    الوصف: Abstract Background As a novel regional analgesic technique, ultrasound-guided pericapsular nerve group (PENG) block has some potential advantages, and we designed a randomized clinical trial (RCT) to investigate whether the ultrasound-guided PENG block combined with general anesthesia can better reduce stress response, maintain intraoperative hemodynamic stability, and reduce postoperative analgesia in elderly hip arthroplasty compared with ultrasound-guided suprainguinal fascia iliaca block (SIFIB) combined with general anesthesia. Methods Seventy-four subjects were enrolled over an 8-month period (20 April 2023 to 31 December 2023). All patients were divided into the test group (group P) and the control group (group S) using the envelope as the randomization method. The test group was treated with preoperative ultrasound-guided PENG block analgesia combined with general anesthesia and the control group was treated with preoperative ultrasound-guided SIFIB analgesia combined with general anesthesia. The primary outcome selected was the patient Visual Analogue Scale (VAS) score at 12 h postoperatively.     الوصف: Abstract While the monolayer sheet is well-established as a Mott-insulator with a finite energy gap, the insulating nature of bulk 1T-TaS2 crystals remains ambiguous due to their varying dimensionalities and alterable interlayer coupling. In this study, we present a unique approach to unlock the intertwined two-dimensional Mott-insulator and three-dimensional band-insulator states in bulk 1T-TaS2 crystals by structuring a laddering stack along the out-of-plane direction. Through modulating the interlayer coupling, the insulating nature can be switched between band-insulator and Mott-insulator mechanisms. Our findings demonstrate the duality of insulating nature in 1T-TaS2 crystals. By manipulating the translational degree of freedom in layered crystals, our discovery presents a promising strategy for exploring fascinating physics, independent of their dimensionality, thereby offering a “three-dimensional” control for the era of slidetronics.

    الوصف: ObjectivesTo identify clinical characteristics and risk factors for pulmonary involvements in asymptomatic and mildly symptomatic patients infected with SARS-CoV-2 Omicron variant by chest imaging analysis.MethodsDetailed data and chest computed tomography (CT) imaging features were retrospectively analyzed from asymptomatic and mildly symptomatic patients infected with Omicron between 24 April and 10 May 2022. We scored chest CT imaging features and categorized the patients into obvious pulmonary involvements (OPI) (score > 2) and not obvious pulmonary involvements (NOPI) (score ≤ 2) groups based on the median score. The risk factors for OPI were identified with analysis results visualized by nomogram.ResultsIn total, 339 patients were included (145 were male and 194 were female), and the most frequent clinical symptoms were cough (75.5%); chest CT imaging features were mostly linear opacities (42.8%). Pulmonary involvements were more likely to be found in the left lower lung lobe, with a significant difference in the lung total severity score of the individual lung lobes (p < 0.001). Logistic regression analysis revealed age stratification [odds ratio (OR) = 1.92, 95% confidence interval (CI) (1.548–2.383); p 8d) [OR = 1.842, 95% CI (1.104–3.073); p = 0.019], and pulmonary diseases [OR = 4.698, 95% CI (1.159–19.048); p = 0.03] as independent OPI risk factors.ConclusionAsymptomatic and mildly symptomatic patients infected with Omicron had pulmonary involvements which were not uncommon. Potential risk factors for age stratification, prolonged NCT, and pulmonary diseases can help clinicians to identify OPI in asymptomatic and mildly symptomatic patients infected with Omicron.

    الوصف: Bisphenol F (BPF) and bisphenol AF (BPAF) are structural analogues of bisphenol A (BPA) that are used in the manufacture of a myriad of BPA-free products; however, there is a paucity of information regarding their developmental effects. The present study investigates the effects of BPF and BPAF on neurodevelopment and pancreatic β-cell differentiation via altering DNA methylation and gene expression patterns using the zebrafish model. BPF and BPAF induced behavioral perturbations: increased average speed, increased maximum acceleration, increased mania time and decreased static time, in 0.3 and 1.0 μM groups in zebrafish embryos. Glucose level was significantly increased in 1.0 μM BPF (28 %); while a monotonic increase of 29 %, 55 %, and 74 % were observed in 0.1, 0.3, and 1.0 μM BPAF, respectively. Consistent with a decreased insulin mRNA level, the expression of two critical transcription factors (pdx-1 and foxa2) essential for the development and functioning of beta-cells decreased following the bisphenols exposure. In addition, embryonic exposure to BPF and BPAF upregulated the transcription of developmental genes (vegfa, wnt8a, and mstn1) and neuron-related genes (mbp, elavl3, gap43, gfap). Also, the expressions of DNA methyltransferases (dnmt1, dnmt3, dnmt4, dnmt5, dnmt6, dnmt7, and dnmt8) were significantly aberrant compared with the control group. The Bisulfite PCR results indicate increased DNA methylation at promoter regions of pdx-1 in BPF (8.2 %) and BPAF (7.6 %); α1-tubulin in BPF (5.3 %) and in BPAF (4.1 %), congruous with the increased dnmt1 and dnmt3 transcription, at early stage of zebrafish development. The present study indicates that zebrafish embryonic exposure to BPF and BPAF elicits islet dysfunction and neuron perturbations resulting in increased DNA methylation levels.

    الوصف: Abstract Objective To further comprehend the phenotype of multiple mitochondrial dysfunction syndrome type 3 (MMDS3:OMIM#615330) caused by IBA57 mutation. We present a case involving a patient who experienced acute neurological regression, and the literature was reviewed. Methods Clinical data and laboratory test results were collected; early language and development progress were tested; and genetic testing was performed. Bioinformatics analysis was performed using Mutation Taster and PolyPhen‐2, and the literature in databases such as PubMed and CNKI was searched using MMDS3 and IBA57 as keywords. Results The child, aged 1 year and 2 months, had motor decline, unable to sit alone, limited right arm movement, hypotonia, hyperreflexia of both knees, and Babinski sign positivity on the right side, accompanied by nystagmus. Blood lactate levels were elevated at 2.50 mmol/L. Brain MR indicated slight swelling in the bilateral frontoparietal and occipital white matter areas and the corpus callosum, with extensive abnormal signals on T1 and T2 images, along with the semioval center and occipital lobes bilaterally. The multiple abnormal signals in the brain suggested metabolic leukoencephalopathy. Whole‐exome sequencing analysis revealed that the child had two heterozygous mutations in the IBA57 gene, c.286T>C (p.Y96H) (likely pathogenic, LP) and c.992T>A (p.L331Q) (variant of uncertain significance, VUS). As of March 2023, a literature search showed that 56 cases of MMDS3 caused by IBA57 mutation had been reported worldwide, with 35 cases reported in China. Among the 35 IBA57 mutations listed in the HGMD database, there were 28 missense or nonsense mutations, 2 splicing mutations, 2 small deletions, and 3 small insertions. Conclusion MMDS3 predominantly manifests in infancy, with primary symptoms including feeding difficulties, neurological functional regression, muscle weakness, with severe cases potentially leading to mortality. Diagnosis is supported by elevated lactate levels, multisystem impairment (including auditory and visual systems), and distinctive MRI findings. Whole‐exome sequencing is crucial for diagnosis. Currently, cocktail therapy offers symptomatic relief.

    الوصف: Hemorrhoids are a common ailment that can cause significant disruptions to one’s daily life. While some researchers have speculated about a potential link between hemorrhoid development and gut microbes, there is currently insufficient evidence to support this claim. In this study, we collected samples from 60 hemorrhoid patients and analyzed the composition and characteristics of microbiomes in hemorrhoids. PCoA results revealed distinct differences between the microbiomes of hemorrhoids, skin-originated microbiomes, and gut microbes, highlighting the complex nature of hemorrhoidal microbiomes. The distribution characteristics of Staphylococcus suggest that the skin microbiome influences the microbiome of hemorrhoids. Additionally, we observed higher levels of Prevotella in two cases of thrombosed hemorrhoids compared to non-thrombosed hemorrhoids. This finding suggests that Prevotella may play a crucial role in the development of thrombosed hemorrhoids.

    الوصف: This paper systematically investigates the effect of multi-tooth numbers on electromagnetic performances of conventional and consequent-pole (CP) vernier permanent magnet (PM) synchronous machines (VPMSMs), including average torque, overload capability, PM utilization, loss, efficiency, and power factor, etc. In particular, the influence of multi-tooth numbers on the unipolar end leakage flux of CP VPMSMs is investigated for the first time. It confirms that CP rotor and multi-tooth stator structures exhibit better field modulation effects compared with conventional north-south pole and non-multi-tooth stator structures. It reveals that there exists an optimal multi-tooth number for torque output capability and PM utilization, as well as unipolar end leakage flux. In particular, it shows that the multi-tooth stator structure helps save PM volume significantly for CP VPMSMs and suppress unipolar end leakage flux. The PM utilization rate of CP VPMSM with 5 multi-teeth is more than 50% higher than that with single-tooth structure while the unipolar end leakage flux is reduced by around 60%. However, the overload capability and power factor will be sacrificed. Finally, the prototypes with 2 and 3 multi-teeth numbers are manufactured and tested to verify the FEA analyses.

    الوصف: Infrared small target detection is a challenging task in which many researchers have made lots of achievements. While the performance of single-frame (SF) detection is still limited due to the lack of usage of multiframe (MF) continuous information, many spatial-temporal detection methods have been developed. However, most algorithms need to register the image or feed a set of images as the input. Inputting a batch of group images usually leads to large computations, which heavily affects their real-time processing capability in resource-limited machines. To tackle the problem, we propose a nonlocal multiframe network (NLMF-Net) with only a few additional computations (no more than 0.01 GFLOPs) compared to the SF baseline while achieving significant performance improvements. The NLMF-Net correlates features from grid cells with high confidence between current and past frames. While most background grid cells are removed after the SF processing, the MF feature fusion only focuses on a few potential target grid cells, resulting in high computation efficiency. The proposed vector length similarity module enlarges the difference between different grid cells and the non max similarity suppression further suppresses the backgrounds during the fusion, promoting the MF performance. The NLMF-Net can be readily deployed on Jetson Nano at a speed of 20 FPS for 288 × 384 image processing or Mi Pad 2 with a speed over 35 FPS for 128 × 128 part image processing. Extensive experiments show that our proposed method achieves state-of-the-art performance on three datasets while maintaining high efficiency in a real-time processing manner.

    الوصف: Abstract This article investigates a mobile edge computing (MEC) network assisted by multiple unmanned aerial vehicles (UAVs) to address the computational and offloading requirements for mobile intelligent terminals (MITs) within crowded venues. The objective is to tackle intricate task processing and diminish MITs' waiting times. Considering the randomness of task arrival at the MITs and the imbalance between the amount of data and computation for complex tasks, a dual‐queue model with data cache queue and computation queue is proposed, with minimizing the weighted system total energy consumption and average delay as the optimization objectives. Lyapunov optimization theory is employed to convert the stochastic optimization problem into a deterministic one, and the initial deployment quantity and hovering position of the UAVs are determined by the density‐based spatial clustering of applications with noise (DBSCAN) method with noise. Then PPO algorithm for MIT task, resource allocation, and UAV trajectory optimization. Numerical results display the proposed scheme can efficaciously diminish energy consumption and delay by 10% and 33% respectively, compared with the baseline scheme. This paper proposes a practical and feasible solution for stochastic computing offloading in UAV‐assisted MEC, which fills the gap in existing research on regarding the consideration of complex task imbalances.
