الوصف: Background: Scant data are available on nutritional status in bilateral lung transplant (BLT) candidates. Methods: All consecutive recipients admitted to the intensive care unit (ICU) of the University Hospital of Padua (February 2016–2020) after bilateral-lung transplant (BLT) were retrospectively screened. Data collected: (i) nutritional indices (body mass index (BMI), albumin level, prognostic nutritional index (PNI), mini nutritional assessment short-form (MNA-SF)); and (ii) muscular indices (creatinine height index (CHI)), skeletal muscle index (SMI), densitometry of paravertebral muscles on chest CT). Results: 108 BLT recipients were enrolled: 55% had a normal BMI, 83% had serum albumin levels > 35 g/L; high PNI and MNA-SF scores were recorded in most of patients. A total of 74% had a “normal or slightly reduced protein state“ according to the CHI score; 17% were identified as “sarcopenic” according to muscle densitometry (Hu < 30). Lower serum albumin was associated with longer invasive mechanical ventilation days (IMV) and ICU length of stay (p-value for non-linearity < 0.01). PNI and BMI were also associated with an increased ICU length of stay (p-value for non-linearity < 0.01). Conclusions: Most of the BLT recipients had normal nutritional and sarcopenia status. Pre-transplant albumin values correlated with the duration of IMV; serum albumin, PNI and BMI were associated with ICU stay. No nutritional or muscle parameters predicted re-intubation, 30-days rejection and overall length of hospital stay.

الوصف: Background: Chronic groin pain (CGP) is a syndrome characterized by pain in the pubic and inguinal-crural regions, resulting in a functional deficit that can lead to severe impairment of different motor tasks. Objective: The main objective of this study is to describe and analyze an algorithm used for the assessment and treatment of chronic groin pain in young sportsmen used for 10 years. Methods: Descriptive, observational, and retrospective study, adapted with recommendations of the STROBE Declaration (Strengthening Reporting of Observational Studies in Epidemiology) for its design. The study was carried out at an Hernia Pathology Center. It is a multidisciplinary team formed by surgeons, physiotherapists, orthopedists, and imaging specialists, with extensive experience in the research field. This team had developed an assessment, diagnosis, and treatment algorithm for CGP which was implemented for more than 15 years. Results: After clinical examination and complementary imaging, 3,858 patients were included to follow the algorithm. A total of 3,289 patients completed the sports physiotherapy and rehabilitation protocol. The most frequent clinical entities registered were tendinopathies: 1,649 iliopsoas-pectineus-related groin pain (42.7%) and 1,522 adductor-related groin pain (39.5%). A total of 569 patients with sportsman’s hernia diagnosis required surgical intervention. There were no intraoperative complications; furthermore, all patients discharging after 6 hours of hospital stay. Conclusions: The assessment, diagnosis, and treatment algorithm used by a multidisciplinary team to treat patients with chronic groin pain has proven to be safe and successful.

الوصف: INTRODUCTION: The development of minimally invasive procedures and analgesic drug evolution has contributed to reduce the rest period after inguinal hernia repair; nevertheless, there is still no scientific evidence to establish optimal postsurgery rest time and sequence for resuming activity until reaching full performance. Early and controlled rehabilitation by physical therapists has proven beneficial in postsurgery for athlete groups, although no publications are available on experiences in nonathlete patients. OBJETIVE: The aim was to analyze the results of a rehabilitation program applied to post-transabdominal preperitoneal (TAPP) hernioplasty in non-athletic patients. PATIENTS AND METHODS: The first 1,000 nonathlete patients who performed the postoperative sports rehabilitation plan from January 2012 to December 2016. We used TAPP laparoscopic hernioplasty. Postsurgery exercise program guided by objectives and supervised by physical therapists in four growing-intensity phases. Postsurgery complications, hernia recurrence, pain intensity (numeric scale) while exercising at 2 and 10 days after surgery; start time and duration for the first phase in the rehabilitation plan; and timing of return to work were evaluated. RESULTS: The objectives of the first phase were reached by 92% of the patients on the 7th day. Work activities started in 97% of the patients before the 5th day. CONCLUSIONS: Application of a sports rehabilitation program does not increase the recurrence rate nor complications and may speed up the return to full physical activity in nonathletic patients.

الوصف: ABSTRACT PURPOSE: To assess the effect of aqueous extract of Peumus Boldus (AEPB) on the liver proliferative response after parcial hepatectomy of 70% (PH) in rodents. METHODS: Twenty Wistar rats were divided in two groups: AEPB100 (whose rats received 100mg/Kg of AEPB, once a day, orally, in 4 days prior to the first surgical procedure) and Vehicle (whose rats were treated similarly with distilled water). Both groups underwent PH. After 24 hours the remaining livers were removed for studying the proliferation of hepatocytes by Ki-67 and 2mL of blood were collected for serological assessment: cholesterol, glucose, triglycerides, aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, and total, direct and indirect bilirubin. All data were analyzed by Gaussian distribution. Statistically significant differences between mean values were analyzed using T Student's test. Non-Gaussian data were analyzed using Mann-Whitney's test. RESULTS: The liver of all these rats presented positive staining of Ki-67, indicating liver proliferation. Laboratory results showed no significant difference in serum values between the analyzed groups. The analysis of Ki-67 was significantly more positive in AEPB100 group than in Vehicle group. CONCLUSION: Aqueous extract of Peumus Boldus acute administration exerts significant positive effect on liver regeneration after 24h in rats that underwent parcial hepatectomy, while maintaining unchanged hepatic function.

الوصف: RESUMO Objetivo: avaliar a percepção de qualidade de vida entre residentes no primeiro ano de Residência Médica em relação aos residentes de outros anos, dada a importância dessa questão na saúde. Métodos: estudo comparativo, transversal e analítico realizado no período de fevereiro a abril de 2016, realizado em um hospital de trauma terciário de referência do Brasil. Médicos residentes foram submetidos voluntariamente ao questionário validado da Organização Mundial de Saúde (OMS) sobre qualidade de vida, o WHOQOL-BREF, com preenchimento online. Os residentes foram divididos em dois grupos: primeiro ano de residência (R1) e outros anos de residência. Resultados: noventa e sete residentes de diversas especialidades médicas responderam ao questionário. Desses, 59 eram homens e 38, mulheres. A média de idade foi de 27,7 anos. Residentes do primeiro ano representaram 49,5% dos entrevistados. A qualidade de vida de maneira global foi considerada regular em ambos os grupos.
