الوصف: Background Postoperative delirium (POD) is an acute altered mental state commonly encountered after cardiac surgery. The pathophysiological mechanisms underlying POD remain unclear. We aimed to identify circulating proteins significantly altered after major cardiac surgery with cardiopulmonary bypass (CPB). We also aimed to enable inferences on associations with POD. Methods Serum and whole blood samples were collected before CPB ( n = 16 patients; n = 8 with POD) and again from the same patients on postoperative day 1. All patients were clinically evaluated for POD on postoperative days 1–3. An aptamer-based proteomics platform (SOMAscan) was used to quantify serum protein abundance in patients with POD compared with non-POD controls. We also performed a lipopolysaccharide (LPS)-based in vitro functional analysis (TruCulture) on whole blood samples from patients with POD and non-POD controls to approximate surgical stress. Cytokine levels were determined using a Luminex immunoassay. Results Cardiac surgery with CPB resulted in a significant ( p adj < 0.01) change in 48.8% (637 out of 1,305) of proteins detected by SOMAscan. Gene set enrichment showed that the most impacted biological processes involved myeloid cell activation. Specifically, activation and degranulation of neutrophils were the top five highest-scoring processes. Pathway analyses with the Kyoto Encyclopedia of Genes and Genomes (KEGG) showed that metabolic enzymes, particularly those of glycolysis, were elevated in serum concentration after surgery. Several proteins were significantly increased postoperatively in patients diagnosed with POD relative to the non-POD controls, with interleukin-6 (IL-6) showing the greatest fold-change. LPS stimulation of whole blood samples confirmed these findings. Linear regression analysis showed a highly significant correlation between Confusion Assessment Method (CAM) scores and CPB-mediated changes in cGMP-inhibited 3′,5′-cyclic phosphodiesterase A (PDE3A). Conclusions Cardiac surgery with CPB resulted ...

الوصف: This study explores how junior and mid-tier resources companies operating in the world's most impoverished countries understand and practice corporate social responsibility (CSR) , and its relationship to their assumed social licence to operate (SLO) . The study drew on emergent research that finds smaller companies, which are less subject to scrutiny by external stakeholders such as NGOs, media and investors, understand and practice CSR differently to the multinational companies which have dominated extant research. The study found that CSR meaning and practice in this large, but little researched, group of companies was highly context-based and community focused, in marked contrast to the strategies and practices of larger resources companies operating in developing nations. Importantly, the study also revealed striking contradictions and ambiguities between participants’ CSR actions and accountability and their assumed SLO.

مستخلص: This report examines the second phase of a longitudinal impact study of the Sixth Cycle Teacher Corps. Phase 1 focused on the relationship of intern background and Teacher Corps Program characteristics to intern exit characteristics. Its goals were to: (a) contrast teacher performance and pupil gains of graduates with those of control group teachers; (b) analyze the relationship of teacher background, teacher education program, teacher performance, and pupil performance for the graduates; and (c) assess the effects of professional support which graduates and controls receive. The 84 subjects taught grades 2-6 in self-contained classrooms. Data about teacher and pupil performance were obtained through a classroom observation guide, teacher questionnaire, data, standardized reading test, and standardized self-concept scale. Graduates were superior to controls in developing ethically relevant curricula, using community resources and initiating contact with parents, and developing positive attitudes about reading development and the causes of poverty in society. Graduates facilitated greater self-concept development in pupils, but there was no significant difference in reading gains between the two groups. The consistent pattern of relationship between pupil variables and Teacher Program characteristics suggests that teacher training makes a difference on pupil behavior and on related teacher performance. مستخلص: This document further analyzes a two-year Teacher Corps study which was conducted by Pacific Training and Technical Assistance Corporation for the U.S. Office of Education. The results of the first and second years of study have been reported elsewhere. This volume presents the results of a further analysis of data collected in the first year of the study dealing with the relationship between intern background characteristics and Teacher Corps program characteristics, and the teaching skills and attitudes of interns at the end of their two years of training. The goals of this study are: (a) to provide information that could serve as a bais for redirecting efforts to recruit interns, selecting new Teacher Corps projects, or providing technical assistance to projects; (b) to provide information that could be used to identify special strengths or weaknesses at any one project site and, consequently, to suggest the need for special recruiting procedures for interns or for technical assistance efforts; (c) to further explore the relationship between teacher background, Teacher Corps program characteristics as related to the teaching skills, and attitudes of interns at the end of their two years of training; and (d) to answer management-oriented questions regarding the development of effective Teacher Corps projects and the assessment of the quality and effectiveness of the projects. (A large section of the document is devoted to tables illustrating the interns' background and Teacher Corps program characteristics.) (JS)
