| الوصف: | A insónia constitui atualmente um grande problema de saúde pública, cuja intervenção a nível individual ou num âmbito mais vasto são necessárias. O mercado de fármacos, usados no tratamento deste tipo de distúrbios, é presentemente dominado pelas benzodiazepinas e pelos hipnóticos não benzodiazepínicos, tendo o tratamento por base uma intervenção farmacológica e não farmacológica. Como nova classe de fármacos desenvolvida com o intuito de induzir o sono sem os efeitos colaterais produzidos pelas benzodiazepinas, surgiram os antagonistas da orexina, que em vários estudos demonstraram resultados bastante promissores. Neste seguimento surge o Suvorexant, o primeiro antagonista seletivo dos recetores orexina 1 e 2 a ser aprovado para o tratamento da insónia. Este fármaco proporciona uma alternativa única para pacientes com insónia que não toleram benzodiazepinas ou hipnóticos não benzodiazepínicos ou em casos de ineficácia da terapêutica. O suvorexant é geralmente seguro e bem tolerado nos pacientes, no entanto os ensaios clínicos desta classe ainda apresentam imensas limitações. Em conformidade com o que aconteceu com muitos outros fármacos inovadores, ao longo dos anos milhares de pacientes serão expostos a fármacos cujo conhecimento científico e a segurança são muito limitados. O médico e o farmacêutico têm assim um papel decisivo na identificação, aconselhamento e tratamento dos distúrbios do sono. Um fármaco nunca deve ser a primeira hipótese terapêutica, devendo ser sempre considerados todos os seus riscos e benefícios associados. A indústria farmacêutica está cada vez mais a demonstrar interesse por este tipo de fármacos, havendo várias moléculas promissoras atualmente em estudo, com o objetivo de se obter a molécula ideal, com o mínimo de efeitos adversos. Insomnia is currently a major public health problem, who requires intervention at the individual or broader level. The market for drugs, used to treat this type of disorders is currently dominated by benzodiazepines and non-benzodiazepine sedatives which treatment is based on a pharmacological and non-pharmacological intervention. As a new class of drugs developed to induce sleep without the side effects produced by benzodiazepines, orexin antagonists appeared, which in several studies have shown very promising results. In this way suvorexant appears as the first selective antagonist of orexin 1 and 2 receptors to be approved for the treatment of insomnia. This drug provides a unique alternative for insomnia patients who do not tolerate benzodiazepines or non-benzodiazepine hypnotics or in cases of ineffective therapy. Suvorexant is generally safe and well tolerated in patients, however the clinical trials of this class still have some limitations. In line with what has happened to many other innovative drugs, over the years thousands of patients will be exposed to drugs whose scientific knowledge and safety are very limited. The physician and pharmacist thus play a decisive role in the identification, counseling and treatment of sleep disorders. A drug should never be the in the first place as the first therapeutic hypothesis, and the associate risks and benefits should always be considered. The pharmaceutical industry is increasingly focused on development of this type of drugs, with several promising molecules currently under study, in order to obtain the ideal molecule with the least adverse effects. |
