مستخلص: This article, published in Neuropediatrics, examines the structural changes in the corticospinal tract (CST) of the contralesional hemisphere following perinatal brain lesions. The study used diffusion-weighted MRI and fixel-based analysis to assess the impact of these lesions on the ipsi- and contralesional CST. The results showed a reduction in fiber density and cross-section in the ipsilesional CST across patient groups, and an increase in fiber density and cross-section in the contralesional CST in patients with persistent ipsilateral projections. The study suggests that perinatal brain lesions lead to detectable white matter changes in the contralesional CST, particularly in patients with an ipsilateral CST wiring pattern.
