الوصف: A low-cost portable analytical device, suitable for performing reflectance measurements on paper supports is proposed. It consists of a simple module consisting of a matrix of six types of photodiodes (six channels) with maximum sensitivity at six different wavelengths in the visible region. The device was used in combination with paper-based analytical devices (PADs) suitably prepared by 3D printing using polycaprolactone, i.e. a biodegradable and biocompatible polymer. These PADs were used both as substrates for the imbibition of colored solutions and of reagents for colorimetric reactions. The constructive and electronic aspects are described and the instrumental noise, as well as the performance of this device were carefully evaluated in terms of repeatability and reproducibility by using three different food dyes and a colorimetric assay for the determination of nitrites with the Griess reagent. The development of this device also included the management of output signals that, once converted from raw signals into reflectance values, have been elaborated by the Kubelka-Munk theory. Finally, the application to real samples consisting of commercial aqueous solutions of Tartrazine yellow (E102), Ponceau red 4R (E124) and Patent blue V (E131), usually adopted for the coloration and/or decoration of homemade cakes and sugarcoated almonds, gave results in good agreement with values obtained by absorbance measurements performed by a benchtop spectrophotometer. This type of comparison pointed out that also the nitrite detection performed by reflectance measurements on synthetic samples allowed more than reliable results to be obtained.

الوصف: Excimer Laser Coronary Atherectomy (ELCA) is a well-established therapy that emerged for the treatment of peripheral vascular atherosclerosis in the late 1980s, at a time when catheters and materials were rudimentary and associated with the most serious complications. Refinements in catheter technology and the introduction of improved laser techniques have led to their effective use for the treatment of a wide spectrum of complex coronary lesions, such as thrombotic lesions, severe calcific lesions, non-crossable or non-expandable lesions, chronic occlusions, and stent under-expansion. The gradual introduction of high-energy strategies combined with the contrast infusion technique has enabled us to treat an increasing number of complex cases with a low rate of periprocedural complications. Currently, the use of the ELCA has also been demonstrated to be effective in acute coronary syndrome (ACS), especially in the context of large thrombotic lesions.

الوصف: Portal vein thrombosis may occur in cirrhosis; nevertheless, its prevalence, and predictors are still elusive. To investigate this issue, the Italian Society of Internal Medicine undertook the “Portal vein thrombosis Relevance On Liver cirrhosis: Italian Venous thrombotic Events Registry” (PRO-LIVER). This prospective multicenter study includes consecutive cirrhotic patients undergoing Doppler ultrasound examination of the portal area to evaluate the prevalence and incidence of portal vein thrombosis over a 2-year scheduled follow-up. Seven hundred and fifty-three (68% men; 64±12years) patients were included in the present analysis. Fifty percent of the cases were cirrhotic outpatients. Viral (44%) etiology was predominant. Around half of the patients had a mild-severity disease according to the Child–Pugh score; hepatocellular carcinoma was present in 20%. The prevalence of ultrasound-detected portal vein thrombosis was 17% (n=126); it was asymptomatic in 43% of the cases. Notably, more than half of the portal vein thrombosis patients (n=81) were not treated with anticoagulant therapy. Logistic step-forward multivariate analysis demonstrated that previous portal vein thrombosis (p<0.001), Child–Pugh Class B+C (p<0.001), hepatocellular carcinoma (p=0.01), previous upper gastrointestinal bleeding (p=0.030) and older age (p=0.012) were independently associated with portal vein thrombosis. Portal vein thrombosis is a frequent complication of cirrhosis, particularly in patients with moderate–severe liver failure. The apparent undertreatment of patients with portal vein thrombosis is a matter of concern and debate, which should be addressed by planning interventional trials especially with newer oral anticoagulants. الوصف: Introduction: The radial artery is currently the most widely used access site for PCI procedures both acute and stable patient settings. Thanks to advantages in pharmacological therapy as well as in interventional devices, the rate of ischemic complications following PCI has significantly decreased. Nevertheless, this has been counterbalanced by an increased risk of periprocedural and late bleeding event, that can occur both at access and non-access sites. Choice of access site for PCI is of paramount importance to reduce the risk of access-related bleeding events. Areas covered: The aim of this review is to provide an overview of the actual available evidence comparing the transradial versus transfemoral approach to reduce hemorrhagic events. The most robust evidence comes from large randomized trials, partly also from observational registries, which compared the transradial and transfemoral approach. Expert opinion: Results show that radial access has proved to be decisive in reducing the incidence of hemorrhagic events. Furthermore, it showed a significant reduction in mortality and AKI compared to transfemoral access. However, increased experience in the use of the radial approach has led to less practice in the use of the femoral approach, which may be useful in cases of emergency, complications or inability to use the radial artery.

الوصف: This paper provides an overview of the approach we adopted as team Lunatic Goats for the ACM RecSys Challenge 2017 [7]. The competition, organized by XING.com, focuses on a cold start job recommendation scenario. The goal was to design and tune a recommendation system able to predict past users' interactions, for the offline stage, and to provide recommendations pushed every day to real users through the XING portal, for the online stage. Our strategy, which saw models coming from different techniques combined in a multi-layer ensemble, granted us the first place in the offline part and the qualification as second best team in the final leaderboard. All our algorithms mainly resort to content-based approaches, that, thanks to its ability to provide good recommendations even for cold-start items allowed us, quite unexpectedly, to achieve good results in terms of prediction quality and computational time.
