مستخلص: 배경 Previous studies using multi-locus sequence test (MLST) showed that clonal transmission or emergence of CRAB occurred occasionally within the hospital. However, their reasons for emergence and persistence of predominant clones within the hospital settings have not been fully elucidated. This study was performed to determine MLST-based genetic relatedness of CRAB blood isolates at a tertiary care hospital over a 3.5 years period and to investigate clinical and microbiologic characteristics of CRAB blood isolates according to sequence types (ST) focusing on predominant clones 방법 All CRAB blood isolates were collected between June 2016 and December 2019 from a University hos- pital, and their clinical data were obtained. MLST was performed using the Oxford scheme, and the STs were assigned using the MLST database. Fitness of dominant ST type strains was evaluated by measure- ment of growth rate and pairwise competition experiments. Virulence was assessed in mice model of intraperitoneal infection. 결과 Of the 180 CRAB blood isolates assigned to specific STs by MLST, ST369 (54%) was the most predominant, followed by ST191 (27%), ST784 (10%) and ST451 (3%). ST191 was the most abundant during June 2016 to July 2017 (61%, 27/44), whereas ST369 was first introduced in August 2017 and replaced ST191 (15%) since then. The 7-day survival rate of patient with ST369 was lower than those with any other STs (vs. ST191, vs. ST784, vs. other STs, P=0.02, P<0.01 and P<0.01, respectively). The in vitro growth of the ST369 was faster than that of ST191 at all time points up to 24 h (P<0.05). The com- petition index of ST369 compared to ST191 is 2.15 ± 0.37 at 24 h after inoculation (P<0.01). Percent recovery of ST369 was higher than that of ST191 after 24 h of incubation (66 (58, 69) vs. 36 (30, 42), P<0.01). ST369 had enhanced virulence with higher mortality than ST191 or ST784 in the mice intra- peritoneal infection model. 결론 This study demonstrates an almost complete repalcement of the predominant ST191 strains clone with ST369 at a tertiary care hospital during study period. ST369 were associated with higher early mortality of patients and may have higher level of virulence and fitness than ST191.
