الوصف: We read Angelucci and colleagues' article (1) about the clinical benefit of iron chelation therapy (ICT) in iron-overloaded patients with lower-risk myelodysplastic syndromes with interest. A large proportion of patients in the placebo group discontinued therapy (83% did so because of reasons other than death, and 47% did so because of personal decisions) and received subsequent ICT (33%). While interpreting event-free survival (EFS) curves, the authors speculated that the clinical benefit of ICT might have been underestimated because of the comparison with crossover patients who were still shown in the intention-to-treat placebo curve.Because of methodological issues that were appropriately recognized by the authors, we unfortunately may never have proof of the superiority of ICT over placebo. Nevertheless, in order to better understand the clinical effects of ICT, it would be of great interest if the authors could provide analyses of EFS and overall survival considering ICT as a timedependent covariate (that is, censoring the placebo group at the time of crossover and continuing to follow crossover patients in the ICT group)
