Insufficient milk supply is common reason to cease breastfeeding. However, the signs of suf- ficient milk supply are not properly recognized. The aim of the study was to describe the experiences of mothers and the perceptions public health nurses on breastfeeding counseling when the problem is insufficient milk supply. The data were collected with semi-structured interviews from mothers (n=8) and public health nurses (n=8) and analyzed separately with inductive content analysis. From the perspective of both mothers and public health nurses, the main challenge was organizing enough time for breastfeeding counseling. An extra visit in the child health clinic was difficult to arrange in short notice. Mother's perceived insufficient milk supply was associated with the time needed in breastfeeding counseling. Mothers could not receive counseling if insufficient milk supply became as a surprise to them. Family home was seen as the most convenient environment for the counseling, however, the mothers preferred atmosphere over the physical setting. The mothers asked for justifications for the advice provided by the public health nurses. Breastfeeding counseling needs still be developed that mothers will be able to identify the signs of sufficient milk supply. Immediate breastfeeding counseling should be available if any problems exist. Supplemental education is needed for public health nurses to ensure correct information about breastfeeding and highquality counseling. [ABSTRACT FROM AUTHOR] |

Kokemus maidon riittämättömyydestä on yleinen imetyksen lopettamiseen johtava syy, mutta todellista maidon riittämättömyyttä ei aina tunnisteta. Tutkimuksen tarkoituksena oli kuvata äitien kokemuksia ja terveydenhoitajien näkemyksiä lastenneuvolan imetysohjauksesta, kun haasteena on todellinen maidon riittämättömyys. Aineisto koostui äitien (n=8) ja terveydenhoitajien (n=8) puolistrukturoidusta haastatteluista, jotka analysoitiin erikseen induktiivisella sisällön analyysillä. Keskeisin haaste sekä äitien että terveydenhoitajien mielestä oli sopivan ja riittävän ajan järjestäminen imetysohjaukselle. Ylimääräisen neuvola- tai kotikäynnin toteuttaminen välittömästä tarpeen ilmaantuessa oli vaikeaa. Äidin ennakkokäsitys maidon riittämättömyydestä oli merkityksellistä. Äitien, joille vauvan vähäinen painonnousu ja maidon riittämättömyys tuli yllätyksenä, oli vaikeaa ottaa vastaan ohjausta heti. Perheen koti koettiin luontevimpana paikkana imetysohjaukselle, mutta ohjauksen tunnelma oli äitien mielestä paikkaa tärkeämpi. Äidit kaipasivat ohjaukselle perusteluja voidakseen sitoutua toteuttamaan tarjottuja neuvoja. Imetysohjausta on edelleen kehitettävä äitien valmiuksien vahvistamiseksi tunnistaa vauvan riittävä maidon saanti. Äideillä on oltava myös mahdollisuus välittömään ohjaukseen imetysongelman kohdattuaan.
