مستخلص: They retrospectively analysed the predictive factors of response and development of liver-related serious adverse events (LrSEAs) in 100 patients with PBC-related cirrhosis treated with OCA from the Italian PBC registry. In patients diagnosed with primary biliary cholangitis (PBC) with insufficient response or intolerance to ursodeoxicholic acid, only two drugs have proven to be safe and effective as second-line options: obeticholic acid (OCA) and bezafibrate, with OCA being the only licenced drug in this scenario.1 However, little is known about its efficacy and safety in patients with cirrhosis, as this population was underrepresented in clinical trials and in real-life cohorts (Table 1 summarizes the limited available data in PBC-related cirrhosis treated with second-line drugs).2-13 Moreover, in May 2021, the United States Food and Drug Administration (FDA) restricted the use of OCA in patients with advanced cirrhosis due to an increased risk of liver injury leading to clinical decompensation.14 This warning was based on the identification of 25 patients presenting rapid clinical deterioration within months of starting OCA.
