الوصف: In clinical obstetrical practice, continuous or intermittent fetal heart rate monitoring is the most common method for assessing the intrauterine condition of the embryo. Although its sensitivity is high, it is a non specific method for hypoxia. Umbilical cord blood gas analysis is a well known marker associated with neonatal impairment and morbidity in neonates with fetal academia. Furthermore, Apgar score after birth is a numerical evaluation of the neonatal oxygenation, the cardiac status and the general status. Fetal pulse oximetry, is a non invasive, non traumatic for the fetus and the mother continuous recording of the fetal oxygenation in real time. Doppler ultrasound is assessing the blood flow in important vessels like, the umbilical artery, the middle cerebral artery and the maternal uterine arteries. The purpose of this study was to evaluate if the additional use of these two techniques, to conventional cardiotocography could provide more information concerning the fetal status. Two hundred and forty (240) pregnant women were recruited and they were subdivided in two groups. In the first group (group A), in 109 women in labor with a non-reassuring fetal heart rate addition of fetal pulse oximetry was added. In the second group (group B), 131 women were examined by Doppler ultrasound in the 3rd trimester of pregnancy (32-34 week) and before delivery. In conclusion, parity and maternal age are related with the mode of delivery. Cardiotocography with electronic fetal heart rate monitoring remains the golden standard and the routine first line method for the evaluation of the fetal status. In the cardiotocography-pulse oximetry group (group A), the most important factors for admission to the Neonatal Intensive Care Unit (NICU) were maternal age, Apgar score and oximetry by amniotic fluid status. Fetal pulse oximetry was not related with the mode of delivery, but lower measurement of oxygen saturation is related with parameters indicative of fetal distress (lower values of pH in blood gas analysis, lower ...
