Acute aortic syndrome comprises life-threatening situations that classically were known as acute aortic dissection-penetrating aortic ulcer, intramural hematoma, aortic dissection, and contained aortic rupture. All these entities are caused by the rupture of aortic media and that is why severe pain is the main symptom in these patients, and, unlike the pain in myocardial infarction, this pain is severe from the start. Although some patients do not have pain—Marfan syndrome, diabetic patients—pain localization may help diagnose which part of the aorta is dissected. Any patient suspected of this entity should undergo a diagnostic test that is accessible and with good results: contrast-enhanced computed tomography, magnetic resonance imaging, or ultrasonography, all of them having diagnostic accuracy near to 100%. Based on the imaging results, a quick, reliable, and often life-saving decision is available for these patients, suggesting surgical or medical management. |
