    الوصف: Abstract Background Growing evidence indicated that to develop of atherosclerosis observed more often by people with Alzheimer’s disease (AD), but the underlying mechanism is not fully clarified. Considering that amyloid-β (Aβ) deposition in the brain is the key pathophysiology of AD and plasma Aβ is closely relate to Aβ deposition in the brain, in the present study, we investigated the relationships between atherosclerosis and plasma Aβ levels. Methods This was a population based cross-sectional study. Patients with high risk of atherosclerosis from Qubao Village, Xi’an were underwent carotid ultrasound for assessment of atherosclerosis. Venous blood was collected on empty stomach in the morning and plasma Aβ1−40 and Aβ1−42 levels were measured using ELISA. Multivariate logistic regression analysis was performed to investigate the relationships between carotid atherosclerosis (CAS) and plasma Aβ levels. Results Among 344 patients with high risk of atherosclerosis, 251(73.0%) had CAS. In the univariate analysis, the plasma Aβ levels had no significant differences between CAS group and non-CAS group (Aβ1−40: 53.07 ± 9.24 pg/ml vs. 51.67 ± 9.11pg/ml, p = 0.211; Aβ1−42: 40.10 ± 5.57 pg/ml vs. 40.70 pg/ml ± 6.37pg/ml, p = 0.285). Multivariate logistic analysis showed that plasma Aβ levels were not associated with CAS (Aβ1−40: OR = 1.019, 95%CI: 0.985–1.054, p = 0.270;Aβ1−42: OR = 1.028, 95%CI: 0.980–1.079, p = 0.256) in the total study population. After stratified by hypertension, CAS was associated with plasma Aβ1−40 positively (OR = 1.063, 95%CI: 1.007–1.122, p = 0.028) in the non-hypertension group, but not in hypertensive group. When the plasma Aβ concentrations were classified into four groups according to its quartile, the highest level of plasma Aβ1−40 group was associated with CAS significantly (OR = 4.465, 95%CI: 1.024–19.474, p = 0.046). Conclusion Among patients with high risk of atherosclerosis, CAS was associated with higher plasma Aβ1−40 level in non-hypertension group, but not in hypertension group. These indicated that atherosclerosis is associated with plasma Aβ level, but the relationship may be confounded by hypertension.

    الوصف: 【Objective】Undersea optical cable plays a leading role in international communication. In the planning of undersea optical cable communication system, several groups of possible schemes are often determined according to experience. The scheme with relatively better transmission performance and investment cost is then selected. However, with the development of communication equipment and communication technology, the number of optional modulation formats and optical fibers keeps are increasing. The number of alternative schemes is too large to quickly determine the optimal scheme. Therefore, it is necessary to establish a cost optimization model of undersea optical cable communication system to solve the cost optimal scheme.【Methods】Aiming at the problem of system planning and configuration in the undersea optical cable communication system, a cost optimization model considering the parameters of transmission performance, transmission capacity and supply voltage is established by taking the span length, optical fiber type, number of optical fiber pairs, repeater output power, modulation format and number of practical channels as the variables. The cascaded-lexicographic method is used to solve the cost optimization model.【Results】The results show that the difference between the planning results of the proposed optimization algorithm and full traversal algorithm are no more than 0.34%. However, the number of operations of the optimization algorithm proposed in this paper is far less than that of the full traversal algorithm, with only 1/100 of that of the full traversal algorithm. Secondly, the number of operations of the cascade genelexicography method is less affected by the range of variables than that of the total traversal method. Finally, by comparing the optimal cost of the undersea cable communication system under different span length, it is found that the planning results have no regularity.【Conclusion】The variables always influence each other. The optimal cost design of the system cannot be obtained according to experience, so it needs to establish a model. The algorithm used in this paper is not only accurate, but also uses fewer operation times.

This study aimed to evaluate the two administration routes of dexmedetomidine on acute pain and chronic neuropathic pain (NeuP) prevention compared with no dexmedetomidine.Methods: A total of 216 patients were randomized to receive TPVB using 0.4% ropivacaine alone (R Group), with perineural dexmedetomidine 0.5 μg·kg− 1 (RD0.5 Group) or 1.0 μg·kg− 1 (RD1.0 Group), or intravenous (IV) dexmedetomidine 0.5 μg·kg− 1·h− 1 (RDiv Group). The primary outcome was the incidence of chronic NeuP, defined as a Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) pain score > 12 points at 3-month after surgery.Results: (1) For the primary outcome, RD0.5 Group and RD1.0 Group demonstrated a decreased incidence of chronic NeuP at 3-month after surgery; (2) Compared with R Group, RDiv Group, RD0.5 Group, and RD1.0 Group can reduce VAS scores at rest and movement and Prince-Henry Pain scores at 12 and 24-h after surgery, the consumption of oral morphine equivalent (OME) and improve QOD-15 at POD1; (3) Compared with RDiv Group, RD0.5 Group and RD1.0 Group can reduce VAS scores at rest and movement and Prince-Henry Pain scores at 12 and 24-h after surgery, the consumption of postoperative OME and improve QOD-15 at POD1; (4) Compared with RD0.5 Group, RD1.0 Group effectively reduced VAS scores at rest at 12 and 24-h after surgery, VAS scores in movement and Prince-Henry Pain scores at 12-h after surgery. However, RD1.0 Group showed an increased incidence of drowsiness.Conclusion: Perineural or IV dexmedetomidine are similarly effective in reducing acute pain, but only perineural dexmedetomidine reduced chronic NeuP.     الوصف: The application of bio-based solvents for living radical polymerization has been a hot topic in recent year. In this article, γ-valerolactone (GVL), a bio-based solvent as green media for ARGET atom transfer radical polymerization (ATRP) of styrene (St) were investigated. We first conducted polymerization of St in γ-valerolactone using copper(ii) bromide as the catalyst, tris(2-pyridylmethyl) amine as the ligand, and only sodium ascorbate as reducing agent. The polymerization achieved moderate conversion; however, the controllability of polymerization was not very good, providing a polymer with a broad molecular weight distribution (M w/M n > 1.30). When sodium carbonate is introduced, excellent results were obtained providing high yields and low M w/M n values under very low catalyst usage (∼5 ppm). 1H NMR spectroscopy, chain extension, and MALDI–MS experiments confirmed the final polymer chains with high fidelity. The use of GVL solvent opens a new route for the easy synthesis of PS through ARGET ATRP with low catalyst usage conditions.

    الوصف: Purposes As a coal based solid waste rich in silicon and aluminum oxides, utilization of coal gangue is crucial for the sustainable development of coal industry. Methods Integrated grinding modification of calcined coal gangue and aluminate coupling agent to prepare rubber reinforcement was proposed. The influences of grinding time and the amount of modifier on the surface modification were investigated. Ultrafine particles were obtained through air flow classification, and the influences of modification or not and particle size on the reinforcing and filling properties of rubber were analyzed. Findings The results show that the best comprehensive modification effect is achieved when the duration of ball milling is 30 min and the amount of aluminate coupling agent is 2%. The characteristic functional groups of aluminate coupling agent appear on the surface of coal gangue. Compared with the rubber filled with commercial silica, the rubber filled with the air-flow graded modified filler possesses higher hardness and crosslinking density, its vulcanization performance is better, the tensile strength, elongation at break and permanent deformation at break are reduced, and the fixed extension stress is increased. The particle size distribution of the filler has an obvious effect on the properties of rubber, with the decreases of the particle size of the filler, the vulcanization property is improved, and the tensile property is also improved. The reinforcement performance of modified superfine coal gangue as filler is close to that of silica, which has the potential to replace silica.

We aimed to compare the efficacy of the pericapsular nerve group (PENG) block with lateral femoral cutaneous nerve (LFCN) block compared to fascia iliaca compartment block (FICB) in patients with AHS.Patients and Methods: A total of 80 patients receiving AHS under general anesthesia were randomized to receive either FICB (group F) or PENG block in combination with LFCN block (group P). The primary outcomes were the rate of quadriceps weakness after block on the afflicted side, as well as muscle strength grading and pain score after block, and the quality of recovery on the second postoperative day.Results: Compared with group F, group P had a lower incidence of quadriceps weakness 48 h after block (76.9% vs 28.2%, P < 0.001), and had less impact on muscle strength grade and lower static pain score at 6, 12, 18, 24, 36, and 48 h after block (P < 0.001), and a lower dynamic pain score at 6 and 12 h after block in group P (p < 0.05). The quality of recovery on the second postoperative day improved (p < 0.05).Conclusion: In comparison to FICB, PENG block in combination with LFCN block can affect less quadriceps muscle strength and reduce the use of postoperative analgesics, which is beneficial for the postoperative recovery of AHS patients.Keywords: analgesia, fascia iliaca compartment block, hip arthroscopy, nerve group block, pericapsular, postoperative pain

The challenge lies in the scarcity of targeted therapies with substantial efficacy for DCM. Additionally, traditional anti-heart failure drugs are constrained due to hypotension propensity or limited symptom improvement. Kuoxin Formula (KXF), internally endorsed at Longhua Hospital, demonstrates clear biological evidence for enhancing cardiac function and myocardial remodeling. Previous clinical studies suggest its potential to enhance patients’ quality of life. This trial aims to further evaluate KXF’s safety and efficacy in managing DCM-related heart failure.Methods: This prospective, randomized, double-blind, placebo-controlled, multicenter trial aims to recruit 230 DCM patients from five centers. Participants will be randomly assigned to either KXF or placebo for 12 weeks, with careful monitoring of key indicators and adverse events. The primary outcome measures the proportion of patients with NT-proBNP reduction exceeding 30%. Secondary outcomes include New York Heart Association functional classification, Traditional Chinese Medicine syndrome scores, 6-minute walk test, Lee’s heart failure score, and Minnesota Heart Failure Quality of Life Scale score. Ventricular remodeling will be assessed using cardiac ultrasound and ELISA. Safety metrics and adverse events will be meticulously recorded.Discussion: This study will be the first multicentered research conducted in China that utilizes a randomized, double-blind, placebo-controlled design to investigate the use of TCM in the treatment of DCM.     الوصف: Objective To investigate the effect of cerebrospinal fluid (CSF) metagenomic next - generation sequencing (mNGS) in diagnosis and treatment of intracranial infection. Methods A retrospective analysis of 117 patients with intracranial infection were admitted to Shenzhen Second People's Hospital from January 2020 to December 2022. Before initiating empirical broad - spectrum antibiotic treatment, CSF samples were collected for microbial culture (conventional culture group, n = 58) and/or mNGS (mNGS group, n = 59). The results were used to adjust the antibiotic treatment plan, and the study compared the pathogen detection rates and result reporting times of the two testing methods, as well as the antibiotic intensity grading, the intensive care unit (ICU) stay, total hospital stay, and mortality rate. Results A total of 65 pathogens were detected from the CSF samples of all the patients through microbial culture and/or mNGS, with viruses being the most prevalent, accounting for 49.23% (32/65), followed by Gram-negative bacteria (24.62%, 16/65), Gram-positive bacteria (18.46%, 12/65) and fungi (7.69%, 5/65). The pathogen detection rate of CSF mNGS was higher than that of CSF microbial culture (χ2 = 22.781, P = 0.000), and the results were reported earlier (t = - 32.588, P = 0.000). Based on the results, 20 cases (33.90%) in the mNGS group adjusted antibiotic treatment plan, with 5 cases downgrading and 15 cases upgrading the intensity of antibiotic application. In the conventional culture group, 30 cases (51.72%) adjusted their antibiotic treatment plan, with 17 cases downgrading and 13 cases upgrading the intensity of antibiotic application. There was no significant difference between the 2 groups (Z = - 1.917, P = 0.055). And there was no statistically significant difference in ICU stay (Z = - 0.716, P = 0.474), total hospital stay (Z = - 0.933, P = 0.351) and mortality rate (Fisher's exact probability: P = 0.496) between the 2 groups. Conclusions The application of mNGS can effectively improve the detection rate of intracranial infection pathogens, which may shorten the duration of antibiotic use and reduce the intensity of broad - spectrum antibiotics, so as to help clinical effective judgment and treatment decision.

    الوصف: Abstract While sanguinarine has gained recognition for antimicrobial and antineoplastic activities, its complex conjugated structure and low abundance in plants impede broad applications. Here, we demonstrate the complete biosynthesis of sanguinarine and halogenated derivatives using highly engineered yeast strains. To overcome sanguinarine cytotoxicity, we establish a splicing intein-mediated temperature-responsive gene expression system (SIMTeGES), a simple strategy that decouples cell growth from product synthesis without sacrificing protein activity. To debottleneck sanguinarine biosynthesis, we identify two reticuline oxidases and facilitated functional expression of flavoproteins and cytochrome P450 enzymes via protein molecular engineering. After comprehensive metabolic engineering, we report the production of sanguinarine at a titer of 448.64 mg L−1. Additionally, our engineered strain enables the biosynthesis of fluorinated sanguinarine, showcasing the biotransformation of halogenated derivatives through more than 15 biocatalytic steps. This work serves as a blueprint for utilizing yeast as a scalable platform for biomanufacturing diverse benzylisoquinoline alkaloids and derivatives.

    الوصف: Abstract Improving rice quality remains a crucial breeding objective, second only to enhancing yield, yet progress in quality improvement lags behind yield. The high temperature and ripening conditions in Southern China often result in poor rice quality, impacting hybrid rice production and utilization. Therefore, to address this challenge, analyzing the molecular basis of high-quality traits is essential for molecular design breeding of high-quality hybrid rice varieties. In this study, we investigated the molecular basis of grain shape, amylose content, gel consistency, gelatinization temperature, and aroma, which influence rice quality. We discovered that quality related alleles gs3, GW7 TFA , gw8, chalk5, Wx b , ALK TT , and fgr can enhance rice quality when applied in breeding programs. Polymerization of gs3, GW7 TFA , gw8, and chalk5 genes improves rice appearance quality. The gs3 and GW7 TFA allele polymerization increasing the grain’s length-width ratio, adding the aggregation of gw8 allele can further reducing grain width. The chalk5 gene regulates low chalkiness, but low correlation to chalkiness was exhibited with grain widths below 2.0 mm, with minimal differences between Chalk5 and chalk5 alleles. Enhancing rice cooking and eating quality is achieved through Wx b and ALK TT gene polymerization, while introducing the fgr(E7) gene significantly improved rice aroma. Using molecular marker-assisted technology, we aggregated these genes to develop a batch of indica hybrid rice parents with improved rice quality are obtained. Cross-combining these enhanced parents can generate new, high-quality hybrid rice varieties suitable for cultivation in Southern China. Therefore, our findings contribute to a molecular breeding model for grain quality improvement in high-quality indica hybrid rice. This study, along with others, highlights the potential of molecular design breeding for enhancing complex traits, particularly rice grain quality.
