Background : Little is known about the role of HE4 and its diagnostic specificity in benign ovarian cyst, and it is important to consider potential cause of false-positive results observed with this tumor markers suggested for use in ovarian cancer. Objective : To determine the characteristic of patient with benign ovarian cyst, to determine hystopathologic distribution of benign ovarian cyst, to determine HE4 expression in various benign ovarian cyst tissue and normal ovaries and to determine HE4 expression base from hystopathologic type. Method : This analytic observational study was performed with case control design. In this study, Human Epididymis Protein 4 immunohystochemistry staining was performed in various benign ovarian cyst (n=20) and normal ovaries (n=20) and interpretation was performed by pathologist using allred score. Results : Most of benign ovarian cyst group aged 20 – 50, menarche ≤12 years old, parity ≥1, and normal body mass index. Most of the benign ovarian cyst hystopathologic distribution is epithelial origin which is mucinous cystadenoma. There is a statistically significant relationship between benign ovarian cyst with HE4 expression (p<0.001; p< 0.05; OR 0.4; CI 95%) hence there is no significant difference between hystopathologic subtype of benign ovarian cyst with HE4 expression. Conclusion : There is a significant relationship between benign ovarian cyst and HE4 expression. More study needed to determine the role of HE4 in the pathogenesis of benign ovarian cyst. ; Abstrak Latar Belakang: Sampai saat ini belum banyak diketahui tentang peran dan spesifisitas HE4 pada berbagai kondisi penyakit ginekologis jinak dan sangat penting untuk mengenali kondisi potensial penyebab hasil positif palsu pada penggunaan penanda tumor ini.
