الوصف: Background. Pleomorphic sarcoma is an aggressive soft tissue sarcoma. In patients with high-risk extremity sarcomas, the significant survival benefits conferred by an intense regimen of neoadjuvant chemoradiotherapy and surgery were reported. To our knowledge, this is the first report in the literature of the neoadjuvant use of trabectedin in a patient with high-grade pleomorphic sarcoma, ineligible for standard neoadjuvant combination therapy with an anthracycline-based regimen. Case Presentation. Here we present a 58-year-old White male with a large tumor in the left thigh, but with no signs of metastases. Owing to the history of severe heart attack, three cycles of neoadjuvant trabectedin were administrated to achieve surgically wide margins. After two cycles, an 18F-FDG-PET showed a large proportion of the central tumor area was without metabolic activity. According to RECIST and Choi criteria, the tumor was stable. After the third cycle of trabectedin, the patient underwent a complete resection, which revealed completely necrotic high-grade pleomorphic sarcoma (stage pT2b), with only a small vital area. Conclusion. The present paper on a promising treatment with neoadjuvant trabectedin of patients with high-grade pleomorphic sarcoma might suggest that such treatment approach may provide a greater chance of cure and survival of such patients.

الوصف: The influence of micromagnetic objects on the dynamic magnetic excitation in magnetic thin films is studied by imprinting periodic domain wall patterns through selective ion irradiation in exchange biased Ni _81 Fe _19 /IrMn structures. For high domain wall densities an increased precessional frequency is achieved. The zero field resonance of the domain wall state hereby depends directly on the stripe period, showing a pronounced increase with decrease of domain wall spacing. With the abrupt annihilation of magnetic domain walls with an applied bias field a jump-like decrease in precessional frequency takes place. The experimental data and micromagnetic simulations prove that the characteristic collective dynamic mode for the domain wall configurations is attributed to strongly coupled tilted magnetization structure. This is evidenced by an overlapping Néel wall structure for the narrowly spaced imprinted antiparallel unidirectional anisotropy state. The controlled introduction of high density frozen-in micromagnetic objects is a novel way to control the dynamic magnetic properties of continuous magnetic thin films.

الوصف: ZusammenfassungEine Totalendoprothese (TEP) der Hüfte stellt einen sicheren und sehr häufigen operativen Eingriff dar. Das perioperative Risiko und die Komplikationen während des stationären Aufenthalts werden wesentlich durch Begleiterkrankungen bestimmt. In dieser Studie wurde die Bedeutung eines Diabetes mellitus als Risikofaktor für perioperative Komplikationen sowie Komplikationen nach drei und zwölf Monaten analysiert. In der untersuchten Kohorte von 458 Patienten war das Risiko für perioperative Komplikationen und Komplikationen während des stationären Aufenthalts nach Hüft-TEP beim Vorliegen eines Diabetes mellitus um das Zwei- bis Dreifache gesteigert. Patienten mit Diabetes mellitus wiesen sowohl häufigere chirurgische als auch internistische Komplikationen auf, was im Mittel zu einer um einen Tag verlängerten stationären Verweildauer führte. Nach zwölf Monaten existierten hingegen keine signifikanten Unterschiede zwischen Patienten mit oder ohne Diabetes mellitus. Zusammenfassend stellt ein Diabetes mellitus bei Hüft-TEP einen Risikofaktor für stationäre Komplikationen dar und verlängert die stationäre Verweildauer. Nach zwölf Monaten weisen Patienten mit Diabetes mellitus nach Hüft-TEP jedoch nicht mehr Komplikationen auf als Patienten ohne Diabetes.

الوصف: ZusammenfassungDiabetes mellitus ist eine der häufigsten Volkskrankheiten, die zu einem erhöhten Frakturrisiko und einer verschlechterten Frakturheilung führen. Obwohl die zugrundeliegenden zellulären und molekularen Mechanismen noch nicht abschließend geklärt sind, scheint eine verminderte Osteoblastenfunktion beteiligt zu sein. Vor Kurzem konnten wir zeigen, dass eine Stimulation der Osteoblastenfunktion durch Aktivierung des Wnt-Signalwegs die Knochenqualität und -regeneration in diabetischen Ratten verbessert. Das Ziel dieser Studie war es, diabetische Konditionen in vitro zu simulieren, um die molekularen Mechanismen der verminderten Osteoblastenfunktion zu untersuchen.Osteoblasten-ähnliche UMR- 106-Zellen wurden in Zellkulturmedium mit normalen (6 mM) oder erhöhten (30–150 mM) Glukosekonzentrationen kultiviert. Anschließend wurde die Vitalität, Proliferation und Mineralisierung sowie die Expression von Osteoblastenmarkern und Wnt- Signalmolekülen untersucht.Nach vier Tagen in einem erhöhten Glukosemilieu nahm die Proliferation der UMR-106-Zellen bei gleichbleibender Zellzahl signifikant ab (–42 %). Ferner verringerte sich das Mineralisierungspotenzial der UMR-106-Zellen um 43 %. Die Genexpression der Osteoblastenmarker Osteokalzin und Osteopontin sank um 38 % bzw. 27 %. Die Expression von Connexin-43 und des Wnt-Inhibitors Dickkopf-1 stieg hingegen unter hyperglykämischen Bedingungen an (+15 % und +20 %). Die Konzentration von zyklischem Adenosinmonophosphat (cAMP), einem intrazellulären, ubiquitären Second messenger, nahm mit steigender Glukosemenge signifikant ab (–61 %). Die Proteinkonzentration der nachgeschalteten aktiven Proteinkinase A war in einem erhöhten Glukosemilieu reduziert.Unsere Ergebnisse weisen darauf hin, dass eine erhöhte Glukosekonzentration in vitro die Osteoblastenfunktion massiv beeinträchtigt. Eine Reduktion von cAMP sowie Veränderungen der Expression Osteoblasten-spezifischer Differenzierungsmarker sind mögliche Grundlagen dieses Defektes.

الوصف: SummaryAim Type 1 diabetes mellitus (T1DM) increases fragility fractures due to low bone mass, micro-architectural alterations and decreased bone formation. Sclerostin is expressed by osteocytes and inhibits osteoblastic bone formation. We evaluated serum sclerostin levels in T1DM and their association with bone mineral density (BMD), bone turnover, glycaemic control and physical activity. Patients and Methods In a cross-sectional study, 128 men and premenopausal women with long-standing T1DM (mean age 43·4 ± 8·8 years, diabetes duration 22·4 ± 9·5 years) and 77 age-, BMI (Body Mass Index) and gender-matched healthy individuals were evaluated. Results Serum sclerostin levels were higher in T1DM compared with controls, irrespective of gender (male 0·55 ± 0·17 vs 0·49 ± 0·12 ng/ml, P = 0·046; female 0·52 ± 0·19 ng/ml vs 0·43 ± 0·12 ng/ml, P = 0·012). Partial correlation analysis adjusted for age and gender revealed a positive correlation between serum sclerostin levels and BMD at lumbar spine and femoral neck in T1DM and between BMD at lumbar spine, femoral neck and total hip in controls. Bone turnover markers, parathyroid hormone, calcium and vitamin D did not correlate with serum sclerostin levels in T1DM or controls. Physical activity was not associated with serum sclerostin levels. A multivariate analysis revealed that only the interaction of T1DM and age affects serum sclerostin levels but not T1DM alone. The influence of age on serum sclerostin levels was more pronounced in T1DM compared with controls. Conclusions Sclerostin serum levels were increased in patients with T1DM, and the positive correlation of age with serum sclerostin levels was stronger in T1DM. There was no effect of serum sclerostin levels on markers of bone metabolism and they do not explain the detrimental effects of T1DM on BMD.

الوصف: Sclerostin is predominantly expressed by osteocytes. Serum sclerostin levels are positively correlated with areal bone mineral density (aBMD) measured by dual-energy X-ray absorptiometry (DXA) and bone microarchitecture assessed by high-resolution peripheral quantitative computed tomography (HR-pQCT) in small studies. We assessed the relation of serum sclerostin levels with aBMD and microarchitectural parameters based on HR-pQCT in 1134 men aged 20 to 87 years using multivariable models adjusted for confounders (age, body size, lifestyle, comorbidities, hormones regulating bone metabolism, muscle mass and strength). The apparent age-related increase in serum sclerostin levels was faster before the age of 63 years than afterward (0.43 SD versus 0.20 SD per decade). In 446 men aged ≤63 years, aBMD (spine, hip, whole body), trabecular volumetric BMD (Tb.vBMD), and trabecular number (Tb.N) at the distal radius and tibia were higher in the highest sclerostin quartile versus the three lower quartiles combined. After adjustment for aBMD, men in the highest sclerostin quartile had higher Tb.vBMD (mainly in the central compartment) and Tb.N at both skeletal sites (p 63 years, aBMD was positively associated with serum sclerostin levels at all skeletal sites. Cortical vBMD (Ct.vBMD) and cortical thickness (Ct.Th) were lower in the first sclerostin quartile versus the three higher quartiles combined. Tb.vBMD increased across the sclerostin quartiles, and was associated with lower Tb.N and more heterogeneous trabecular distribution (higher Tb.Sp.SD) in men in the lowest sclerostin quartile. الوصف: Type 2 diabetes mellitus results in increased risk of fracture and delayed fracture healing. ZDF fa/fa rats are an established model of type 2 diabetes mellitus with low bone mass and delayed bone healing. We tested whether a sclerostin-neutralizing antibody (Scl-AbVI) would reverse the skeletal deficits of diabetic ZDF rats. Femoral defects of 3 mm were created in 11-week-old diabetic ZDF fa/fa and nondiabetic ZDF +/+ rats and stabilized by an internal plate. Saline or 25 mg/kg Scl-AbVI was administered subcutaneously (s.c.) twice weekly for 12 weeks (n = 9–10/group). Bone mass and strength were assessed using pQCT, micro–computed tomography (µCT), and biomechanical testing.
