Minimally invasive heart surgery has many advantages for the patient, however, difficulties in performing and implementing this procedure are not only found in surgical technique, but in the design of the anesthetic technique, which becomes a challenge for the anesthesiologist. This article presents the first two cases of minimally invasive mitral valve replacement performed in the country. The anesthetic techniques and obtained results are described, and the advantages and complications of these two techniques (anesthesia and surgery) are discussed. Although this series is small, we believe that it is the basis for developing this technique in our center, which is a safe option for patients with mitral valve disease who are not accepted for interventional cardiology. La cirugía cardíaca mínimamente invasiva tiene muchas ventajas para el paciente; sin embargo, las dificultades para realizar e implementar este procedimiento no se encuentran solamente en la técnica quirúrgica, sino en el diseño de la técnica anestésica, que se convierte en un reto para el anestesiólogo. En este artículo se presentan los primeros dos casos de sustitución valvular mitral mínimamente invasiva realizados en el país. Se describen las técnicas anestésicas, los resultados obtenidos, y se discuten las ventajas y complicaciones de estas dos técnicas (anestésica y quirúrgica). Aunque esta serie es pequeña, creemos que constituye la base para que esta técnica se desarrolle en nuestro centro, opción segura para los pacientes con valvulopatía mitral no aceptados por cardiología intervencionista.
