    الوصف: Introduction: Normal-pressure hydrocephalus (NPH) is a neurological condition characterized by the dilation of the cerebral ventricles in the absence of a significant increase in intracranial pressure. This disease presents with progressive symptoms, including gait disorders, cognitive impairment, and bladder control alterations. Objective: To detail the main diagnostic and treatment methods for Hakim Adams Syndrome. Methodology: An exhaustive literature search was conducted in specialized databases such as PubMed, Scopus, and Web of Science. Inclusion criteria were applied to select clinical studies and literature reviews published in the last five years. Results: The most common symptoms of normal-pressure hydrocephalus included gait disorders, cognitive impairment, urinary incontinence, and altered mental state. Imaging techniques like magnetic resonance imaging and positron emission tomography proved to be useful for early and accurate diagnosis of normal-pressure hydrocephalus. Ventriculoperitoneal shunt surgery emerged as an effective therapeutic option to improve function and quality of life in affected patients. Conclusions: Normal-pressure hydrocephalus poses a diagnostic challenge due to its nonspecific symptoms. The use of advanced imaging techniques and clinical evaluation scales is crucial for accurate diagnosis. Ventriculoperitoneal shunt surgery presents as a promising therapeutic option. ; Introducción: La hidrocefalia de presión normal (HPN) es una condición neurológica caracterizada por la dilatación de los ventrículos cerebrales en ausencia de un aumento significativo de la presión intracraneal. Ludwika Rydygiera w Bydgoszczy, Uniwersytet Mikołaja Kopernika w Toruniu Streszczenie Wodogłowie normotensyjne (normociśnieniowe) to postać wodogłowia komunikującego (hydrocephalus communicans), przebiegającego z prawidłowym ciśnieniem płynu mózgowo - rdzeniowego z okresowymi zwyżkami. Opisane zostało po raz pierwszy przez Solomona Hakima w 1965 roku (zespół Hakima-Adamsa). Przyczyny zespołu Hakima-Adamsa upatruje się w zaburzeniu wchłaniania płynu mózgowo - rdzeniowego. Wystąpienie choroby czasem jest poprzedzane urazem głowy. Wodogłowie normotensyjne cechuje triada objawów (tzw. triada Hakima). Stosuje się leczenie operacyjne – implantację zastawki niskociśnieniowej z wytworzeniem połączenia komorowo - otrzewnowego. Autorzy przedstawiają opis przypadku klinicznego 61-letniego pacjenta leczonego operacyjnie w Klinice Neurochirurgii, Neurotraumatologii i Neurochirurgii Dziecięcej Szpitala Uniwersyteckiego nr 1 im. dr A. Jurasza w Bydgoszczy z rozpoznanym w badaniu obrazowym wodogłowiem normociśnieniowym oraz torbielą pajęczynówki w okolicy nadsiodłowej. Abstract Normal pressure hydrocephalus (NPH) is a form of communicating hydrocephalus (hydrocephalus communicans) that runs with normal pressure of cerebrospinal fluid with periodic advances. First described by Solomon Hakim in 1965 (Hakim-Adams syndrome). Hakim-Adams syndrome causes of the disorder is seen in the absorption of cerebrospinal fluid. The disease is sometimes preceded by a head injury. NPH is characterized by a triad of symptoms (Hakim’s triad). Apply surgery - implantation of the valve low pressure to form a combination kiln-peritoneal. The authors present a case of 61-year-old patient operating treated in the Department of Neurosurgery, Neurotraumatology and Pediatric Neurosurgery, Dr Antoni Jurasz University Hospital No.1 in Bydgoszcz imaging test diagnosed with normal pressure hydrocephalus and suprasellar arachnoid cysts.     الوصف: Idiopathic normal pressure hydrocephalus (Hakim-Adams syndrome, iNPH) is a chronic neurodegenerative disease of elderly associated with ventricular expansion, and characterized by gait disturbance, mental deterioration, and urinary incontinence. It becomes more important with increasing of elderly part of population. Important feature of iNPH is it’s reversibility in case of early surgical treatment. One of the main problem is the differential diagnosis between iNPH and other neurodegenerative diseases and detection of patients which would be most benefit with shunting. In this purpose a number of radiological symptoms and signs have been proposed. Assessment of MR symptoms of iNPH increases the accuracy of diagnosis and allows in some cases to avoid the necessity of invasive diagnostic procedures (e.g. tap-test, infusion test, external lumbar drainage, ICP monitoring etc.) with lower risk of complications, less duration of treatment and minimizing negative personal experience. At the same time, assessing of most sensitive iNPH radiological symptoms allows to evolve candidates for shunting at prehospital stage and reduce the duration of treatment.Background and purpose: systematization and synthesis of data on radiological symptoms of iNPH in order to identify most simple and at the same time significant diagnostic and prognostic criteria.Matherials and methods. The search was performed in PubMed, Google Scholar and Cochrane Library by key words «normal pressure hydrocephalus», «neuroimaging», «magnetic resonance imaging», «computed tomography, radiological symptoms». In addition, data from references were used.Results. Most specific and sensitive radiological criteria for diagnosis of iNPH are following: Evans index, DESH, temporal horns dilation, callosal angle, periventricular edema, perivascular spaces dilation and local sulci dilation.     الوصف: We report the case of a 72 years old patient presenting in our ICU with persistent postoperative coma in a context of recent unexplored neurological dysfunction. Detailed medical history taking from the patient’s family revealed he recently suffered from gait instability, urinary incontinence and slight cognitive impairment. These constituted the clinical triad of normal pressure hydrocephalus syndrome.The presence of normal cerebrospinal fluid pressure and distinctive radiological findings confirmed the diagnosis of normal pressure hydrocephalus or Hakims-Adams syndrome.After cerebrospinal fluid volume soustraction, the patient recovered a normal level of consciousness and was successfully weaned from mechanical ventilation. Normal pressure hydrocephalus should be included in the differential diagnosis of delayed postoperative arousal, especially in the elderly.

At the same time, assessing of most sensitive iNPH radiological symptoms allows to evolve candidates for shunting at prehospital stage and reduce the duration of treatment. Background and purpose: systematization and synthesis of data on radiological symptoms of iNPH in order to identify most simple and at the same time significant diagnostic and prognostic criteria. Matherials and methods. The search was performed in PubMed, Google Scholar and Cochrane Library by key words «normal pressure hydrocephalus», «neuroimaging», «magnetic resonance imaging», «computed tomography, radiological symptoms». In addition, data from references were used. Results. Most specific and sensitive radiological criteria for diagnosis of iNPH are following: Evans index, DESH, temporal horns dilation, callosal angle, periventricular edema, perivascular spaces dilation and local sulci dilation.     الوصف: The review presents the modern interpretation of the term «normal pressure hydrocephalus», data on the etiology, pathogenesis, clinical manifestations, diagnosis. The approaches to neurosurgical and conservative treatment are described. A clinical observation of verified normal pressure hydrocephalus (Hakim-Adams syndrome) is presented, its feature was the young age of a patient, favorable clinical and neuroradiological course of the disease after neurosurgical and medical treatment. According to the literature data, this pathology is most common in the elderly and may be a cause of dementia, walking disorders and pelvic organ function. The work proved the importance of further study of normal pressure hydrocephalus in clinical practice.
    В обзоре отражены современная трактовка термина «нормотензивная гидроцефалия», данные об этиологии, патогенезе, особенностях клинических проявлений, диагностики. Описаны подходы нейрохирургического и консервативного лечения. Представлено личное наблюдение клинического случая верифицированной нормотензивной гидроцефалии (синдрома Хакима — Адамса), особенностью которого были молодой возраст пациента, благоприятное клиническое и нейрорадиологическое течение заболевания после нейрохирургического и медикаментозного лечения. Согласно литературным данным, эта патология чаще всего встречается у лиц пожилого возраста и может быть причиной деменции, нарушения ходьбы и функции тазовых органов. В работе обоснована важность дальнейшего изучения нормотензивной гидроцефалии в клинической практике.
    В огляді відображено сучасне трактування терміну «нормотензивна гідроцефалія», дані про етіологію, патогенез, особливості клінічних проявів, діагностики. Описано підходи нейрохірургічного та консервативного лікування. Наведено особисте спостереження клінічного випадку верифікованої нормотензивної гідроцефалії (синдрому Хакіма — Адамса), особливістю якого були молодий вік пацієнта, сприятливий клінічний і нейрорадіологічний перебіг захворювання після нейрохірургічного та медикаментозного лікування. Згідно з літературними даними, ця патологія найчастіше зустрічається в осіб похилого віку та може бути причиною деменції, порушення ходьби та функції тазових органів. У роботі обґрунтовано важливість подальшого вивчення нормотензивної гідроцефалії в клінічній практиці.
