Normal sapheno-femoral and sapheno-popliteal crosses can be found in evaluating so-called primary varicose veins. In these cases superficial refluxes can be related only to primary deep venous insufficiency or to incompetence of typical or atypical perforating veins. These ones have to be considered the main cause of some postoperative relapses. [1,2,3]. For these reasons the role of a complete and carefoul preoperative evaluation has to be underlined.
