    الوصف: Amaç: Üçüncü basamak bir doğum hastanesinde bir yılda gerçekleşen sezaryen doğumların Robson On Gruplu Sınıflandırma Sistemi kullanılarak analiz edilmesi amaçlanmıştır. Gereç ve Yöntem: 1 Ocak 2019 - 31 Aralık 2019 tarihleri arasında hastanemiz doğum salonuna kabul edilip, hastanemizde doğum yapmış olan ve herhangi bir obstetrik risk faktö rü olmayan gebeler çalışmaya dahil edilmiştir. Çalışmamız retrospektif olarak dizayn edilmiştir. Bulgular: Çalışmada 10781 doğuma ait veriler incelendi. Bu doğumların 4391’i sezaryen doğum ile gerçekleşmiştir. Tüm doğumlar içerisinde sezaryen doğum oranı %40,7’dir. Sezaryen doğum yapan gebelerin %9,3’lük kısmı Robson sınıflandırmasına göre birinci grupta, %13,6’sı Robson sınıflandırılmasına göre ikinci grupta yer almıştır. Sezaryen doğum yapan gebelerin %12,1’lik kısmı Robson sınıflandırmasına göre üçüncü grupta, %10,8’i dördüncü gruptadır. Grup 5, grup 6 ve grup 7’nin tüm sezaryen doğumlardaki oranları sırasıyla % 47,7, %2,5 ve %2,6’dır. Robson sınıflandırmasına göre Grup 9 ’un tüm sezaryen doğumlar arasındaki oranı ise %1,4’tür. Sonuç: Sonuç olarak, hastanelerin yıllık Robson On Gruplu Sınıflandırma Sistemi Formundaki yüzdelere gö re sezaryen doğumlarının izlenmesi ve değerlendirilmesi, kendi sezaryen eylem planlarını hazırlamaları ve belirledikleri sezaryen hedefine gö re kalite notlarının değerlendirilmesi de sezaryen oranının azaltılması hedefine katkı sağlayacaktır.

    الوصف: Objectives Endometrial cancer is the most frequent tumor of the female genital tract. Ubiquitin is a small protein (8.5 kDa) found in all eukaryotic cells, binds to substrate proteins via a three-phase enzymatic pathway referred to as ubiquitination and plays an important role in cellular stability. Neural precursor cell-expressed developmentally down-regulated 4-like (NEDD4L) functions in the last phase of this enzymatic process. In this study, we investigated NEDD4L protein expression in endometrial cancer. Methods The study participants were divided into patients with benign endometrial pathologies (Group 1, n = 23), patients with endometrial hyperplasia (Group 2, n = 21) and patients with endometrial cancer (Group 3, n = 20). NEDD4L expression was detected by immunohistochemical staining and H scores were calculated to standardize staining intensity. Statistical analysis was performed using SPSS 16.0. Results NEDD4L expression levels according to H scores were significantly lower in patients diagnosed with endometrial cancer compared with those with benign endometrial pathologies. Conclusion NEDD4L is involved in maintaining cell stability, and reduced NEDD4L expression as a result of gene mutation may disrupt this balance in favor of tumorigenesis.

    الوصف: İlk olarak Çin'in Wuhan kentinde tanımlanan yeni tip koronavirüs hastalığı (COVID-19), küresel bir halk sağlığı sorunu olan, bir çeşit solunum yolu enfeksiyonudur. Vaka raporları ve küçük kohort çalışmalarından oluşan, gebelikte COVID-19 ile ilgili veriler sınırlıdır. Doğum sonrası ürosepsis de küresel olarak yaygın bir hastalıktır ve pekçok yayın bulunmaktadır. Sepsis bulguları olan postpartum hastalar, salgın sırasında; halk sağlığı ve hastanın kendi iyiliği açısından COVID-19 için test edilmelidir. Ürosepsisin klinik özellikleri, doğum sonrası hastalarda COVID-19 hastalığına benzer olabilir. Bu olgu sunumu ile; postpartum dönemde ani akut solunum bozukluğu ve septik şok bulguları geliştiren bir vakada COVID-19 enfeksiyonu ve ürosepsis ayırıcı tanısının ve tedavisinin nasıl yönetildiğini anlatmak amaçlanmıştır.

    الوصف: Background/Aim: Ovarian cancer is the second most common gynecologic malignancy worldwide and is the deadliest among gynecological cancers. It is important that this cancer, which is usually diagnosed in advanced stages, is referred to a gynecologist oncologist without delay. An ideal screening method does not yet exist. Although CA 125 is still the most used tumor marker, it cannot detect early-stage ovarian cancer. Also, CA 125 is not specific for ovarian malignancy. Therefore, new serum markers, such as HE4, and more complex algorithms, like ROMA and RMI, have emerged. Here we evaluate the preoperative potential of patients with adnexal mass to have a malignant or benign mass with morphological index, CA 125, HE4, RMI, and ROMA tests. Methods: This study is a prospective cohort study. A power analysis was done before starting the study. The sample size was at least 80 when the Type I error was set at 0.05, and the confidence interval was 95%. We included into the study 84 patients admitted to our clinic because of pelvic mass and underwent operation between March 2016 and October 2018. To homogenize the benign and malignant groups, 42 patients were collected from each group. CA 125 and HE4 levels of the samples were studied by the electrochemiluminescence method. ROMA and RMI values were calculated, and the data were entered into SPSS. Data were analyzed using SPSS 22.0 statistical package program. Results: Each of the CA 125 (P = 0.002), HE4 (P < 0.001), morphological index (P < 0.001), ROMA (P < 0.001), and RMI (P < 0.001) tests has been successful in differentiating malignant masses from benign masses. In the malignant-benign differentiation of adnexal masses preoperatively, CA 125 was the test with the lowest sensitivity, and RMI had the highest sensitivity. However, in the ROC analysis, the morphological index has a higher area under the curve.Conclusion: Although CA 125 is still the most frequently used marker in the preoperative evaluation of adnexal masses, it has low specificity and sensitivity, especially in premenopausal patients. The use of new tumor markers (e.g., HE4) and other algorithms (e.g., ROMA and RMI) is supported by our findings and the literature. However, here we show that an expert ultrasonographic evaluation with morphological index alone could be effective.

    الوصف: Aim: To test the hypothesis that altered A Disintegrin and Metalloproteinase Domains with Thrombospondins motifs (ADAMTS) is implicated in the etiopathogenesis of gestational diabetes mellitus (GDM). Methods: All pregnant women who underwent elective amniocentesis for karyotype analysis between January 1, 2016, and January 1, 2018, were included in this study. From this cohort, the study group consisting of 20 patients diagnosed with GDM was selected and compared against a control group consisting of 20 age- and body mass index (BMI)-matched patients without GDM. ADAMTS4, ADAMTS5, interleukin-6 (IL-6) and tumor necrosis factor alpha (TNF-α) levels were compared in the second trimester amniotic fluid of patients with GDM and normoglycemic pregnant women. Results: No significant differences were observed between GDM and control groups regarding age, BMI, gestational age at amniocentesis and indication for amniocentesis. Mean amniotic fluid ADAMTS4 and ADAMTS5 levels were significantly increased in the GDM group compared with the control group (253.5 ± 18.7 pg/mL and 188.5 ± 21.3 pg/mL, P < 0.001; 192.9 ± 16.4 pg/mL and 154.8 ± 19.9 pg/mL, P = 0.021, respectively). Significant increases in IL-6 and TNF-α levels were also detected in the amniotic fluid of GDM patients relative to controls (136.2 ± 17.3 pg/mL and 98.3 ± 11.5 pg/mL, P < 0.001; 154.2 ± 12.5 pg/mL and 86.2 ± 10.8 pg/mL, P < 0.001, respectively). Conclusion: The data presented here suggest that increased levels of ADAMTS4, ADAMTS5, IL-6 and TNF-α may play an important role in the progression of GDM. © 2019 Japan Society of Obstetrics and Gynecology
