الوصف: Purpose: We investigated whether spectral analysis with fast Fourier transform (FFT) of inspiratory lung sounds is useful in the diagnosis and evaluation of the severity of interstitial pneumonia (IP). Subjects and Methods: The study population included 10 healthy volunteers (healthy group) and 21 patients with IP (IP group). We generated inspiratory averaged linear intensities using FFT and determined frequency at maximum sound intensity (Fmax), and quartile frequencies (f25, f50, and f75), compared these values between the groups, generated receiver operating characteristic curves to compare the detectability of IP between the indices and auscultation in all cases, and tested for the correlation of these indices with pulmonary function tests and the fibrosis scores from high-resolution computed tomography images assessed by 3 observers. Results: Both f50 and f75 were significantly higher in the IP group, but their ability to detect IP was inferior to that of auscultation. They had negative correlations with percent vital capacity and had positive correlations with the fibrosis scores calculated by the 3 different observers. Discussion: These results were considered to reflect the presence of fine crackles and alterations in pulmonary sound-conduction characteristics caused by IP and indicate that spectral analysis of lung sounds is useful in the diagnosis and evaluation of the severity of IP, although future study is necessary to improve its utility.

الوصف: A 41-year-old woman with a history of epilepsy was referred for multiple nodular ground-glass opacities on a chest computed tomography (CT) scan. They were initially suspected of representing atypical adenomatous hyperplasia or well-differentiated adenocarcinoma. However, the subsequent brain CT and magnetic resonance imaging (MRI) scans revealed a coarse nodular calcification and cortical tubers. A subungual fibroma was also noted. Histological examination of a video-assisted thoracoscopic lung biopsy specimen disclosed multiple nodules of type II pneumocyte hyperplasia with septal thickening. Based on all of these findings taken together, a diagnosis of tuberous sclerosis complex with multifocal micronodular pneumocyte hyperplasia (MMPH) was made.

الوصف: The matrix metalloproteinases (MMPs) are likely to contribute to tumor cell invasion, metastasis and angiogenesis. Several MMP inhibitors have been developed, recently and their anti-tumor efficacy is being evaluated in clinical trials. FYK-1388 is a novel broad MMP inhibitor which blocks the activity of MMP-1, -2, -3, -7, -9, -13 and -14 (MT-MMP-1). It is especially effective against MMP-2 and -9 more so than other MMP inhibitors such as Marimastat, Ro 32-3555 and D-2163. Here, we investigated the anti-tumor efficacy of FYK-1388 using the human fibrosarcoma cell line HT-1080. These cells produced MMP-2 and -9, which FYK-1388 inhibited at a dose of 10(-8) M. FYK-1388 at 0.2 mg/mouse/day significantly suppressed tumor growth when given by s.c. injection for 22 days, experimental lung metastasis after 5 days s.c. injection and also suppressed tumor-induced angiogenesis in the dorsal air sac assay after 7 days s.c. injection. In the MTT assay, FYK-1388 had no effect on the in vitro growth of HT-1080 cells. These results suggest that FYK-1388 possesses anti-tumor efficacy as a result of inhibiting angiogenesis through the suppression of MMP-2 and -9 activity.
