    مستخلص: Background: Due to the increase of the elderly's population and related social and economic problems, it is very important to provide strategies on health. In this regard, induction of T lymphocytes responses, the most important cells of the immune system, may be a good approach. Among different agents considered as antiaging factors, mTORC1 pathway inhibitors are significant. So, the purpose of this study was to evaluate the effect of two mTORC1 inhibitors, Everolimus and Metformin, on age-related features of activated T cells. Materials and Methods: Optimum doses of drugs was determined with evaluating the effect of treatments on IL-2 gene expression. T cells isolated from old and young mice were treated with drugs and PHA. IL-2 production was evaluated by ELISA. Also, the expression of CD28, PD-1, and KLRG-1, proliferation, and intracellular oxidative stress were assessed by flow cytometry-based assays, phenotyping, CFSE, and DCF-DA assay respectively. Results: Both drugs increased IL-2 production in the T cells of old mice. Also, using drugs especially Metformin could improve age-related phenotypical markers and increase the proliferation of T cells of old mice significantly. In addition, Metformin and Everolimus reduced intracellular oxidative stress in aged cells. However, the effect of both drugs on the T cells of young mice wasn't significant or was in opposite to the results of old mice T cells. Discussion: In line with studies noting mTOR inhibitors as antiaging drugs, Metformin and Everolimus may improve T cells affected from aging in vitro, and a decrease in intracellular oxidative stress may be one of their mechanism of function.     مستخلص: In this study, the pollution of heavy metals in the sediments of the Otaghvarrud, Shalmanrud, and Polrud rivers was assessed to determine the geochemical and environmental contamination. Samples were collected from 19 stations along these rivers. The investigation focused on six heavy metals: arsenic (As), lead (Pb), chromium (Cr), nickel (Ni), copper (Cu), vanadium (V), and zinc (Zn), using ICP-OES analysis. Various statistical parameters, including the Pearson coefficient, dendrogram, and principal component analysis (PCA), were evaluated. The results indicated a high correlation among Cu, Ni, V, Zn, and Cr, suggesting a geogenic origin. As exhibited characteristics of a combination of anthropogenic, geogenic, and metal origins, while Pb showed an anthropogenic origin likely from agriculture and industry. Additional parameters, such as the mean of geoaccumulation index (Igeo), enrichment factor (EF), contamination factor (CF), pollution load index (PLI), Nemerow integrated pollution index (NIPI), ecological risk (ER), and environmental risk index (RI), were assessed to determine the pollution levels of the heavy metals. The findings revealed low to medium pollution for the analyzed metals. Notably, Otaghvarrud exhibited higher pollution levels compared to the other rivers due to the presence of basalts, pillow lavas, and splits along its route. These geological features contributed to the transport and deposition of these metals in the river. Based on the research findings, it was concluded that the sediments in the studied rivers are on the verge of heavy metal pollution. Furthermore, an examination of the health risk assessment indicators, including the HI (hazard index) and CRI (carcinogenic risk index), indicated a higher potential risk of carcinogenic and non-carcinogenic diseases in children compared to adults.     الوصف: Background: The medical profession has always been an inspiration for human societies throughout its diverse history. This position and historical authority in the field of ethics has had a different and higher status, in such a way that many of the norms of general ethics and professional ethics, especially principles, such as trust, confidentiality and respect for human dignity, have been developed by medical professionals. Developing guidelines of general and professional ethics is one of the inherent duties of the Medical Council of the Islamic Republic of Iran (IRIMC) as a professional organization. In this regard, the Supreme Council of IRIMC has approved the "Code of Ethics for Medical Professionals"and, in accordance with its legal authority, has annexed it to the disciplinary regulations of IRIMC. Methods: A draft document, the result of extensive literature review, was discussed in 27 expert panel meetings and after receiving and endorsing the stakeholders' point of view, was approved by the IRIMC Supreme Council. Results: The first edition of "Code of Ethics for Medical Professionals, Medical Council of Islamic Republic of Iran"was developed on July 6, 2017 by the Supreme Council of IRIMC. The guideline was set to take effect one year after its enactment. The first edition was revised and completed and final edition was adopted on August 9, 2018 by IRIMC in 13 chapters and 140 articles (original full text is available in the Supplementary file 1). Conclusion: According to the approved decision by the Supreme Council of IRIMC on May 10, 2018, the final edition takes effect as of October 7, 2018. © 2020 The Author(s).

    الوصف: Radon gas is one of the most influential sources of indoor exposure. All its physical properties together make it a significant risk factor for lung cancer in the population. The research aims are outlined as (1) to measure the radon concentration in Tehran city and compare results with the international standards (2) to determine spatial distribution of radon gas concentration using Geographical Information System (GIS) software and (3) to estimate the annual effective dose and potential risk of lung cancer by radon-222 in Tehran city. In this study, 800 Alpha Track detectors were installed in houses in 22 regions of Tehran city and retrieved after 3 months. The measurements were repeated for spring and summer and autumn seasons. The annual effective dose and risk of lung cancer were assessed using standard equations. Data were analyzed using SPSS 20. Result showed the minimum and maximum radon concentration were observed in and Ghalee-kobra (0.13 Bq.m-3) and Charbagh-ponak district (661.11 Bq.m-3) respectively. There was no observed relationship between radon concentration and houses' model, cracking condition and constructionn materials. Expectedly, the storehouses and basements had significantly higher (P = 0.016) radon concentration than occupied rooms. The min and max of the estimated annual effective dose were 0.65 and 2.03 mSv, respectively. Result showed that around 5 of the sampling sites had higher level of radon than the maximum allowed by EPA. A rough estimation of the expected radon-attributed lung cancer incidences yielded approximately 5958 cases in the total population of Tehran every year. In view of the growing trend in cancer incidences, appropriate measures addressing radon should be undertaken in areas of increased exposure to this noble gas. © 2020 Springer Nature Switzerland AG.

    الوصف: Background: Respecting patients' privacy is an essential professional responsibility for physicians and other health team members. In this regard, this study investigates medical students' knowledge and attitude about confidentiality and disclosure of patients' information. Methods: In this cross-sectional study, 160 medical students of Iran University of Medical Sciences participated who were selected using stratified random sampling. Data were gathered using a valid and reliable self-report questionnaire. Student's knowledge and attitude toward medical confidentiality were assessed using self-administered and researcher-made questionnaires. Cronbach's alpha coefficients for knowledge and attitude levels were 79.7 and 82.2, respectively. Results: The average of medical students' responses to knowledge and attitude questions were 56.6 (9.6/ 17) and 55.3 (9.4 out of 17), respectively. On average, females had an acceptable attitude about 57.5 of the questions, whereas this was 50.9 for males. On average, females had an acceptable knowledge about 59.5 of the questions, whereas this was 50.6 for males. Therefore, female's attitudes and knowledge were more correct than their male counterparts (p < 0.001). Conclusion: The low level of knowledge and attitude of medical students towards medical confidentiality indicates that revision of Iranian medical education curriculum to reinforce attention and knowledge of medical students on this issue to render appropriate care to patients is a necessity. Medical students' knowledge and attitude towards patient's confidentiality rights is not fulfilling. © Iran University of Medical Sciences.

    الوصف: It has long been a common goal for both medical educators and ethicists to develop effective methods or programs for medical ethics education. The current lecture-based courses of medical ethics programs in medical schools are demonstrated as insufficient models for training â��good doctorsâ��. In this study, we introduce an innovative program for medical ethics education in an extra-curricular student-based design named Studentsâ�� Medical Ethics Rounds (SMER). In SMER, a combination of educational methods, including theater-based case presentation, large group discussion, expert opinions, role playing and role modeling were employed. The pretest-posttest experimental design was used to assess the impact of interventions on the participantsâ�� knowledge and attitude regarding selected ethical topics. A total of 335 students participated in this study and 86.57 of them filled the pretest and posttest forms. We observed significant improvements in the knowledge (P < 0.0500) and attitude (P < 0.0001) of participants. Interestingly, 89.8 of participants declared that their confidence regarding how to deal with the ethical problems outlined in the sessions was increased. All of the applied educational methods were reported as helpful. We found that SMER might be an effective method of teaching medical ethics. We highly recommend the investigation of the advantages of SMER in larger studies and interdisciplinary settings. © 2016 Medical Ethics and History of Medicine Research Center, Tehran University of Medical Sciences.     الوصف: Background: Professionalism is a core competency of physicians. This study was conducted to design a model for formative assessment of professional commitment in medical students according to stages of change theory. M ethods: In this qualitative study, data were collected through literature review & focus group interviews in the Tehran University of Medical Sciences in 2013 and analyzed using content analysis approach. Results: Review of the literature and results of focus group interviews led to design a formative assessment model of professional commitment in three phases, including pre-contemplation, contemplation, and readiness for behavior change that each one has interventional and assessment components. In the second phase of the study, experts' opinion collected in two main categories: the educational environment (factors related to students, students' assessment and educational program); and administrative problems (factors related to subcultures, policymakers or managers and budget). Moreover, there was a section of recommendations for each category related to curriculum, professors, students, assessments, making culture, the staff and reinforcing administrative factors. Conclusion: This type of framework analysis made it possible to develop a conceptual model that could be effective on forming the professional commitment and behavioral change in medical students.

    الوصف: Clinical education is an essential part of medical traineesâ�� education process, and curriculum planners agree that it should be based on ethical standards and principles in the medical field. Nevertheless, no explained and codified criteria have been developed for ethics in clinical teaching. This study was aimed to develop an ethical guideline for medical students and teachers as the first and most important step in respecting patients' rights in educational centers. The initial draft included the codes of ethics in clinical education and was developed based on library studies. Subsequently, it was improved through a qualitative study using semi-structured interviews and focus group sessions with medical students, patients, and medical teachers in educational hospitals affiliated to Tehran University of Medical Sciences. The improved draft was reviewed and validated by a medical expert panel to prepare the final draft. The codes derived from this study included patientsâ�� choices and rights in purely educational procedures, and special considerations for a) obtaining informed consent for educational procedures; b) performing procedures on deceased persons, patients under anesthesia and those lacking decision making capacity; c) educational visual recordings of the patients; and d) safety monitoring in clinical education. The guideline developed in this study incorporates codes of ethics into clinical training. Therefore, in addition to providing efficient education, the interests of patients and their rights are respected, and the ethical sensitivity of learners in primacy of patientsâ�� best interests will be preserved and enhanced. © 2015 Fariba Asghari et al.
