    مستخلص: BACKGROUND: Variability in patient care settings and the range of patient handling tasks present challenges in developing and evaluating safe patient handling and mobilization (SPHM) programs. OBJECTIVE: We performed a systematic meta-analysis of SPHM program evaluations. METHODS: Systematic literature review identified published SPHM program evaluations. Injury Rate Ratios (IRR), pre-to post-intervention, were used to estimate intervention effects and to examine the influence of patient care level, program components, and follow-up time using meta-regression. RESULTS: 27 articles reported evaluations from 44 sites. Combined effect estimate for all SPHM programs was 0.44 (95% CI 0.36, 0.54), reflecting substantial injury reductions after program implementation. While specific program components were not associated with greater effectiveness, longer follow-up duration was associated with greater injury rate reduction (p = 0.01) and intervention effects varied by level of care (p = 0.01), with the greatest effect in intensive care unit interventions (IRR 0.14; 95% CI 0.07, 0.30). CONCLUSIONS: SPHM programs appear to be highly effective in reducing injuries. More research is needed to identify the most effective interventions for different patient care levels.     مستخلص: Objective: Evaluate the validity of two self-report symptoms surveys with two disorder classification protocols. Participants: 100 graduate students at a private school in the Southwest United States. Methods: Study participants completed two self-report upper extremity musculoskeletal symptoms surveys: a nine item 10 cm Visual Analogue Scale (VAS) and a nine item Likert categorical scale anchored from "None" to "Very severe". Clinical examinations were administered using two musculoskeletal disorder classification protocols. Results: For the nine body regions, concordance between the two self-report symptoms scales ranged from 0.49-0.75. Overall there was greater than 80% agreement for the two disorder classification protocols. Using either symptom survey with either disorder classification protocol provided high sensitivities and specificities (Youden's J ⩾ 0.70). Three of possible six symptom survey/classification protocol pairings provided high sensitivities and specificities across all disorder groups. Conclusion: In this graduate student sample, none of the self-report symptom survey-classification protocol pairings was demonstratively more useful than any other pairing for studies of musculoskeletal disorders among computer users.     مستخلص: Objective: The study examines temporal variations in upper-extremity musculoskeletal symptoms throughout the day, over a week and throughout the semester. Methods: 30 undergraduates were followed in a repeated measures study throughout a semester. Upper extremity musculoskeletal symptoms data were collected on handheld computers randomly throughout the day for seven days over three data collection periods. Multilevel statistical models evaluated associations between time-related predictors and symptoms. Results: In adjusted models, pain reported at baseline was associated with increased odds of experiencing both any symptoms (OR=15.64; 90% CI 7.22–33.88) and moderate or greater symptoms (OR=16.44; 90% CI 4.57–29.99). Any symptoms were less likely to be reported if responses occurred at 58–76 days (OR=0.66; 90% CI 0.49–0.86), 77–90 days (OR=0.29; 90% CI 0.20–0.42) and 91–117 days (OR=0.54; 90% CI 0.39–0.75) into the semester compared to 35–57 days. Similarly, responding after midnight was associated with greater odds of reporting moderate or greater symptoms (OR=21.33; 90% CI 6.49–65.97). There was no association observed for day of week and symptoms. Conclusion: This pilot work suggests upper extremity musculoskeletal symptoms exhibit temporal variations related to time of day and days into semester. Understanding the natural history of musculoskeletal symptoms and disorders is needed when designing epidemiologic research and/or intervention studies using symptom outcome measures.     مستخلص: Objective: To determine agreement between two posture assessment survey instruments and which, if any, were correlated with experiencing upper extremity musculoskeletal symptoms. Methods: Thirty undergraduate participants had three postural assessment surveys completed, one each for three separate 7-day data collection periods during a semester. Two observation assessment tools were used, a modified Rapid Upper Limb Assessment (mRULA) for computer users for the right and left limbs and the University of California Computer Use Checklist. Concurrently, upper extremity musculoskeletal symptom experience paired to each postural assessment was measured. Lin's concordance correlation coefficient evaluated survey agreement and multi-level statistical models described associations between survey responses and symptoms. Results: There was no agreement between the two postural assessment tool scores (p> 0.85). In adjusted models, the UC Computer Use Checklist was positively associated with symptoms occurrence (OR=1.4, 90% CI 1.2–1.6 for any symptoms; OR=1.3, 90% CI 1.0–1.6 for moderate or greater symptoms). Associations with mRULA scores were inconsistent in that they were sometimes protective and sometimes indicators of risk, depending on the covariates included in the models. Conclusion: The mRULA for computer users and the UC Computer Use Checklist were independent of each other; however, due to the inconsistent associations with symptoms we cannot conclude one instrument is superior to the other. Our data do suggest the UC Computer Use Checklist demonstrates a traditional relationship with symptoms, where increasing scores signifiy greater risk. We observed a nontraditional relatioship with symptoms for the mRULA for computer users that needs to be further examined. This is a pilot study and, thus, findings should be interpreted as exploratory. Associations observed in the current study will be used to test hypotheses in the cohort study recently conducted.     مستخلص: Purpose: To characterize undergraduate computer use using different data collection methods, emphasizing computing-related postures, use patterns and upper extremity musculoskeletal symptoms. Subjects and methods: In Spring, 2004, undergraduate students from a single dormitory at a private university agreed to complete a College Computing & Health Survey. For three separate data collection periods each lasting a week, we observed postures during computer once per period and continuously measured computer input device usage. During these three periods, students self-reported computer usage and symptoms 3–5 times daily. Results: Thirty students participated and all completed the study. Eighty-six percent reported ever experiencing symptoms after computer work. There were no time-related trends across data collection periods for posture, symptoms, and computing activities and patterns. Typed work and communicating (when compared with playing games) were usually the predominant computing activities throughout the semester. There was significantly greater self-reported computer use than that directly measured (p<0.05). Conclusion: This is the first study that utilized several methods of exposure assessment to describe computing postures, use patterns and upper extremity musculoskeletal symptoms among a college student cohort. Epidemiological studies need to explore time-related changes such as time of day, weekday, and days into the semester to further understand symptoms, posture, and computer use changes.     مستخلص: Purpose: To replicate Hupert et al.'s [5] evaluation of computer-related upper extremity musculoskeletal symptoms, functional limitations, academic performance impact, medication use and health services utilization among a college student population. Subjects and methods: A cross-sectional survey of undergraduate students living in a single residential dormitory at a private southwestern university who agreed to participate completed the College Computing & Health Survey in the Spring of 2001. Results: Of the 127 dormitory residents, 116 students participated and 54% reported experiencing symptoms associated with computer usage. Sixty-two percent of students surveyed experienced functional limitations. More women than men reported symptoms, functional limitations and neck and shoulder pain; those with functional limitations reported higher use of medications than other participants. Conclusion: These findings, which match the general findings of the previous research study conducted with the same instrument but in a different college student population, suggest a high level of computer-use-related musculoskeletal symptoms among college students. Universities may want to consider providing ergonomic training designed for college students to: conduct workstation assessments; identify computer related problems (risk factors); and, propose ergonomic solutions.     مستخلص: Recently, researchers have reported high musculoskeletal symptom prevalence at several US colleges. Since ergonomic interventions have been shown to prevent and reduce disability, it is important to identify the risk factors for developing symptoms among college students. A nested case-control study was completed to determine computer-related ergonomic risks associated with musculoskeletal symptoms. A trained observer completed ergonomic assessments on 52 randomly selected cases and controls. More than 75 percent (cases and controls combined) of the population was exposed to nine potential postural strains including: arms not along side during keying or mousing; lower back not supported; not having chair accessories; computer monitor not adjustable; mouse being too high or low; hand/wrist/forearm in contact with the desk edge; lack of wrist support; and keyboard not being adjustable. Cases and controls were equally likely to have substantially elevated risks but because the sample was small and lacked power, no risks were statistically significant. Since many known risk factors were prevalent in cases and controls, more research is required to evaluate and prevent injury in this population. [ABSTRACT FROM AUTHOR]

    مستخلص: Purpose: Occupational computer use has been associated with upper extremity musculoskeletal disorders among working-age adults, but little is known about computer-related musculoskeletal problems among college students. We carried out a descriptive epidemiological study of computer use-associated symptoms, functional limitations, and medication and health care utilization in this population. Subjects and methods: Cross-sectional survey of 240 undergraduates in the second through fourth years at a residential dormitory at a four-year college with random housing assignments. Results: 194 students returned useable surveys (81% response rate). 42% reported upper extremity pain or discomfort when using a computer in the preceding two weeks. 41% said this pain or discomfort caused functional limitation and 9% said that these symptoms hindered academic or extracurricular performance. 23% reported taking medications for upper extremity pain related to computing (4% regularly) and 16% had seen a health care provider for computer-related symptoms. Female students, students of racial/ethnic minority groups, and students who experienced symptoms with ≤ 1 hour of computer use were more likely to report symptom-related functional limitation than others. Conclusion: College students report high rates of computer use-associated upper extremity musculoskeletal symptoms and symptom-related functional limitation.     مستخلص: A participatory approach was used to create a computer ergonomics workshop for college students, incorporating an instructional systems design process and adult learning inquiry perspectives. The primary objective of this participatory ergonomic pilot intervention was to involve students throughout the training design process in solving computer workstation ergonomic problems and adopting healthy computing behaviors. Students' level of participation included becoming part of the training design team, a co-facilitator, or a student trainee. A second objective was to examine the translation of an industrial office ergonomics training program into a college computer ergonomics training program. The long term goal was to reduce upper extremity symptoms and disability. The program was piloted at one private university. The three student trainees significantly increased their knowledge of computer ergonomics from 69% post-training. Trainees were also successful in conducting computer ergonomic evaluations of students' computing work areas. They achieved 100% proposing solutions in five ergonomic workstation assessments in the field. This approach was successful in creating a sense of ownership among the student developers and facilitators as reflected in their self-reports during a post-intervention debriefing. The results of this pilot study justify formal controlled trials of this intervention in university students, who will become tomorrow's workers. [ABSTRACT FROM AUTHOR]
