الوصف: Objectives The Modern Innovative Solutions to Improve Outcomes in Asthma, Breathlessness and Chronic Obstructive Pulmonary Disease (COPD) (MABC) service aimed to enhance disease management for chronic respiratory conditions through specialist multidisciplinary clinics, predominantly in the community. This study assesses the outcomes of these clinics.Design This study used a prospective, longitudinal, participatory action research approach.Setting The study was conducted in primary care practices across Hampshire, UK.Participants Adults aged 16 years and above with poorly controlled asthma or COPD, as well as those with undifferentiated breathlessness not under specialist care, were included.Interventions Participants received care through the multidisciplinary, specialist-led MABC clinics.Primary and secondary outcome measures Primary outcomes included disease activity, quality of life and healthcare utilisation. Secondary outcomes encompassed clinic attendance, diagnostic changes, patient activation, participant and healthcare professional experiences and cost-effectiveness.Results A total of 441 participants from 11 general practitioner practices were recruited. Ninety-six per cent of participants would recommend MABC clinics. MABC assessments led to diagnosis changes for 64 (17%) participants with asthma and COPD and treatment adjustments for 252 participants (57%). Sanders parte de la pregunta de si el creciente interés por las discusiones éticas desde la disciplina de los estudios literarios tiene relación con las discusiones sobre ética que se desarrollan en la actualidad en el campo de la filosofía, e indaga si un diálogo interdisciplinario podría resultar útil a ambas disciplinas. El análisis de Sanders muestra de qué manera la discusión ética en los estudios literarios ha seguido un linaje particular. La preocupación por la alteridad, tema central a los estudios literarios dado el carácter de la literatura como “generadora de alteridades”, ha implicado el interés por autores tales como Michel Foucault, Emmanuel Lévinas, Jacques Derrida, Luce Irigaray, Giorgio Agamben y Alain Badiou. Entre tanto, la filosofía contemporánea se ha centrado más en una discusión ligada a la teoría política y la ética aplicada, por lo cual autores tales como Charles Taylor, John Rawls, Will Kymlicka, Iris Young y Seyla Benhabib han ocupado el centro de la discusión. الوصف: IMPORTANCE:. Factors associated with mortality in coronavirus disease 2019 patients on invasive mechanical ventilation are still not fully elucidated. OBJECTIVES:. To identify patient-level parameters, readily available at the bedside, associated with the risk of in-hospital mortality within 28 days from commencement of invasive mechanical ventilation or coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS:. Prospective observational cohort study by the global Coronavirus Disease 2019 Critical Care Consortium. Patients with laboratory-confirmed coronavirus disease 2019 requiring invasive mechanical ventilation from February 2, 2020, to May 15, 2021. MAIN OUTCOMES AND MEASURES:. Patient characteristics and clinical data were assessed upon ICU admission, the commencement of invasive mechanical ventilation and for 28 days thereafter. We primarily aimed to identify time-independent and time-dependent risk factors for 28-day invasive mechanical ventilation mortality. RESULTS:. One-thousand five-hundred eighty-seven patients were included in the survival analysis; 588 patients died in hospital within 28 days of commencing invasive mechanical ventilation (37%). Cox-regression analysis identified associations between the hazard of 28-day invasive mechanical ventilation mortality with age (hazard ratio, 1.26 per 10-yr increase in age; 95% CI, 1.16–1.37; p < 0.001), positive end-expiratory pressure upon commencement of invasive mechanical ventilation (hazard ratio, 0.81 per 5 cm H2O increase; 95% CI, 0.67–0.97; p = 0.02). Time-dependent parameters associated with 28-day invasive mechanical ventilation mortality were serum creatinine (hazard ratio, 1.28 per doubling; 95% CI, 1.15–1.41; p < 0.001), lactate (hazard ratio, 1.22 per doubling; 95% CI, 1.11–1.34; p < 0.001), Paco2 (hazard ratio, 1.63 per doubling; 95% CI, 1.19–2.25; p < 0.001), pH (hazard ratio, 0.89 per 0.1 increase; 95% CI, 0.8–14; p = 0.041), Pao2/Fio2 (hazard ratio, 0.58 per doubling; 95% CI, 0.52–0.66; p < 0.001), and mean arterial pressure (hazard ratio, 0.92 per 10 mm Hg increase; 95% CI, 0.88–0.97; p = 0.003). CONCLUSIONS AND RELEVANCE:. This international study suggests that in patients with coronavirus disease 2019 on invasive mechanical ventilation, older age and clinically relevant variables monitored at baseline or sequentially during the course of invasive mechanical ventilation are associated with 28-day invasive mechanical ventilation mortality hazard. Further investigation is warranted to validate any causative roles these parameters might play in influencing clinical outcomes.

الوصف: Carbon Capture and Sequestration (CCS) has been recognized as an important means of mitigating global climate change, but apart from several pilots, it has not yet been successfully implemented on the large scale needed to live up to the expectations as a mitigation method. In Netherlands, the option of CCS has been the subject of debate for a long time, as three unsuccessful projects – two onshore in Barendrecht and the Northern regions, and one offshore near the Port of Rotterdam – demonstrate. Nevertheless, CCS has been accorded an important place in the current Dutch climate policies, being expected to contribute up to 7 Megaton of CO2 reduction. This is reflected in a fresh crop of CCS project plans. For the most, these plans have a long way to go from the drawing board to actual operations due to the technical, economic, legal and societal challenges ahead. In this article we review the status and possibilities of CCS in Netherlands based on an analysis of existing literature in the relevant disciplines. First, a brief overview of the technology options for carbon capture and storage or utilization is given. This is followed by a detailed analysis of the governmental support for CCS, given the vital role that fit-for-purpose legal frameworks and policy instruments will play in CCS deployment. Technical, legal and policy uncertainties translates into factors inhibiting CCS investment and so the paper then presents a CCS investment project to illustrate how such risks affect the business case for CCS. Finally, bearing in mind that societal acceptance has proved to be a major barrier for CCS, both in Netherlands and elsewhere, the conditions that enhance public acceptance of CCS are examined. Our work shows that while CCS is technically a straightforward proposition, its deployment has historically been hindered by the lack of a sound business case and a compelling and stable socio-technical narrative. The main argument in favor of CCS today is that it offers a transition pathway for rapidly and massively reducing CO2 emissions beyond what could be accomplished by alternative methods like electrification and renewable fuels in near future. The introduction of new financial instruments, increased government support and an improvement in social engagement appear to have enhanced the prospects of CCS in Netherlands, but we feel it is premature to assume that this time everything is different.

الوصف: As the energy transition accelerates and renewable energy technologies become cost-competitive with fossil fuels in many countries, the availability of finance could become a bottleneck. Integrated assessment models (IAMs) and other macro-economic transition (MET) models typically do not feature detailed financial markets and do not sufficiently consider financing barriers and opportunities for the transition to carbon neutrality. While progress has been made in the representation of financial markets in macro-models since the financial crisis of 2008 the focus has been on financial (in)stability of the financial sector, not its ability to finance investment projects in the energy transition. Hence, a crucial gap remains, preventing macro model-based analysis of financing barriers and policy interventions that may accelerate the energy transition. In this article we review how state-of-the-art macro-economic models consider the financial sector. From this review we identify what elements are still missing to adequately model the financial dynamics and challenges for the energy transition specifically. Based on a discussion of relevant parts of the finance literature, we then propose four steps to improve the representation of finance in global IAMs and MET models more generally.

الوصف: Abstract Background Self-administration of medicines by patients whilst in hospital is being increasingly promoted despite little evidence to show the risks and benefits. Pain control after total knee replacement (TKR) is known to be poor. The aim of the study was to determine if patients operated on with a TKR who self-medicate their oral analgesics in the immediate post-operative period have better pain control than those who receive their pain control by nurse-led drug rounds (Treatment as Usual (TAU)). Methods A prospective, parallel design, open-label, randomised controlled trial comparing pain control in patient-directed self-management of pain (PaDSMaP) with nurse control of oral analgesia (TAU) after a TKR. Between July 2011 and March 2013, 144 self-medicating adults were recruited at a secondary care teaching hospital in the UK. TAU patients (n = 71) were given medications by a nurse after their TKR. PaDSMaP patients (n = 73) took oral medications for analgesia and co-morbidities after two 20 min training sessions reinforced with four booklets. Primary outcome was pain (100 mm visual analogue scale (VAS)) at 3 days following TKR surgery or at discharge (whichever came soonest). Seven patients did not undergo surgery for reasons unrelated to the study and were excluded from the intention-to-treat (ITT) analysis. Results ITT analysis did not detect any significant differences between the two groups’ pain scores. A per protocol (but underpowered) analysis of the 60% of patients able to self-medicate found reduced pain compared to the TAU group at day 3/discharge, (VAS -9.9 mm, 95% CI -18.7, − 1.1). One patient in the self-medicating group over-medicated but suffered no harm. Conclusion Self-medicating patients did not have better (lower) pain scores compared to the nurse-managed patients following TKR. This cohort of patients were elderly with multiple co-morbidities and may not be the ideal target group for self-medication. Trial Registration ISRCTN10868989. الوصف: Many of N.P. van Wyk Louw’s essays address the role of the intellectual. In the 1930s, Louw struggled to define a place for the intellectual in relation to the Afrikaner volk and its cultural movements and political parties. At the end of “Kultuurleiers sonder kultuur” (Cultural Leaders without Culture) (1939), Louw turns to the simile of the cave from book seven of Plato’s Republic. There the cave represents the city and its people, who are trapped in illusion. Plato’s “philosopher” escapes the dark chamber where the prisoners observe only shadows, gains enlightenment, and returns to open the eyes of his fellow inhabitants. Returning is the duty of the philosopher. How did N.P. van Wyk Louw imagine descending back into the “cave,” into the midst of the city, to be among the people of his country? One answer lies in a pair of unpublished fragments dating from the 1940s entitled “In die bus afgeluister” (Overheard on the Bus). In these fragments, Louw eavesdrops, as he takes the bus to and from work, on the conversations of people of Cape Town of various races. We get a slice of city life, and a sense of how Louw tried to embrace that life rather than isolate himself from it. These two urban sketches nevertheless show that the task of enlightening one’s fellow citizens proves more complicated than Louw expects because the intellectual is more deeply implicated in the illusory play of shadows than he imagines.
