مستخلص: The existence of rare vascular variations increases the risk of vascular injury and therefore may increase the morbidity of the procedure.1 We report two cases regarding the presence of a rare anatomical variation: the presence of a hepatomesenteric trunk (HMT): common hepatic artery (CHA) originated from the superior mesenteric artery (SMA). Pancreatoduodenectomy (PD) is the surgical procedure performed to remove benign and malignant tumours located in the head of the pancreas, duodenum or distal common bile duct. In published series, the prevalence of HMT was between 1.5% and 3.8%.1-3,5,6 Right hepatic artery was partially ruptured and needed reconstruction in only one case with an HMT of all patients with AHAs.6 Vascular injury during PD procedure can lead to severe intra- or post-operative bleeding.
