective source for growth recovery in iron-deficient media for S. chromogenes CCM and BTM strains. This finding was further corroborated by analyzing potential ferritin iron acquisition genes using whole-genome sequencing data, which showed that all S. chromogenes strains contained hits for all three proposed ferritin reductive pathway genes. Furthermore, a qualitative assay indicated siderophore production by all strains, except for S. equorum. This lack of siderophore production in S. equorum was supported by a quantitative assay, which revealed significantly lower or negligible siderophore amounts compared to S. aureus and S. chromogenes.     الوصف: This text observes and analyzes the risks of medical-health responsibility on the part of a state social enterprise (ESE), Valle del Cauca, Colombia. It is based primarily on the rates of decrease of frequency in the attention of health service users as a consequence of the autonomy in the application of the "technical notes" of the IPS (institutions providing health services), as of decree 441 of March 28, 2022. The methodology corresponds to a systematic review study, which allows an analysis of the data provided by an ESE (Empresa Social del Estado), representative of Valle del Cauca during the period of validity of 2022. The review exercise reports the new contracting models, the agreement of wills, the technical notes and the prospective payment modalities in an annex that is an integral part of the agreement. The exercise reveals and gives as a result that, upon validation with the institutional historical data, the asymmetry of the data provided in the "technical notes" does not coincide with the frequency of use of institutional health services and technologies in the last five years. Subsequently, there is evidence of a significant decrease in the user's health care services, the effect of which is demarcated in the benefit per event that would not be paid to the different entities due to the great lack of specifications in the "technical note". What is evident is that they take the values of the capita and the event to the note, that is to say, it is proposed to pay a lower rate than what is billed, now adding the capita and the event (the note goes to 321 million in the note, and the capita is currently 307, without adding the event).     الوصف: Introduction: Lumbar spondylolysis is a common fatigue fracture of the pars interarticularis of the lamina of the lumbar spine in adolescent athletes presenting with pars clefts. Some pseudarthrotic lumbar spondylolysis causes low back pain or radiculopathy. This study presents a case of pseudarthrotic lumbar spondylolysis that was successfully treated using a modified smiley face rod technique. Technical Note: We developed a modified smiley face rod technique, which places pedicle screws in the lateral edge of the pedicle to preserve the erector spinae muscles and inserts a U-shaped rod between the spinous processes to preserve the supraspinous ligament. When a U-shaped rod penetrates the interspinous ligament subcutaneously, the resection of the supraspinous ligaments can be avoided. When the screw head is positioned more anterolaterally, a compression force is applied perpendicular to the surface of the pars cleft by rod clamping. This intrasegmental fusion technique preserves the mobile segment and simultaneously repairs the pars cleft. It is less invasive and more appropriate than interbody fusion for young athletes to avoid the possibility of future adjacent segment disorders. Conclusions: This is a minimally invasive procedure that can easily achieve bone fusion and should be introduced for patients who are suffering from the symptoms of pseudarthrotic lumbar spondylolysis.

    الوصف: Anatomic total shoulder arthroplasty, when used for treatment of primary glenohumeral arthritis, is historically very successful. We propose a novel technique for subscapularis repair during closure of a deltopectoral approach to the shoulder with subscapularis peel. Our technique allows for early motion following surgery and also provides for improved subscapularis repair integrity and resilience during postoperative rehabilitation. Postoperatively, we allow passive and active assisted range of motion at week 1, limited active range of motion at week 2, and unrestricted external rotation range of motion beginning at week 6. The use of our technique has led to improved patient outcomes with regard to range of motion postoperatively following anatomic total shoulder arthroplasty and we recommend its adoption into practice.

    الوصف: Intrathecal targeted drug delivery provides effective relief for cancer-related pain. However, its validation in management of craniofacial pain remains much less widely practiced, mainly due to the localized diffusion of analgesic agent with current approach. Here, we report our experience of prepontine cisternal routine for placement and implantation of intrathecal targeted drug delivery in two cases of cancer-related craniofacial pain. Lumbar cannulation was applied and the intrathecal catheter tip was positioned at the prepontine cistern under fluoroscopic guidance during the surgical implantation. Postoperative imaging confirmed that the catheter tip was successfully placed in the prepontine cisternal space. Satisfactory control of pain was achieved after intrathecal therapy, with significant reduction of background and breakthrough cancer pain. None obvious complications were observed in this study. Thus, our novel intrathecal routine may provide an alternative option for craniofacial pain caused by tumor, who were insufficiently treated by oral analgesic agents.

    الوصف: The incidence of reverse shoulder arthroplasty (RSA) has increased since the Food and Drug Administration approved its use in the United States in 2004. With the current RSA implants available for surgeon use within the United States of America, each design, regardless of humeral inlay vs. onlay, distalizes the shoulder’s center of rotation. This new center of rotation purposely increases tension to the deltoid, which is the main working muscle in RSA, but also retensions the adjacent tendons. Some patients after RSA experience continued anterior shoulder discomfort that limits their active range of motion and overall patient satisfaction. It has been isolated with physical examination that there is tenderness to palpation at the coracoid process and throughout the conjoint tendon. We have completed conjoint tendon lengthening procedures on this subset of patients with excellent clinical results. This article describes the technique for patients who underwent conjoint tendon lengthening after RSA for recalcitrant anterior shoulder pain.
