الوصف: [EN] Allogeneic hematopoietic stem cell transplantation (HSCT) represents the only curative option for patients affected by haematological neoplasms such as acute myeloblastic or lymphoblastic leukemia or by congenital diseases such as immunodeficiencies. In allogeneic HSCT, a donor¿s immune repertoire is infused into a patient to reestablish hematopoiesis and represents massive interference with immune system homeostasis in patients. Currently, thanks to the increase in the number of donors worldwide, in the register of unrelated donors, the use of cord blood and partially compatible related donors has secured virtually one donor for all people in need of a HSCT. However, between 30-40% of patients who undergo HSCT have ABO incompatibility, because the inheritance of HLA genes is independent of the genes that encode ABO antigens. Rh compatibility between donor and recipient is not necessary to perform HSCT. There is currently no consensus on the impact of ABO incompatibility on HSCT, although recent studies have shown that ABO incompatibility does not affect survival, relapse rate, mortality rate or GvHD incidence. The information on the kinetics of blood and serum ABO group change is scarce; therefore the present work studies the erythrocyte uptake in peripheral blood by determining the serum and blood group serially over time, as well as in the monitoring of isoagglutinins in the receptor, evaluating erythropoietic chemerism and transfusion requirements. The erythrocyte uptake is different depending on the type of ABO incompatibility, a delay in the graft is being observed in the mayor incompatibility, generating more complications when the donor is A and the recipient 0. A variable kinetics is observed in isoagglutinins, generating most changes on day 45 post-HSCT, visibly increasing the titre of IgG IHA. In a large number of patients, complete chemerism was observed on day 30 post-HSCT and there were no differences in transfusion requirements between patients who underwent identical ABO HSCT or incompatible ...
