الوصف: Abstract Detecting and attributing sea‐level rise over different spatiotemporal scales is essential for low‐lying and highly populated coastal regions. Using the Detection and Attribution Model Intercomparison Project (DAMIP) from the Coupled Model Intercomparison Project Phase 6, we evaluate the role of anthropogenic forcing in sea‐level change in the historical (1950–2014) period in the Indo‐Pacific warm pool region. We use three models that have at least 10 ensemble members, corresponding to different DAMIP simulations. We determined the changes in regional sea level from both natural and anthropogenic forcings. Our results demonstrate: (a) the emergence of an anthropogenic footprint on regional sterodynamic sea‐level change has a large spatiotemporal diversity over the Indo‐Pacific warm pool region with the earliest emergence in the western Indian Ocean; (b) a significant rise in dynamic sea level (DSL) (up to 25 mm) and thermosteric (up to 40 mm) sea level over the western Indian Ocean due to greenhouse gas forcing; (c) a positive Indian Ocean Dipole‐like pattern in the DSL changes over the tropical Indian Ocean; (d) a significant increase in the halosteric contribution to sea‐level rise in the Indo‐Pacific warm pool region, and (e) a pronounced rise of manometric sea level (up to 20 mm) over shallow oceans and coastal regions in recent decades. These results provide a comprehensive spatiotemporal analysis of the attribution of anthropogenic factors to sea‐level changes in the Indo‐Pacific warm pool region.

الوصف: Background: Access to services remains the biggest barrier to helping the most vulnerable in the South African Stuttering Community. This novel stuttering therapy, harnessing an unconscious link between eye and tongue movement, may provide a new therapeutic approach, easily communicated and deliverable online. Objectives: This study provides both objective and subjective assessments of the feasibility of this intervention. Assessment tools holistically address all components of stuttering in line with comprehensive treatment approaches: core behaviours, secondary behaviours, anticipation and reactions. Method: On receipt of ethical approval, this single-subject case design recruited one adult (21-year-old) male with a developmental stutter (DS). The participant gave informed consent and completed four scheduled assessments: baseline, after 5-week training, 3 months post-intervention and 24 months post-completion. The study used objective assessment tools: Stuttering Severity Instrument-4 (SSI-4); Subjective-assessment tools: SSI-4 clinical use self-report tool (CUSR); Overall Assessment of Speaker’s Experience of Stuttering (OASES-A); Premonitory Awareness in Stuttering (PAiS) and Self-Report Stuttering Severity* (SRSS) (*final assessment). Results: The participant’s scores improved across all assessment measures, which may reflect a holistic improvement. The participant reported that the tool was very useful. There were no negative consequences. Conclusion: This case report indicates that this innovative treatment may be feasible. No adverse effects were experienced, and the treatment only benefited the participant. The results justify the design of a pilot randomised feasibility clinical trial. Contribution: The results indicate that this is a needed breakthrough in stuttering therapy as the instructions can be easily translated into any language. It can also be delivered remotely reducing accessibility barriers.

An important step is to search large databases of materials properties to find examples that resemble known carbon capture agents or electrolytes and then test them for effectiveness. This paper describes novel computational tools for accelerated discovery of solvents, nano-porous materials, and electrolytes. These tools have produced interesting results so far, such as the identification of a relatively isolated location in amine configuration space for the solvents with known carbon capture use, and the demonstration of an end-to-end simulation and process model for carbon capture in MOFs.

الوصف: As the planet warms due to the accumulation of anthropogenic CO2 in the atmosphere, the interaction of surface ocean carbonate chemistry and the radiative forcing of atmospheric CO2 leads to the global ocean sequestering heat and carbon in a ratio that is nearly constant in time. This ratio has been approximated as globally uniform, enabling the intimately linked patterns of ocean heat and carbon uptake to be derived. Patterns of ocean salinity also change as the Earth system warms due to hydrological cycle intensification and perturbations to air–sea freshwater fluxes. Local temperature and salinity change in the ocean may result from perturbed air–sea fluxes of heat and fresh water (excess temperature, salinity) or from reorganisation of the preindustrial temperature and salinity fields (redistributed temperature, salinity), which are largely due to circulation changes. Here, we present a novel method in which the redistribution of preindustrial carbon is diagnosed and the redistribution of temperature and salinity is estimated using only local spatial information. We demonstrate this technique in the NEMO ocean general circulation model (OGCM) coupled to the MEDUSA-2 biogeochemistry model under an RCP8.5 scenario over 1860–2099. The excess changes (difference between total and redistributed property changes) are thus calculated. We demonstrate that a global ratio between excess heat and temperature is largely appropriate regionally with key regional differences consistent with reduced efficiency in the transport of carbon through the mixed layer base at high latitudes. On centennial timescales, excess heat increases everywhere, with the North Atlantic being a key site of excess heat uptake over the 21st century, accounting for 25 % of the total. Excess salinity meanwhile increases in the Atlantic but is generally negative in other basins, consistent with increasing atmospheric transport of fresh water out of the Atlantic. In the North Atlantic, changes in the inventory of excess salinity are detectable in the late 19th century, whereas increases in the inventory of excess heat do not become significant until the early 21st century. This is consistent with previous studies which find salinification of the subtropical North Atlantic to be an early fingerprint of anthropogenic climate change. Over the full simulation, we also find the imprint of Atlantic meridional overturning circulation (AMOC) slowdown through significant redistribution of heat away from the North Atlantic and of salinity to the South Atlantic. Globally, temperature change at 2000 m is accounted for by both redistributed and excess heat, but for salinity the excess component accounts for the majority of changes at the surface and at depth. This indicates that the circulation variability contributes significantly less to changes in ocean salinity than to heat content. By the end of the simulation excess heat is the largest contribution to density change and steric sea level rise, while excess salinity greatly reduces spatial variability in steric sea level rise through density compensation of excess temperature patterns, particularly in the Atlantic. In the Atlantic, redistribution of the preindustrial heat and salinity fields also produces generally compensating changes in sea level, though this compensation is less clear elsewhere. The regional strength of excess heat and salinity signals grows through the model run in response to the evolving forcing. In addition, the regional strength of the redistributed temperature and salinity signals also grows, indicating increasing circulation variability or systematic circulation change on timescales of at least the model run.

الوصف: The ocean takes up 93 % of the excess heat in the climate system and approximately a quarter of the anthropogenic carbon via air–sea fluxes. Ocean ventilation and subduction are key processes that regulate the transport of water (and associated properties) from the surface mixed layer, which is in contact with the atmosphere, to the ocean's interior, which is isolated from the atmosphere for a timescale set by the large-scale circulation. Utilising numerical simulations with an ocean–sea-ice model using the NEMO (Nucleus for European Modelling of the Ocean) framework, we assess where the ocean subducts water and, thus, takes up properties from the atmosphere; how ocean currents transport and redistribute these properties over time; and how, where, and when these properties are ventilated. Here, the strength and patterns of the net uptake of water and associated properties are analysed by including simulated seawater vintage dyes that are passive tracers released annually into the ocean surface layers between 1958 and 2017. The dyes' distribution is shown to capture years of strong and weak convection at deep and mode water formation sites in both hemispheres, especially when compared to observations in the North Atlantic subpolar gyre. Using this approach, relevant to any passive tracer in the ocean, we can evaluate the regional and depth distribution of the tracers, and determine their variability on interannual to multidecadal timescales. We highlight the key role of variations in the subduction rate driven by changes in surface atmospheric forcing in setting the different sizes of the long-term inventory of the dyes released in different years and the evolution of their distribution. This suggests forecasting potential for determining how the distribution of passive tracers will evolve, from having prior knowledge of mixed-layer properties, with implications for the uptake and storage of anthropogenic heat and carbon in the ocean.

الوصف: Background In the United States, nearly 25,000 patients annually undergo percutaneous ventriculostomy for the management of increased intracranial pressure with little consensus on extraventricular drain management. To characterize relationships between external ventricular drain management, permanent ventriculoperitoneal shunt placement, and hospital disposition, we hypothesized that patients requiring extended drainage would have greater association with ventriculoperitoneal shunt placement and unfavorable disposition. Methods Adult patients admitted to the Duke University Hospital Neuroscience Intensive Care Unit between 2008 and 2010 with extraventricular drains were analyzed. A total of 115 patient encounters were assessed to determine relative impact of age, sex, days of extraventricular placement, weaning attempts, cerebrospinal fluid drainage volumes, Glasgow Coma Scale, and physician’s experience on disposition at discharge and ventriculoperitoneal shunt placement. Univariate logistic regression was first used to test the effect of each variable on the outcome, followed by backward selection to determine a final multivariable logistic regression. Variables in the final model meeting p < 0.05 were declared as significant factors for the outcome. Results Increased extraventricular drain duration (odds ratio [OR] = 1.17, confidence interval [CI] = 1.05–1.30, p = 0.0049) was associated with ventriculoperitoneal shunt placement, while older age (OR = 1.05, CI = 1.02–1.08, p = 0.0027) and less physician extraventricular drain management experience (OR = 4.04, CI = 1.67–9.79, p = 0.0020) were associated with unfavorable disposition. Conclusion In a small cohort, exploratory analyses demonstrate potentially modifiable factors are associated with important clinical outcomes. These findings warrant further study to refine how such factors affect patient outcomes.

الوصف: Background: Postoperative acute kidney injury (AKI) is a common complication of major gastrointestinal surgery with an impact on short- and long-term survival. No validated system for risk stratification exists for this patient group. This study aimed to validate externally a prognostic model for AKI after major gastrointestinal surgery in two multicentre cohort studies. Methods: The Outcomes After Kidney injury in Surgery (OAKS) prognostic model was developed to predict risk of AKI in the 7 days after surgery using six routine datapoints (age, sex, ASA grade, preoperative estimated glomerular filtration rate, planned open surgery and preoperative use of either an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker). Validation was performed within two independent cohorts: a prospective multicentre, international study ('IMAGINE') of patients undergoing elective colorectal surgery (2018); and a retrospective regional cohort study ('Tayside') in major abdominal surgery (2011-2015). Multivariable logistic regression was used to predict risk of AKI, with multiple imputation used to account for data missing at random. Prognostic accuracy was assessed for patients at high risk (greater than 20 per cent) of postoperative AKI. Results: In the validation cohorts, 12.9 per cent of patients (661 of 5106) in IMAGINE and 14.7 per cent (106 of 719 patients) in Tayside developed 7-day postoperative AKI. Using the OAKS model, 558 patients (9.6 per cent) were classified as high risk. Less than 10 per cent of patients classified as low-risk developed AKI in either cohort (negative predictive value greater than 0.9). الوصف: Abstract Background Multimodal perioperative analgesia including acetaminophen is recommended by current guidelines. The comparative efficacy of intravenous vs oral acetaminophen in sinus surgery is unknown. We aimed to determine whether intravenous or oral acetaminophen results in superior postoperative analgesia following sinus surgery. Methods This was a prospective randomized trial with blinded endpoint assessments conducted at a single large academic medical center. Subjects undergoing functional endoscopic sinus surgery were randomized to intravenous vs oral acetaminophen in addition to standard anesthetic and surgical care. The primary outcome was visual analogue scale pain score at 1 hour postoperatively. Results One hundred and ten adult patients were randomized; 9 were excluded from the data analysis. Fifty patients were assigned to intravenous acetaminophen and 51 to oral acetaminophen. Postoperative pain scores at 1 hour (primary endpoint) were not significantly different between the intravenous and oral acetaminophen groups. Similarly, there was no significant difference in pain scores at 24 hours postoperatively. Finally, there was no significant difference in postoperative opioid usage in the postanesthesia care unit or over the first 24 hours postoperatively. Conclusions This is the first comparative efficacy trial of oral vs intravenous acetaminophen in sinus surgery. There was no significant difference in pain scores at 1 or 24 hours postoperatively, and no difference in postoperative opioid use. Intravenous acetaminophen offers no apparent advantage over oral acetaminophen in patients undergoing sinus surgery. Level of Evidence 1b
