Objective: Peritoneal or genital tuberculosis is an unusual gynecologic problem and it is usually secondary to pulmonary tuberculosis. Being a rare condition and having similar clinical and laboratory findings with ovarian cancer, patients with peritoneal and/or genital tuberculosis are commonly misdiagnosed as having ovarian carcinoma. In addition, subtle primary pulmonary infections which are not diagnosed previously can cause omitting tuberculosis in differential diagnosis and thus patients are subjected to unnecessary extended surgery. Materials and Methods: We present 15 patients with peritoneal or genital tuberculosis who were referred to our hospital. In this retrospective study we evaluated preoperative clinical, imaging and laboratory findings and postoperative surgical and pathological findings of the patients. Results: Primary diagnosis before surgical exploration in patients was genital malignancy, most probably ovarian cancer, in patients. Laparoscopic exploration was performed in one patient whereas the rest of the patients had laparotomy. Two of the patients concomitantly had tubal adenocarcinoma, and one patient had mature cystic teratoma besides peritoneal tuberculosis. Another patient had granulosa cell ovarian tumor along with tuberculosis salpingitis. Preoperative clinical, imaging findings and CA125 levels did not discriminate the patients from ovarian carcinoma. Excluding the concomitant genital malignancy cases 2 patients underwent unnecessary extended surgery. Discussion: Because peritoneal and genital tuberculosis can mimic ovarian carcinoma, it should be considered as a part of differential diagnosis of ovarian carcinoma. Frozen section analysis is essential to avoid unnecessary extensive surgery and increased morbidity. |
