Cel pracy. Celem pracy było ukazanie przebiegu stwardnienia rozsianegow zaawansowanym stadium na podstawie opisu przypadku pacjenta orazzidentyfikowanie jego problemów, zaplanowanie interwencji i ocena wyników opieki.Materiał i metody. W pracy zastosowano metodę indywidualnego przypadku.Wykorzystano technikę analizy dokumentacji medycznej, wywiadu i obserwacjipacjenta.Wyniki. W pracy ukazano naturalny przebieg stwardnienia rozsianego na podstawiestudium przypadku pacjenta. Zidentyfikowano 12 diagnoz pielęgniarskich,zaplanowano interwencje pielęgniarskie i oceniono wyniki opiekiWnioski: Pierwszy rzut choroby wystąpił u pacjenta 23 lata temu. Obecnie stopieńniepełnosprawności chorego jest bardzo zaawansowany. U pacjenta występuje wieleproblemów zdrowotnych, zarówno w sferze fizycznej jak i psychicznej. ; Aim. The aim of the thesis was to show the course of multiple sclerosis on the basis ofcase-study and to identify the most important problems which are faced by the patient,to plan nursing interventions and evaluate its results.Materials and methods: The case study method was used - analysis of medicalrecords, interview and observation of the patient.Results: The natural course of multiple sclerosis on the basis of case study was shown.Twelve nursing diagnoses were identified, nursing interventions were planned andnursing outcomes were evaluated.Conclusions: The first symptom occurred in a patient 23 years ago. Now the degree ofdisability of the patient is very advanced. The patient have multiple health problems,both in the physical and mental. |
