The patient presented with symptoms such as voice hoarseness, exertional dyspnea, breathlessness, weight loss, and chest flattening. Diagnostic tests revealed bilateral upper-lobe dominant pleural thickening with subpleural fibrosis, consistent with PPFE. The patient was diagnosed with idiopathic PPFE accompanying left VCP.
