مستخلص: The three main steps of tumoral orbital surgery are:a. A correct combined subfrontal and anterior approach with the cooperation of the ophthalmologist or simple anterior approach by the advanced technologies for clear exposure of the surgical field.b. An extirpation of neoplastic tissue as radical as possible, avoiding in the same time damages to nervous and ocular structures. This aim has been achieved using laser and CUSA in sequence, especially for deep-seated tumors and for neoplastic infiltration of the optic nerve.c. A correct reconstruction, when necessary, of the roof or wall of the orbit by bone grafts, cranioplastic acrylic grafts, lyodura plus fibrin glue. The authors present 20 selected cases of orbital tumors, originating from the orbit itself (15 cases) or spreading from the surrounding structures, such as anterior fossa, ethmoid, parasellar region, (five cases) treated by these criteria. The results are promising because they noticed an uneventful post-operative course and postoperative CT scan with no evidence of neoplastic residual tissue. Preservation of visual and oculomotor function, appearance and esthetics of the face intact, X-ray evidence of correct reconstruction of the orbital walls were also achieved.
