الوصف: Professor Sérgio Mascarenhas was a Brazilian researcher with a vast legacy. His work paved the way for new research possibilities by consolidating the use of innovation and transdisciplinary science. In Medicine, he proposed changes to what had previously been well-accepted concepts, and his contributions have influenced medical practices. Although many authors consider intracranial pressure (ICP) as an unrivaled variable for monitoring and diagnosis of many diseases, its clinical applicability is still the subject of debate in the literature because of the difficulty in standardizing protocols. Mascarenhas's research and the creation of a device for noninvasive monitoring of intracranial compliance are discussed and are shown to have led to the creation of Brain4care, a start-up, and a new perspective on the debate on ICP monitoring.

الوصف: Background: Candida is one of the main etiological agents of bloodstream nosocomial infections, and is associated with high morbidity/mortality in children. High use of empirical antifungals incurs in higher health care costs and concerns on emergent resistant Candida spp. Therefore, a prediction model should optimize prescribing empirical antifungals in pediatrics. Objective: Based on real-world evidence data, we aimed at assessing how accurate is a regression model for predicting central venous catheter (CVC) candidemia in pediatric patients. We also explored how this prediction model might be useful for prescribing antifungals empirically. Methods: A case-control study was conducted based on 144 patients with positive and negative blood cultures for Candida spp. A model for predicting Candida spp. was developed through univariate and multivariate analyses (logistic regression). Data were reported as odds ratio (OR) and p-value less than 0.05 was considered statistically significant. Results: Candidemia was predicted by the presence of CVC (OR 2.561, p=0.0042). For this model, an area under the curve (AUC) of 0.550 (p=0.314) was estimated, representing the Receiver Operating Characteristic curve of the present study, with a sensitivity of 86% and a specificity of 24%. Conclusion: Our study demonstrated that CVC is a fragile predictor for candidemia, however, its clinical significance and low specificity obtained in the ROC curve suggests that other covariates rather than those investigated should be considered to assess its impact on a prediction model for pediatric patients. ; Introducción: Candida es uno de los principales agentes etiológicos de infecciones nosocomiales del corriente sanguínea, asociada con una elevada morbimortalidad en niños. الوصف: Introduction: Intracranial hypertension (IH) is a clinical condition secondary to the loss of cerebral compensatory mechanisms. There are distinct methodologies used to diagnose IH and among them is the brain4care (b4c) sensor, which allows non-invasive monitoring of intracranial volume and pressure variations, intracranial compliance. Objective: To characterize the epidemiological profile of patients with suspected IH in the pediatric age range. Method: A cross-sectional study was carried out in a reference pediatric hospital in Brazil, with patients with signs and symptoms of IH monitored with the non-invasive B4C sensor. The patients underwent neurological evaluation and were submitted to exams - ophthalmological evaluation, lumbar puncture, magnetic resonance imaging (MRI) and computed tomography (CT). Result: 58 patients were evaluated, of whom 32 are female (55.2%) and 26 are male (44.8%); most patients presented symptoms such as drowsiness (81%), nausea (77.6%), headache (74.1%), vomiting (63.8%). The ophthalmoscopic examination 77.6% (n=58) of the patients did not show signs of papilledema. On CT and MRI, no changes were found in 84.5% (n=58) and 69.2% (n=26), respectively. Lumbar puncture showed changes in 57.1% (n=21). Considering the interpretation that would be made in adult patients, the b4c sensor monitoring (n=58) showed probable changes in the lying and sitting positions, respectively, in 46.3% and 38.9%. Conclusion: It was found that the b4c device can provide a possible complement of clinical information in the process of monitoring cerebral compliance. ; Introducción: La hipertensión intracraneal (HI) es una condición clínica secundaria a la pérdida de los mecanismos compensadores cerebrales.
