    مستخلص: BACKGROUND: Conducting research on the work outcomes of first episode psychosis (FEP) samples may extend our understanding of the factors associated with the work outcome of people with schizophrenia and other psychotic illnesses. OBJECTIVE: To conduct a detailed study of the work outcome of an FEP sample. METHODS: Members of a FEP cohort, who had completed a 12-year clinical outcome assessment, were invited to participate in an adjunctive work outcome study. Engagement in paid and non-paid work was first established and the relationship with potentially influential baseline characteristics investigated. Subsequently the influence of work outcome to participants' level of quality of life, mental health, recovery, and social inclusion were examined. RESULTS: Among the 38 participants the mean percentage of time spent in work was 62% of which 50% was in paid work and 12% was in non-paid work. Being employed at inception was the only independent predictor of the duration of the follow-up period spent in work. Relationships between work outcome and all measures of wellbeing were found. CONCLUSION: The paid and non-paid work attained by people affected by a psychotic illness played an important role in the extent of their wellbeing, recovery, and social inclusion.     الوصف: Two matched groups are compared, 100 industrial trainees and 870 juvenile offenders, who comprised the total population of Ireland's one Juvenile Open Centre during a 4-year period. Variables examined include family size, birth order and whether the family was broken by death, divorce, separation or desertion. The family size of the offenders was almost twice the national average and significantly greater than that of the comparison group. Among the offenders there was a considerable under-representation of last-borns, and first-borns from larger families. However, the over-representation of middle-borns was slight. There were no birth order effects for the trainees. No difference was found between the 2 groups with respect to loss of either parent through death. However, the offenders were 5 times more likely to come from a home broken by divorce, separation or desertion.

    الوصف: Objective: To determine levels of satisfaction with information and advice received about stroke disease and relevant issues by community-dwelling stroke survivors. Design: An interview questionnaire survey. Setting: A family health services authority area in northern England. Subjects: Stroke survivors identified by a valid screening questionnaire from a stratified random sample of 2000 subjects aged 45 years and over. Main outcome measure: Proportion of subjects interviewed responding positively to the question 'Do you think you have received enough advice and information on ... ?' eighteen topics considered relevant to stroke survivors. Results: The screening process identified 116 stroke survivors, of whom 76 agreed to be interviewed. The majority (> 80%) of patients were satisfied with information and advice received on lifestyle and health promotion issues, incontinence and their current treatment. However, satisfaction was poor for the areas of stroke disease in general, its effects, available services, and legal and financial affairs (range, 28-75% satisfied). Conclusion: Deficits in the provision of adequate information and advice to stroke patients on relevant issues have been identified in this study of a representative sample of community-dwelling stroke survivors. These deficits need to be addressed by those providing care for stroke patients in order to improve patient satisfaction.

    الوصف: During the last trimester of pregnancy, 127 primiparous Irish mothers were interviewed to ascertain their history of alcohol and tobacco use. Confounding effects due to other drugs were not a factor in this sample. Mothers consumed an average of .21 ounces absolute alcohol (AA) per day, with 62% classified as moderate drinkers, 10.6% as heavy drinkers, and 26% as nondrinkers. Neurobehavioral status was measured using acoustic characteristics of the infant's cry, collected on the third day of life. Multiple regression analysis showed that more ounces AA per day was related to more dysphonation and higher first formant, while more cigarette smoking was related to higher pitch, higher second formant, and more variability in the second formant. Analysis of variance comparisons of these 3 alcohol groups demonstrated significant cry effects on infants of heavy drinking mothers.
