    الوصف: Retinal vasculitis is a sight-threatening inflammatory condition affecting the retinal vessels. It can occur due to several etiologies and spectrum of other ocular inflammatory conditions. It can occur as an isolated idiopathic condition, as a complication of infective or neoplastic disorders, or in association with systemic inflammatory disease. The various known etiologies have been discussed here with their characteristic features. Etiological diagnosis of retinal vasculitis can be challenging. Clinical examination, tailored laboratory investigations and multimodal imaging with fundus photography, fundus autofluorescence, fundus fluorescein angiography, indocyanine green angiography, swept-source optical coherence tomography (OCT), OCT angiography, B scan ultrasonography and in recent years widefield fundus fluorescein angiogram, imaging aids in the early diagnosis, monitoring and provides newer insights about these diseases. Newer treatment modalities and therapies such as immunomodulators and biologics have been added to the armamentarium against these sight-threatening conditions. Anti-vascular endothelial growth factor agents and laser photocoagulation are particularly beneficial to arrest the neovascularization process. The diagnosis of vasculitis may uncover an unknown underlying systemic cause. Early, effective, and prompt management can be vision saving and can minimize comorbidities. We feel that there is a necessity to update our knowledge on existing and newer trends in diagnosis, investigations, and management of retinal vasculitis. This review focuses on some of the basic concepts related to retinal vasculitis with an update.

    الوصف: Central retinal vein occlusion (CRVO) is a significant cause of marked or total loss of vision in the middle-aged and elderly population. The patient was a 54-year-old man with a history of uncontrolled diabetes and was given seven intravitreal bevacizumab (Avastin) injections as treatment. The patient was advised to follow-up during the next 6 months, including gonioscopy and undilated examination of the iris to look for the iris/disc neovascularization. We presented a classic case of unilateral ischemic CRVO with macular edema, in whom diabetes is the leading risk factor. Close follow-up and diabetes control are advisable to prevent a similar scenario.

    الوصف: Objective: The aim of this research is to present a novel computer-aided decision support tool in analyzing, quantifying, and evaluating the retinal blood vessel structure from fluorescein angiogram (FA) videos.Methods: The proposed method consists of three phases: (i) image registration for large motion removal from fluorescein angiogram videos, followed by (ii) retinal vessel segmentation, and lastly, (iii) segmentation-guided video magnification. In the image registration phase, individual frames of the video are spatiotemporally aligned using a novel wavelet-based registration approach to compensate for the global camera and patient motion. In the second phase, a capsule-based neural network architecture is employed to perform the segmentation of retinal vessels for the first time in the literature. In the final phase, a segmentation-guided Eulerian video magnification is proposed for magnifying subtle changes in the retinal video produced by blood flow through the retinal vessels. The magnification is applied only to the segmented vessels, as determined by the capsule network. This minimizes the high levels of noise present in these videos and maximizes useful information, enabling ophthalmologists to more easily identify potential regions of pathology.Results: The collected fluorescein angiogram video dataset consists of 1, 402 frames from 10 normal subjects (prospective study). Experimental results for retinal vessel segmentation show that the capsule-based algorithm outperforms a state-of-the-art convolutional neural networks (U-Net), obtaining a higher dice coefficient (85.94%) and sensitivity (92.36%) while using just 5% of the network parameters. Qualitative analysis of these videos was performed after the final phase by expert ophthalmologists, supporting the claim that artificial intelligence assisted decision support tool can be helpful for providing a better analysis of blood flow dynamics.Conclusions: The authors introduce a novel computational tool, combining a wavelet-based video registration method with a deep learning capsule-based retinal vessel segmentation algorithm and a Eulerian video magnification technique to quantitatively and qualitatively analyze FA videos. To authors' best knowledge, this is the first-ever development of such a computational tool to assist ophthalmologists with analyzing blood flow in FA videos.

    الوصف: Diabetic retinopathy (DR) is a well-known risk factor in the development of radiation maculopathy (RM). Steroids have been shown to improve the vision and reduce the macular thickness in patients with RM. This observational case report highlights altered course of DR after a course of radiotherapy for orbital lymphoma, after a single dose of intravitreal dexamethasone implant showed a dramatic revascularization of the ischemic macula, with a significant reduction in the size of ischemic area. This appears to be the first case in literature corroborating the favorable effect on steroids on retinal vasculature, seen angiographically.

    الوصف: Burkholderia pseudomallei is a known great mimicker responsible for melioidosis. Ocular involvement in melioidosis is unusual, with eyelid and orbital infection as the commonest presentation. We describe a 41-year-old, healthy woman who complained of reduced vision in her left eye. On examination, vision in the left eye was 6/9. There was evidence of occlusive retinal vasculitis on fundoscopy examination. Fundus fluorescein angiogram showed extensive capillary fallout. Diagnosis was established by a rise in the serum antibody titre for the bacterium and further supported by clinical improvement of vision after completion of treatment antibiotics. Sectoral panretinal photocoagulation at the capillary fallout area successfully arrested the sequelae of retinal ischaemia. Occlusive retinal vasculitis is a rare presentation of melioidosis. Early prompt diagnosis in an immunocompetent individual helps prevent visual-related morbidity. The ability of this bacteria to cause recurrent infection in an endemic area should not be underestimated.

    الوصف: Purpose To determine classification criteria for Vogt-Koyanagi-Harada (VKH) disease Design Machine learning of cases with VKH disease and 5 other panuveitides. Methods Cases of panuveitides were collected in an informatics-designed preliminary database, and a final database was constructed of cases achieving supermajority agreement on the diagnosis, using formal consensus techniques. Cases were split into a training set and a validation set. Machine learning using multinomial logistic regression was used on the training set to determine a parsimonious set of criteria that minimized the misclassification rate among the panuveitides. The resulting criteria were evaluated on the validation set. Results One thousand twelve cases of panuveitides, including 156 cases of early-stage VKH and 103 cases of late-stage VKH, were evaluated. Overall accuracy for panuveitides was 96.3% in the training set and 94.0% in the validation set (95% confidence interval 89.0, 96.8). Key criteria for early-stage VKH included: 1) exudative retinal detachment with characteristic appearance on fluorescein angiogram or optical coherence tomography or 2) panuveitis with ≥2 of 5 neurologic symptoms/signs. Key criteria for late-stage VKH included history of early-stage VKH and either: 1) sunset glow fundus or 2) uveitis and ≥1 of 3 cutaneous signs. The misclassification rates in the learning and validation sets for early-stage VKH were 8.0% and 7.7%, respectively, and for late-stage VKH 1.0% and 12%, respectively. Conclusions The criteria for VKH had a reasonably low misclassification rate and appeared to perform sufficiently well for use in clinical and translational research.

    الوصف: BACKGROUND/PURPOSE Report a case of markedly asymmetric retinal tessellations and propose mosaicism as a mechanism. METHODS AND RESULTS A 59-year-old pseudophakic woman presented with uncorrected 20/20 vision and was found to have markedly different retinal tessellation appearances in both eyes. The axial lengths were 25.66 mm and 25.88 mm in the right and left eyes, respectively, and no significant asymmetrical choroidal thinning was seen on optical coherence tomography or optical coherence tomography angiography. Fluorescein angiogram showed significant hyperfluorescence, representing the underlying choroid, which correlated with the tessellation patterns in the left eye. She had no other ocular or systemic findings such as stripes or whorled skin. CONCLUSION This is the first reported case of markedly asymmetric retinal tessellation patterns that are not due to asymmetric axial myopia or choroidal thinning. We propose that mosaicism is a possible mechanism causing this finding.

    الوصف: Hand, foot, and mouth disease (HFD) is a common systemic infection occurring in childhood or immunocompromised adults caused by enteroviruses, the most common being coxsackievirus A16 and enterovirus 71. It is characterized by maculopapular eruptions over the hands and feet and ulcerative stomatitis. Ocular involvement is a rare complication and commonly manifests as inflammatory macular pathology. We report a case of HFD in an immunocompetent adult male with unilateral ocular involvement presenting as hemorrhagic maculopathy and its management with complete anatomical and functional recovery.
