الوصف: Objective: to study the anamnestic, clinical and laboratory features of the acute period of ischemic stroke (IS) and to determine the risk factors for its development in young patients. Patients and methods. Clinical and statistical processing of data of 256 patients aged 18 to 44 years included, who had IS, confirmed by computed and/or magnetic resonance imaging of the brain in the acute period, was carried out. Furthermore, in 154 patients and in 117 healthy participants, who made up the control group, eight polymorphisms of the thrombophilic spectrum genes were determined – FGB: -455G>A, F2: 20210G>A, F5: 1691G>A, F7: 10976G>A, F13: 103 G>T, ITGA2: 807C>T, ITGB3: 1565 T>C, PAI-1: -675 5G>4G. Results and discussion. 154 (60.15%) patients demonstrated good recovery (achievement of a level of ≤2 points on the Rankin scale by the patient). None of the patients died during their hospitalization. In the evaluated group of patients, we identified allelic variants of the thrombophilic spectrum genes and gene-gene combinations, the carriage of which increased the likelihood of IS development at the young age by 1.74 and 2.19 times, respectively. Taking into consideration additional examination methods, the pathogenetic variant of IS according to the TOAST classification was verified in 226 (88%) patients. Conclusion. In IS at a young age a detailed assessment of risk factors is required, including an analysis of carrier variants and combinations of procoagulant and prothrombotic spectrum gene polymorphisms. © 2022 Ima-Press Publishing House. الوصف: The article describes the urgent problem of ischemic stroke prevention in patients with atrial fibrillation. It is proved that ischemic stroke in combination with AF is the most severe in terms of developing stable motor and speech disorders and disability. The frail older patients, as well as patients with swallowing disorders and reduced medical adherence present a special problem from this point of view. The most famous clinical studies on secondary prevention of cardioembolic stroke are RE-LY, ROCKET-AF, and ARISTOTLE. Based on subanalyses of randomized controlled trials, direct oral anticoagulants demonstrated a favorable efficacy profile in patients with atrial fibrillation and stroke/ transient ischemic attack, but the level of knowledge on each of them remained different. A number of advantages of rivaroxaban for primary and secondary prevention of stroke in patients with atrial fibrillation, including the elderly and patients with cognitive impairments and swallowing disorders, have been demonstrated. © 2021 Vserossiiskoe Obshchestvo Kardiologov. الوصف: The method of calculation of the resonance characteristics is developed for the metastable states of the Coulomb three-body (CTB) system with two disintegration channels. The energy dependence of K-matrix in the resonance region is calculated with the use of the stabilization method. Resonance position and partial widths are obtained by fitting the numerically calculated K(E)-matrix with the help of the generalized Breit-Wigner formula.

الوصف: The article analyzes the relationship between the idiopathic form of spastic torticollis and scoliosis of the thoracic spine in fifty patients. According to the constructed graphs of the distribution of values of the scoliotic angle according to Cobb relative to the severity of the condition of patients with cervical dystonia, a close to logarithmic dependence of these parameters on each other is determined. Based on the results obtained, it is concluded that the assessment of deformity of musculoskeletal structures can be used as one of the tools for determining the severity of the condition of patients with cervical dystonia ; В статье анализируется связь между идиопатической формой спастической кривошеи и сколиозом грудного отдела позвоночника у пятидесяти пациентов. По данным построенных графиков распределения значений сколиотического угла по Коббсу относительно тяжести состояния пациентов с цервикальной дистонией определяется близкая к логарифмической зависимость этих параметров друг от друга. На основании полученных результатов делается вывод о том, что оценка деформации костно-мышечных структур может быть использована в качестве одного из инструментов определения тяжести состояния пациентов с цервикальной дистонией

الوصف: Dopa-responsive dystonia (DRD) is a rare progressive genetically heterogenous disorder with pediatric onset. DRD is 3 times as prevalent in women than in men. This article reports a clinical case of DRD in a young female presenting with paraparesis, foot dystonia (more pronounced in the right foot) and pronounced walking impairment, who was admitted for emergency treatment to a Neurology Unit. Based on the additional tests, which included a levodopa trial and Sanger sequencing, the patient was diagnosed with DRD. Levodopa caused a considerable improvement of the symptoms. The article describes the clinical features of the disease, talks about its differential diagnosis, genetic predisposition and treatment strategy. © 2020 Pirogov Russian National Research Medical University. الوصف: In this work were analyzed objective laws of material allowance removal that are common for different schemes of grinding with the use of field theory. Each level of the working surface of the circle corresponds to a certain scalar quantity, which characterizes the relative reference surface area of the diamond grains and the total grain width at this level. The total grains width determines the probability of material removal from the component surface in the process of grinding under the influence of the field of vectors of instrument’s grains. Changes which occur in the area of contact between material and grinding wheel lead to negative divergence of material flow and wheel’s grains. The change in the divergence of the fields is characterized respectively efficiency of machining and destruction of an instrument’s working area.
