الوصف: Objective Determine the pharmacokinetics (PK) and exposure–response of hydroxychloroquine (HCQ) and desethylhydroxychloroquine (DHCQ) in paediatric SLE (pSLE).Methods We conducted an exploratory phase 2, direct-to-family trial. Children enrolled in the Childhood Arthritis and Rheumatology Research Alliance (CARRA) Registry with a diagnosis of pSLE were eligible if they were receiving HCQ as standard of care for ≥3 months. Biological samples were collected at up to four visits over a 6-month period. At each visit, plasma was obtained to measure the concentrations of HCQ and DHCQ, as well as cytokines. HCQ and DHCQ plasma PK data were analysed using a population PK modelling approach.Results Twenty-five subjects provided a total of 88 plasma concentrations for PK analysis. There was a poor linear fit between HCQ concentrations and total body weight (R2=0.03). There was a decline in both interferon (IFN)-alpha and IFN-gamma with higher concentrations of HCQ and DHCQ. Volume of distribution for HCQ in plasma was higher in children compared with published values in adults (73 000 L vs 44 000 L), but clearance values in children were similar to adults.Conclusions We report the first population PK model for HCQ and DHCQ in children using data from a novel direct-to-family clinical trial. We observed high interindividual variability in HCQ PK and found that weight-based dosing for HCQ is poorly correlated with drug concentrations, suggesting the need to use therapeutic drug monitoring to individualise dosing. Furthermore, our results suggest that the current weight-based dosing paradigm for HCQ may result in suboptimal drug exposures, particularly for children with obesity. Accordingly, additional studies of HCQ are needed in pSLE to determine the optimal drug concentration and dosing to reduce disease activity and improve outcomes.Trial registration number NCT04358302.

الوصف: Introduction Direct-to-family clinical trials efficiently provide data while reducing the participation burden for children and their families. Although these trials can offer significant advantages over traditional clinical trials, the process of designing and implementing direct-to-family studies is poorly defined, especially in children with rheumatic disease. This paper provides lessons learnt from the design and implementation of a self-controlled, direct-to-family pilot trial aimed to evaluate the effects of a medication management device on adherence to hydroxychloroquine in paediatric SLE.Methods Several design features accommodate a direct-to-family approach. Participants meeting eligibility criteria from across the USA were identified a priori through a disease registry, and all outcome data are collected remotely. The primary outcome (medication adherence) is evaluated using electronic medication event-monitoring, plasma drug levels, patient questionnaires and pill counts. Secondary and exploratory endpoints include (1) lupus disease activity measured by a remote SLE Disease Activity Index examination and the Systemic Lupus Activity Questionnaire; and (2) hydroxychloroquine pharmacokinetics and pharmacodynamics. Recruitment of the initial target of 20 participants was achieved within 10 days. Due to initial recruitment success, enrolment was increased to 26 participants. Additional participants who were interested were placed on a waiting list in case of dropouts during the study.Discussion and dissemination Direct-to-family trials offer several advantages but present unique challenges. Lessons learnt from the protocol development, design, and implementation of this trial will inform future direct-to-family trials for children and adults with rheumatic diseases. Additionally, the data collected remotely in this trial will provide critical information regarding the accuracy of teleresearch in lupus, the impact of adherence to hydroxychloroquine on disease activity and a pharmacokinetic analysis to inform paediatric-specific dosing of hydroxychloroquine.Trial registration number ClinicalTrials.gov Registry (NCT04358302).

الوصف: We present a repeatable workflow that produced a comprehensive wildfire and timber harvesting database for Ontario (1972–2021) that accommodates annual updates after each new fire season. Training sites for classification are identified at the individual scene-level to avoid spectral variations introduced by time and distance. ISODATA classification on the training data produces clusters that are modelled by a smooth polynomial function to identify a local minimum point along the classification clusters that distinguishes disturbances from non-disturbances. This threshold is then applied to map disturbances on independent scenes of Landsat MSS, TM, ETM+, or OLI imagery. Results are aggregated to 1.44 ha cells and converted to points for dissemination; we do not map explicit boundaries to avoid issues of context- and scale-dependence, or the realities of transitional boundaries. Disturbance points are cross-referenced through time, ensuring that the earliest date for each disturbance is recorded. Disturbance points are intersected with other harvesting and fire databases to assess their ensemble confidence which is attached to each mapped location. We present summaries of Ontario’s boreal disturbance mapping with respect to levels of assessed confidence.

الوصف: BackgroundIn contrast to infection or mechanical issues joint replacement failure following inflammatory adverse reactions is poorly understood.ObjectiveTo assess the association of IL-1β polymorphisms and history of allergy with aseptic non-mechanical complications following arthroplasty.MethodsIn 102 patients with aseptic non-mechanically caused symptomatic knee or hip arthroplasty (SA) and 93 patients with asymptomatic arthroplasty (AA) questionnaire-based history, patch test with at least standard series, lymphocyte transformation test (LTT) with nickel, cobalt and chromium and interleukin-1 polymorphism analysis were done. Three polymorphisms of the IL1B gene [IL-1b -3954 (rs1143634), IL-1b -511 (rs16944) and IL-1b -31 (rs1143627)] and one polymorphism of the IL1RN gene [IL1RN intron 2, variable number of tandem repeats, VNTR (rs2234663)] were assessed by PCR and gel electrophoresis.ResultsWe found no significant difference in smoking history and atopy but 25% versus 10% of self-reported metal allergy in SA versus AA; the patch test (respective, LTT) for metal sensitivity was more often positive in SA patients. The allele 498 bp of the IL1RN polymorphism occurred significantly more often in the SA group (37% versus 11%; p < 0.0001). Upon additional presence of atopy, the difference was even greater (60% vs 10%) (p < 0.000001). There was no association of IL-1 polymorphisms with metal allergy.ConclusionThe IL1RN VNTR allele 498 bp was strongly associated with SA. In patients with a history of atopy, presence of the IL1RN VNTR allele 498 bp led to a four-fold higher SA prevalence compared to patients without this allele.

This work scrutinizes binary classes mapped onto regular grids and counts the relative frequencies of all first-order configuration components and then converts these measurements into empirical probabilities of occurrence for either of the two landscape classes. The approach takes into consideration configuration explicitly and composition implicitly (in a common framework), while the construction of a frequency distribution provides a generic model of landscape structure that can be used to simulate structurally similar landscapes or to compare divergence from other landscapes. The technique is first tested on simulated data to characterize a continuum of landscapes across a range of spatial autocorrelations and relative compositions. Subsequent assessments of boundary prominence are explored, where outcomes are known a priori, to demonstrate the utility of this novel method. For a binary map on a regular grid, there are 32 possible configurations of first-order orthogonal neighbours. The goal is to develop a workflow that permits patterns to be characterized in this way and to offer an approach that identifies how relatively divergent observed patterns are, using the well-known Kullback–Leibler divergence.

الوصف: A new method for measuring the porosity of individual 2D raster patches in a GIS for characterizing the combined complexity of a shape’s edge in conjunction with its internal perforations is developed. The method is centered on comparing the number of cellular edge–edge joins relative to the theoretical maximum number of similar joins possible given a set number of cells comprising a landscape patch. As this porosity (Φ) increases, the patch (or shape) can be viewed as deviating from a maximally compact form, comprising higher edge complexity and internal heterogeneity (inclusion of perforations). The approach is useful for characterizing shapes for which a simple perimeter- or area-based metric misses the internal complexity and where the porosity of the patch may provide insight into spatial processes leading to the development of the landscape fabric. I present theoretical results to illustrate the mechanics of the approach and a small case study of boreal wildfire residual vegetation patches in Ontario, where real resulting wildfire process-driven landscape patches are assessed for their porosity at five spatial resolutions. The results indicate that naturally occurring and unsuppressed boreal wildfires in the study area typically produce residual vegetation patches with an average porosity of 17.6%, although this value varies slightly with the spatial resolution of the data representation.

الوصف: Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P < 0.001). Diabetes and coronary artery disease (CAD) were prevalent in 36.2% and 28.1% of patients as compared with global prevalence of 22.2% and 21.6%, respectively (P < 0.001 for both). Antiplatelet therapy was the most common antithrombotic treatment in India. With increasing stroke risk, however, patients were more likely to receive oral anticoagulant therapy [mainly vitamin K antagonist (VKA)], but average international normalized ratio (INR) was lower among Indian patients [median INR value 1.6 (interquartile range {IQR}: 1.3–2.3) versus 2.3 (IQR 1.8–2.8) (P < 0.001)]. Compared with other countries, patients from India had markedly higher rates of all-cause mortality [7.68 per 100 person-years (95% confidence interval 6.32–9.35) vs 4.34 (4.16–4.53), P < 0.0001], while rates of stroke/systemic embolism and major bleeding were lower after 1 year of follow-up. Conclusion: Compared to previously published registries from India, the GARFIELD-AF registry describes clinical profiles and outcomes in Indian patients with AF of a different etiology. الوصف: Reduced rainfall in tropical regions decreases the availability of food resources for overwintering songbirds, with negative impacts on their body condition. Increasingly dry conditions at tropical wintering sites as a result of climate change may impact the ability of temperate-breeding songbirds to prepare for and execute their spring migration. We studied the carryover effects of natural climatic fluctuations created by the El Niño–Southern Oscillation (ENSO) in tropical overwintering areas on 7 long-distance migratory songbirds at a Canadian stopover site. We used the Normalized Difference Vegetation Index as a proxy for tropical habitat productivity and resource availability and a 34-year bird banding dataset from Long Point, Ontario, Canada to assess migration timing and stopover body condition. To increase our ability to detect potential carry-over effects, we employed a novel approach of using recent migratory connectivity studies to restrict the wintering ranges to areas most likely used by individuals passing through southern Ontario. Using linear models, we found that overwinter habitat productivity was significantly negatively influenced by dry ENSO events in the overwintering ranges in 3 of 7 species, with a fourth near-significant. Subregional differences in the effect of ENSO on precipitation patterns may explain why we did not detect a consistent effect of ENSO on overwinter habitat productivity for all species. Despite restricting the wintering range and using a robust dataset for species with diverse life histories, we detected only weak and often conflicting evidence of population-level carry-over effects from dry ENSO events and overwinter habitat productivity. الوصف: Recent research into improving the effectiveness of forest inventory management using airborne LiDAR data has focused on developing advanced theories in data analytics. Furthermore, supervised learning as a predictive model for classifying tree genera (and species, where possible) has been gaining popularity in order to minimize this labor-intensive task. However, bottlenecks remain that hinder the immediate adoption of supervised learning methods. With supervised classification, training samples are required for learning the parameters that govern the performance of a classifier, yet the selection of training data is often subjective and the quality of such samples is critically important. For LiDAR scanning in forest environments, the quantification of data quality is somewhat abstract, normally referring to some metric related to the completeness of individual tree crowns; however, this is not an issue that has received much attention in the literature. Intuitively the choice of training samples having varying quality will affect classification accuracy. In this paper a Diversity Index (DI) is proposed that characterizes the diversity of data quality (Qi) among selected training samples required for constructing a classification model of tree genera. The training sample is diversified in terms of data quality as opposed to the number of samples per class. The diversified training sample allows the classifier to better learn the positive and negative instances and; therefore; has a higher classification accuracy in discriminating the “unknown” class samples from the “known” samples. Our algorithm is implemented within the Random Forests base classifiers with six derived geometric features from LiDAR data.
