    الوصف: Ageing of the global population represents a challenge for national healthcare systems and healthcare professionals, including medico-legal experts, who assess personal damage in an increasing number of older people. Personal damage evaluation in older people is complex, and the scarcity of evidence is hindering the development of formal guidelines on the subject. The main objectives of the first multidisciplinary Consensus Conference on Medico-Legal Assessment of Personal Damage in Older People were to increase knowledge on the subject and establish standard procedures in this field. The conference, organized according to the guidelines issued by the Italian National Institute of Health (ISS), was held in Bologna (Italy) on June 8, 2019 with the support of national scientific societies, professional organizations, and stakeholders. The Scientific Technical Committee prepared 16 questions on 4 thematic areas: (1) differences in injury outcomes in older people compared to younger people and their relevance in personal damage assessment; (2) pre-existing status reconstruction and evaluation; (3) medico-legal examination procedures; (4) multidimensional assessment and scales. The Scientific Secretariat reviewed relevant literature and documents, rated their quality, and summarized evidence. During conference plenary public sessions, 4 pairs of experts reported on each thematic area. After the last session, a multidisciplinary Jury Panel (15 members) drafted the consensus statements. The present report describes Conference methods and results, including a summary of evidence supporting each statement, and areas requiring further investigation. The methodological recommendations issued during the Conference may be useful in several contexts of damage assessment, or to other medico-legal evaluation fields.

The activities undertaken so far at the site have included aerial mapping and geophysical surveys, which have provided new data for our knowledge of the archaeological context.

    الوصف: Résumé Les Auteurs exposent les résultats d'une première série d'observations cytogénétiques et cytochimiques obtenus sur les lymphocytes de sujets jeunes et âgés. Les données cytogénétiques confirment les observations relevées dans la littérature sur l'instauration d'une condition d'aneuploïdie dans les plaques métaphasiques des sujets âgés des deux sexes par rapport aux jeunes. Les observations cytochimiques se réfèrent à la détermination cytophotométrique du contenu du DNA nucléaire des lymphocytes, relevé par la réaction de Feulgen, après : a) hydrolyse chloridrique standard afin de mettre en évidence une altération générique dans la réponse à cette réaction par rapport à l'âge ; b) hydrolyse chloridrique à temps croissants afin de mettre en évidence, par une analyse de la cinétique de la réaction de Feulgen, les variations des fractions acide-labile et acide-résistante du DNA nucléaire, en se référant aux phénomènes d'aneuploïdie observés sur les sujets âgés. Après hydrolyse acide on a pu rencontrer un accroissement statistiquement significatif de la réponse de Feulgen sur les lymphocytes des sujets âgés des deux sexes, alors que la cinétique de la réaction de Feulgen suggère une diminution significative de la fraction de DNA acide-labile et une croissance de celle acide-résistante. Cette dernière donnée est confirmée par la cytophotométrie des lymphocytes après vert de méthyle, et elle ne semble pas être directement en corrélation, dans l'état actuel de nos recherches, avec les phénomènes d'aneuploïdie observés.
    Summary The Authors show the first results of a cytogenetical and cytochemical study on young and old subjects. Cytogenetic data confirm previous observations about the presence of aneuploidy in the metaphases of the old subjects of both sexes to the respect of the youngs. Cytochemical observations concerns cytophotométrie determination of the nuclear contents of the lymphocytes after exposure to : a) standard chloridric hydrolysis in order to point out the differences to the Feulgen reaction in the lymphocytary nucleus of young and old subjects ; b) chloridric hydrolysis to different times, in order to stress, by mean of the analysis of the Feulgen kinetic curves, the variation of the resistence to the hydrolysis of the nuclear DNA in relation to the age. In the old subjects of both sexes, after the standard hydrolysis a statistically significative growth of the Feulgen réponse is shown, while the Feulgen kinetic curves suggest a significative diminution of the acid-labile fraction of the nuclear DNA and increase of the acid-resistant one. Cytophotomteric evaluations of the metile-green nuclear réponse in lymphocytes of old subjects confirm this last finding. The increase of the acid-resistant Feulgen-DNA in old subjects seems not to be directly correlated to the observed aneuploidy.

    الوصف: Background and aim The application of laparoscopy in elderly patients with gastric cancer is controversial, especially in Western population. The aim of the study is to evaluate the feasibility of a laparoscopic approach and compare short-term outcomes with results obtained using traditional open surgery. Methods Data concerning 52 elderly patients (≥70 years), who underwent distal gastrectomy at our institution from January 2017 to August 2020, were retrospectively analyzed. We divided the population into two cohorts according to the surgical technique (laparoscopic or open surgery) and we compared the short-term outcomes. The open and laparoscopic cohorts were compared in terms of demographics, ASA score, pTNM stage, the pathological pattern according to Lauren classification, time of surgery, selected type of gastrointestinal reconstruction, the extension of lymphadenectomy and number of lymph nodes resected, postoperative complications classified according to modified Clavien-Dindo system, length of stay (LOS), postoperative mortality within 30 days. Univariate and multivariate analyses were performed for length of stay, including age, sex, duration of surgery, BMI, technique, tumor localization, grading, reconstruction, ASA, Lauren stage, extension of lymphadenectomy, TNM stage, and neoadjuvant chemotherapy as variables. Results Demographics, pathological characteristics and surgical outcomes were comparable in both cohorts, except for the length of hospital stay (LOS) that was significative longer in the open cohort (12,31 vs 8,42 P = 0.02). Number of harvested lymph nodes and complete resection (R0) rate were similar, leading to comparable oncological results. Based on the univariate analysis, in addition to the approach, ASA score was another factor significantly associated to LOS (ASA 2 8,4 ± 2,1 days vs ASA 3 13,5 ± 7,8 days). A multiple regression was run to predict LOS from ASA and surgical approach. The multiple regression model statistically significantly predicted LOS, F(3,44) = 6,906, P Conclusion Laparoscopic distal gastrectomy is effective and safe in elderly patients with similar short-term outcomes compared to open distal gastrectomy. The main advantage of laparoscopic technique is a shorter length of hospital stay.

La valutazione del danno alla persona nelle fasce di età geriatrica è complessa e la scarsità di prove ostacola lo sviluppo di linee guida sull'argomento. L'obiettivo principale della prima Consensus Conference Multidisciplinare sulla valutazione medico-legale del danno biologico nella persona anziana è di accrescere le conoscenze sull'argomento e definire procedure standard in questo campo. La conferenza, organizzata secondo le linee guida emanate dall'Istituto Superiore di Sanità si è tenuta a Bologna l'8 giugno 2019 con il supporto di società scientifiche nazionali, organizzazioni professionali e stakeholder. Il Comitato Tecnico Scientifico ha preparato 16 domande sulle seguenti 4 aree tematiche: - differenze negli esiti delle lesioni traumatiche nelle persone anziane rispetto ai giovani e loro rilevanza nella valutazione del danno biologico; - ricostruzione e valutazione dello stato anteriore; - prassi della visita medico-legale; - valutazione multidimensionale e scale. La segreteria scientifica ha esaminato la letteratura e i documenti pertinenti, ha valutato la loro qualità e riassunto le evidenze. Durante le sessioni pubbliche della conferenza plenaria, 4 coppie di esperti hanno presentato la sintesi della letteratura per ciascuna area tematica. Conclusa l'ultima sessione, una giuria multidisciplinare si è riunita a porte chiuse e ha redatto le dichiarazioni di consenso presentate pubblicamente al termine della conferenza. Sono descritti i metodi e i risultati della Conferenza, il riepilogo delle prove a sostegno delle dichiarazioni della Giuria e le aree che richiedono ulteriori indagini. Le raccomandazioni metodologiche emesse durante la Consensus Conference possono essere utili in diversi contesti di valutazione del danno alla persona, o per altri ...

    الوصف: The incidence of endometriosis in middle-aged women is not minimal compared to that in the reproductive age group. The treatment of affected women after childbearing age to the natural transition toward menopause has received considerably poor attention. Disease management is problematic for these women due to increased contraindications regarding hormonal treatment and the possibility for malignant transformation, considering the increased cancer risk in patients with a long-standing history of the disease. This state-of-the-art review aims for the first time to assess the benefits of the available therapies to help guide treatment decisions for the care of endometriosis in women approaching menopause. Progestins are proven effective in reducing pain and should be preferred in these women. According to the international guidelines that lack precise recommendations, hysterectomy with bilateral salpingo-oophorectomy should be the definitive therapy in women who have completed their reproductive arc, if medical therapy has failed. Strict surveillance or surgery with removal of affected gonads should be considered in cases of long-standing or recurrent endometriomas, especially in the presence of modifications of ultrasonographic cyst patterns. Although rare, malignant transformation of various tissues in endometriosis patients has been described, and management is herein discussed.

To the best of our knowledge, available information on the appropriateness of lipid-lowering treatment is extremely limited. Aim: The aim of the present study was to quantify and characterize lipid-lowering drug use in a population of complex in-hospital older patients. Methods: We analyzed data from 87 units of internal medicine or geriatric medicine in the REPOSI (Registro Politerapie della Società Italiana di Medicina Interna) study, with reference to the 2010 and 2012 patient cohorts. Lipid-lowering drug use was closely correlated with the clinical profiles, including multimorbidity markers and polypharmacy. Results: 2171 patients aged >65years were enrolled (1057 males, 1114 females, mean age 78.6years). The patients treated with lipid-lowering drugs amounted to 508 subjects (23.4%), with no gender difference. Atorvastatin (39.3%) and simvastatin (34.0%) were the most widely used statin drugs. Likelihood of treatment was associated with polypharmacy (≥5drugs) and with higher Cumulative Illness Rating Scale (CIRS) score. At logistic regression analysis, the presence of coronary heart disease, peripheral vascular disease, and hypertension were significantly correlated with lipid-lowering drug use, whereas age showed an inverse correlation. Diabetes was not associated with drug treatment. Conclusions: In this in-hospital cohort, the use of lipid-lowering agents was mainly driven by patients’ clinical history, most notably the presence of clinically overt manifestations of atherosclerosis.
