目的:近年,骨粗鬆症の指標として骨代謝マーカーが用いられるようになり,骨代謝異常を変形性関節症の発症増悪因子とする報告が散見される.今回われわれは閉経後の変形性膝関節症患者を対象として,膝関節変形と骨代謝動態の関係について検討した.対象と方法:変形性膝関節症と診断された閉経後女性273 例を対象に,骨密度,骨代謝マーカー,膝関節X 線Grade,脛骨内顆関節面の陥凹距離を測定した.対象を骨代謝高回転型,骨吸収亢進型,骨代謝正常型の3 群に分類し比較した.結果:3 群間で骨代謝マーカーと骨密度の関連性は認めなかった.脛骨内顆関節面の陥凹距離は骨代謝高回転型が骨代謝正常型に比し有意に大きかった.結論:骨代謝回転の亢進状態において有意に脛骨関節面が陥凹していたことから,骨吸収の亢進状態がある一定期間継続することで全身的な骨脆弱性を来し,膝関節の変形が進行したと考えられた.Background: Recently, markers of bone metabolism are used as an index of osteoporosis. Several studies have observed an association between knee osteoarthritis and bone metabolism, but the evidence for such a relation is not yet conclusive. We examined the association between knee deformity and markers of bone metabolism among elderly Japanese women. Methods: Postmenopausal women with medial compartment knee osteoarthritis (N=273) were classified into 3 groups: those with high-turnover bone metabolism (high values for markers of bone resorption and bone formation), high-resorption bone metabolism (high value for marker of bone resorption), and normal bone metabolism (normal value for marker of bone resorption). Depression distance was defined as the distance between the center of the intercondylar eminence and the deepest point of the medial tibial condyle, and values were compared among the 3 groups. Results: Depression distance was 11.4 mm for patients with high-turnover bone metabolism, 9.8 mm for those with high-resorption bone metabolism, and 9.5 mm for those with normal bone metabolism. Depression distance was significantly higher among patients with high-turnover bone metabolism than among thosewith normal bone metabolism. Conclusions: High-turnover bone metabolism but not high-resorption bone metabolism was associated with knee deformity.
