In an earlier pilot study, we found that treatment with the serotonin norepinephrine reuptake inhibitor antidepressant, milnacipran, significantly improved metabolic parameters in diabetic patients with comorbid depression who had an antidepressant response. We sought to replicate these results in a larger cohort (n = 135). Patients received milnacipran and metformin for 6 months and metabolic parameters and depressive symptoms were measured at baseline and after 3 and 6 months. At the end of the study, 72.6% of patients had an antidepressant response (≥50% reduction of baseline Beck Depression Inventory score). Overall, there was significant improvement in the metabolic and anthropometric parameters measured. The number of patients with glycated hemoglobin > 8% (>63.9 mmol/mol), an indicator of poor metabolic control requiring intensive therapeutic intervention, decreased from 31.9% at baseline to 11.9% during the study. As found in the pilot study, levels of total cholesterol and triglycerides were only significantly decreased in antidepressant responders. Body weight was significantly reduced in both responders and non-responders but the effect size was significantly greater in the responder group. In contrast to the pilot study, fasting blood glucose and glycated hemoglobin were significantly decreased to a similar extent in both antidepressant-responders and nonresponders. The present study thus replicates some of the original findings. The main difference between the present and the pilot study is that in the larger cohort significant reductions in fasting blood glucose and glycated hemoglobin were found in all patients irrespective of whether or not they responded to antidepressant treatment. The present data underline the importance of diagnosis and treatment of comorbid depression in patients with type 2 diabetes mellitus with milnacipran.Keywords: depression, diabetes, milnacipran, metabolic control, metformin

الوصف: Impaired regulation of extracellular matrix remodeling by matrix metalloproteinases (MMPs) and tissue inhibitor of metalloproteinase (TIMP) may contribute to vascular complications in patients with type 1 diabetes. We investigated associations between plasma MMP-1, -2, -3, -9, -10 and TIMP-1, and cardiovascular disease (CVD) or microvascular complications in type 1 diabetic patients. We also evaluated to which extent these associations could be explained by low-grade inflammation (LGI) or endothelial dysfunction (ED). Methods: 493 type 1 diabetes patients (39.5 ± 9.9 years old, 51% men) from the EURODIAB Prospective Complications Study were included. Linear regression analysis was applied to investigate differences in plasma levels of MMP-1, -2, -3, -9, -10, and TIMP-1 between patients with and without CVD, albuminuria or retinopathy. All analyses were adjusted for age, sex, duration of diabetes, Hba1c and additionally for other cardiovascular risk factors including LGI and ED. Results: Patients with CVD (n = 118) showed significantly higher levels of TIMP-1 [β = 0.32 SD (95%CI: 0.12; 0.52)], but not of MMPs, than patients without CVD (n = 375). Higher plasma levels of MMP-2, MMP-3, MMP-10 and TIMP-1 were associated with higher levels of albuminuria (p-trends were 0.028, 0.004, 0.005 and 0.001, respectively). Severity of retinopathy was significantly associated with higher levels of MMP-2 (p-trend = 0.017). These associations remained significant after further adjustment for markers of LGI and ED. Conclusions: These data support the hypothesis that impaired regulation of matrix remodeling by actions of MMP-2, -3 and-10 and TIMP-1 contributes to the pathogenesis of vascular complications in type 1 diabetes.

الوصف: Für Patienten mit Diabetes mellitus kann das Lenken eines Kraftfahrzeuges durch das Auftreten von Hypoglykämien mit erheblichen Gefahren für den Lenker und für andere Verkehrsteilnehmer verbunden sein. Insbesondere bei Vorliegen einer Wahrnehmungsstörung für Symptome der Hypoglykämie, wie dies bei Patienten mit Insulintherapie der Fall sein kann, ist eine genaue Evaluierung zur Beurteilung der Lenkfähigkeit erforderlich. Auch bestimmte Spätschäden des Diabetes mellitus können zu einer Einschränkung der Fahrtauglichkeit führen, sodass eine genaue Erhebung von Retinopathie, Nephropathie, Neuropathie und Atherosklerose sinnvoll ist. In vielen Fällen ist das Einholen einer fachärztlichen Stellungnahme zur Beurteilung der Lenkfähigkeit für die Amtsärzte unumgänglich. Die Europäische Union publizierte 2012 eine Empfehlung zur Umsetzung der Fahrbeschränkungen für verschiedene Krankheitsbilder. Basierend auf dieser Empfehlung entstand im Auftrag des Bundesministeriums für Verkehr, Innovation und Technologie (BMVIT) unter der Leitung des Kuratoriums für Verkehrssicherheit im Rahmen einer Arbeitsgruppe eine Leitlinie für Amts- und Fachärzte zur Umsetzung dieser Empfehlungen. Diese Leitlinien sollen den Umgang mit der komplexen Problematik Diabetes und Führerschein vereinfachen.

الوصف: Das metabolische Syndrom ist eine relativ häufige Stoffwechselerkrankung mit in der Zukunft wachsender Prävalenz. Die Parameter des metabolischen Syndroms sind gut definiert und werden ständig aktualisiert. Die wichtigsten Parameter sind: viszerale Adipositas, gestörte Glukosetoleranz, erhöhte Triglyzeridwerte, niedrige HDL-Cholesterinspiegel und Hypertonie. Sexuelle Dysfunktion kommt bei Menschen, die an metabolischem Syndrom leiden, häufig vor. Zumindest 30 % der vom metabolischen Syndrom Betroffenen leiden auch an sexueller Dysfunktion. Sowohl das metabolische Syndrom als auch die sexuelle Dysfunktion sind häufig mit niedrigen Testosteronspiegeln assoziiert. Bis dato ist nicht klar, ob Hypoandrogenismus das metabolische Syndrom verursacht oder vice versa. Interessant ist, dass Hypoandrogenismus häufig verschiedenen Stoffwechselstörungen wie Adipositas, Insulinresistenz und metabolischem Syndrom vorausgeht. Diese Tatsache macht wahrscheinlich, dass Hypoandrogenismus und metabolisches Syndrom gemeinsame Wurzeln haben und auch das gleiche Risikofaktorprofil teilen. Die sehr komplexen pathophysiologischen Mechanismen der hypothetischen Kausalität sind bei weitem noch nicht geklärt. Endotheliale Dysfunktion als Präkursor der Atherosklerose scheint dabei eine wesentliche Rolle für beide metabolisches Syndrom und sexuelle Dysfunktion zu spielen. Die intakte endotheliale Funktion ist Voraussetzung für den Funktionsablauf der Erektion. Rezente Studien beleuchten unter anderem auch diesen Zusammenhang. Abschließend kann festgestellt werden, dass sowohl Hypoandrogenismus als auch endotheliale Dysfunktion offenbar eine signifikante Rolle bei der Entstehung des metabolischen Syndroms spielen. Weitere Studien sind notwendig, um den hormonellen und metabolischen Hintergrund dieser komplexen Erkrankung endgültig auszuleuchten und die Sinnhaftigkeit gezielter therapeutischer Interventionen wie z. B. Testosteronsubstitution zu untersuchen.
