    مستخلص: AIM: The main objective is to investigate the value of the treadmill exercise test (TET) in relation to myocardial bridge through various clinical characteristic and risk factor for guiding the clinical therapy of myocardial bridging (MB) patients. METHOD: This was cross-sectional, single centre, observational prospective study conducted from January 2022 to June 2023. Total coronary angiography was performed on 4,321 cases of angina pectoris and suspected myocardial infarction at our hospital, 104 cases were confirmed as myocardial bridges, and exercise stress test was performed on 44 cases among them was investigated and evaluated the relationship between the TET results and the patients clinical characteristics, including sex, age, bridge length, reference diameter in systole and diastole and the degree of systolic stenosis. RESULTS: No Statistical differences were found between baseline clinical characteristic (age, sex, clinical features, ECG findings) between the Nobel I with less than 50% of systolic stenosis and the Nobel II with more than 50% of systolic stenosis. Quantitative coronary data in study population mean ref. vessel diameter was 2.41±0.33 mm, and the diastolic vessel diameter was 2.16±0.44 mm, 85% of the ref. vessel diameter, and the systolic vessel diameter was 1.16±0.43 mm, 46% of the ref. vessel diameter. The average systolic stenosis rate was 45±18% on average. The average myocardial bridge length was 11.86 ±4.92 mm, which did not match the degree of systolic compression. 44 patients performed the Treadmill exercise test in which 34 cases (77%) positive and 10 cases (23%) negative. No relationships were found between the quantitative coronary data, bridge length, lesion location, bridge compression, and Treadmill exercise test results and between the positive and negative groups. CONCLUSION: Our study found that patients with MB had high positive rate of treadmill exercise test. it is judged that various mechanisms other than simple mechanical compression may be involved as the cause of myocardial ischemic symptoms. The treadmill exercise test can improve our understanding of the characteristics of blood supply to them yo cardium,clarify whether MB can cause myocardial ischemia and help develop appropriate treatment strategies for MB patients [ABSTRACT FROM AUTHOR]

    مستخلص: Background: Cardiovascular diseases (CVD) such as ischemic heart disease (IHD) and cerebrovascular accidents such as stroke lead to 17.7 million deaths and are the leading cause of death worldwide. Global Burden of Disease study found that age-standardized CVD death rate in India (272/100,000 population) is much higher than the global average (235/100,000 population). Aims and Objectives: This study aimed to compare among different coronary risk factors like dyslipidemia, diabetes mellitus hypertension (HTN), and obesity which may lead to coronary artery disease (CAD) and early detection of ischemic heart disease by the stress treadmill test (TMT) among the patients having coronary risk factors. Materials and Methods: This is a prospective and observational study among 170 patients of age group 18-60 years having one or more coronary risk factors such as dyslipidemia, HTN, diabetes mellitus, and obesity. Patients with diagnosed IHD and other chronic debilitating diseases were excluded from the study. Results: Out of 170 patients, 153 patients were males and 17 patients were females. Among all these patients, 42 patients were suffering from dyslipidemia, 44 patients were suffering from Type 2 diabetes, 45 patients were suffering from HTN, and 39 patients were having more than one risk factor. All 170 patients with coronary risk factors performed TMT. Out of those patients, 53 were found to be TMTpositive. Binary regression analysis showed among all the selected risk factors, dyslipidemia (P<0.05; odds ratio [OR]-2.104) and HTN (P<0.05; OR-2.159) were found to be significantly associated with the positive outcome of TMT. All 53 TMT-positive patients were selected for CAG. Out of those patients, 37 were found to be suffering from CAD. Binary regression analysis showed among all the selected risk factors, dyslipidemia (P<0.05; OR-1.964) and HTN (P<0.05; OR-2.125) were found to be significantly associated with the positive outcome of CAG. Conclusion: Dyslipidemia and HTN were found to be significantly associated with the TMT-positivity as well as CAD. Diabetes mellitus and obesity were not statistically significant, though OR was more than 1 for both these risk factors. [ABSTRACT FROM AUTHOR]

    مستخلص: Introduction: Diabetes mellitus, is an ever progressive, disease. It is also considered as coronary artery disease equivalent. People with type 2 diabetes mellitus (T2DM) have a higher cardiovascular morbidity and mortality, and are disproportionately affected by CVD compared with non-diabetic subjects1,8. Diabetic vascular disease is responsible for two to four-fold rise in the occurrence of coronary artery disease (CAD) and stroke, and two-eightfold improve in the risk of heart failure2. It affects roughly one-third of all acute myocardial infarction patients. According to the global burden of disease assessment, India is expected to have significant increase in diabetes prevalence and subsequently coronary artery disease. Materials and Methods: This cross-sectional study was done at Venkateshwara institute of medical sciences, Gajraula among 50 patients who attended the institute (both in and out patient between January 2022 and august 2022. Initial step consisted of consent for study followed by medical history, physical and systemic examinations. After the preliminaries, routine and detailed investigations were done. All those who were asymptomatic with regard to cardiac symptoms such as dyspnea, chest pain etc and were having non-insulin dependent diabetes mellitus for one year or more duration were included. Investigations done included CBC, Renal function test, lipid profile, fasting and post prandial blood sugar, fundus examination to assess end organ damage, ECG, echocardiography and treadmill stress test or dobutamine stress test. Results: This study included 50 NIDDM patients. There was a little difference among males (n=28) and females (n=22) in the study which correlates with other studies that there is no sex difference in the incidence of Diabetes. With regard to age group 30 patients (60%) belong to 5th decade. 40 % patients In this study (20 out of 50) had diabetes less than 5 years, 20 patients (40 %) had diabetes of 5-10-year duration and 10 patients (20%) had diabetes more than 10 years. 14 (28%) patients showed silent myocardial ischemia on TMT or Stress echo. 2 patients (4%) developed minor angina on stress test but were able to complete the test. 3 (6%) patient showed baseline regional wall motion abnormality with/or LV systolic dysfunction. Conclusion: There was only minor difference in gender distribution in favor of males. As per age distribution the majority of the patients were in 5th decade. Our study showed lipid abnormalities included Hypertriglyceridemia as most prevalent 25 (50%), however LDL level was more than 130mg/dl in 20 (40%) of patients as well. With stress tests, 28% (14 out of 50) of patients had asymptomatic inducible ischemia and 2 patients (4%) developed minor angina on stress test but were able to complete the test. 3 (6%) patient showed baseline regional wall motion abnormality with/or LV systolic dysfunction.     مستخلص: Background: Heart rate variability (HRV) is becoming one of the most valuable tools for assessing a healthy heart's complex and constantly changing oscillations. This study was a pioneering attempt to establish normative data on HRV during treadmill exercise for monitoring the cardiovascular health of the central Indian population. Methodology: This was a cross-sectional study in the Sports Physiology Laboratory of the Department of Physiology in a Rural Medical College in central India. One hundred and twenty healthy subjects in the age range 17-40 years were recruited. Short-term HRV (5 min) was extracted from ECG recordings obtained usingthe Power lab system, AD Instruments, Australia. Results: Time domain indices for males were - Standard deviation of N-N interval (SDNN): 162.61±162.11; Square root of mean squared difference of N-N intervals (RMSSD): 355.79±798.27; the percentage of adjacent NN intervals that differ from each other by more than 50 ms (pNN50): 23.10±27.87. Frequency domain indices in males were-LF power (%)-535.74±3625.96; HF power(%) - 33.15±24.31, LF nu: 33.12± 16.06; HF nu: 57.22±14.89; LF/HF:0.77±0.74. Time-domain indices for females were SDNN as 168.49±130.09; RMSSD: 182.41±154.85; pNN50:32.33±26.59. Frequency domain indicesin females were LF power (%)-19.85±6.13; HF power (%) 43.03±16.39, LF nu-30.53±9.88; HF nu - 60.95±8.70; LF/HF:0.54±0.27. Conclusion: Baseline normative values for HRV spectral and time-domain analysis have been established for their clinical use in comparing the HRV of a healthy individual to that of a deceased individual or an athlete.     مستخلص: The objective of this study was to examine the influence of fear of falls (FOF) and history of falls on gait parameters in community-dwelling older adults. A total of 659 active adults above 60 years of age participated in this cross-sectional study Demographic details, FOF, and history of falls (HOF) were recorded using a semi-structured questionnaire. Gait parameters were measured using wearable sensors during the 'timed- up and go test. Mean differences in gait parameters of those with and without fear of fall and fall history were calculated using one-way ANO VA. FOF was reported by 21.7 per cent and a history of falls was reported by 24.7 percent of the participants. Participants with both history and fear of falls had significantly slower stride velocity, decreased cadence, increased gait cycles, time taken to stand, walk time, and turn time (p< 0.05) as compared to those without a history of falls and fear of fall. Therefore, FOF can be used as a marker to assess the risk of gait impairments. in low-resource settings, FOF can be employed in routine clinical examination to identify older adults at risk of falls and developed interventions targeting FOF and gait parameters.     مستخلص: Osteoarthritis (OA) is the most common in India. The incidence of osteoarthritis in adults over the age of 45. Although there is no evidence to suggest that genetics, heredity, histology and biochemistry play a strong role in its development. To date no cure for the disease exists. However, it has been proven that risk factors for the onset and progression of disease can be decreased or avoided through lifestyle modifications such as weight loss, increased physical activity and dietary changes. Twenty subjects were divided into two Groups. Training exercise group 7 subjects and functional task training 13 subjects. Baseline measures for performance outcome measures were insignificant between groups for all of the following: Berg Balance Scale (BBS), (p=0.304), stair climb test (SCT), (p=0.567), timed up and go test (TUG), p=0.970.
