مستخلص: Infective endocarditis represents the inflammation of the inner tissue of the heart caused by infectious agents. The agents are usually bacterial, but other organisms can also be responsible. Because the incidence is still high, in the past decade, this study aims to define the latest trend in the clinical, diagnostic and therapeutic aspects of the infective endocarditis. We conducted a retrospective study on a series of 70 patients admitted to "Sf. Parascheva" Infectious Diseases Hospital Iasi in the interval 2008 – 2013, diagnosed with infective endocarditis according to Durack-Duke diagnostic and classification criteria. The study protocol included the following data: epidemiological features, clinical picture, positive diagnosis, treatment, and prognosis. An increasing trend in the number of patients with infective endocarditis showed the distribution of the study cases. The patient from our study had the following profile: male, over 60 years old, coming from the urban area, and having cardiovascular history. The fever was noted from the admission in almost all of the cases (97%). As clinical picture, the most frequent signs were cardiac murmur, embolic events, hepato-splenomegaly, Roth spot, onycomycosis andhippocratic fingers. The most common entry gate was surgical (35%), followed by the dental (20%), urinary (15%), and cutaneous (12%) ones. The positive diagnosis of infective endocarditis sustained on clinical and laboratory findings has showed in hematological and biochemical parameters the presence of the inflammatory syndrome with an increased number of white blood cells and of the polymorphonuclears. Bacteriological tests, showed the positivity of the blood cultures; in the studied patients for 28 cases (40%). The detected strains were 28. Most commonly, the first choice treatment was a combination of two antibiotics, as aminopenicillin and aminoglycoside in 24.3% of the cases, the last generation treatment being used as first choice treatment in 18.6% of the all study patients. In our study, a favorable outcome existed in most cases (42.9%), lethality being recorded in 5.7% and a reserved prognosis in 10% of the investigated patients. [ABSTRACT FROM AUTHOR]
