    الوصف: A 38-year-old male presented with an avulsion fracture of the anterior half of the foramen magnum due to a traffic accident. He had palsy of the bilateral VI, left IX, and left X cranial nerves, weakness of his left upper extremity, and crossed sensory loss. He was treated conservatively and placed in a halo brace for 16 weeks. After immobilization, swallowing, hoarseness, and left upper extremity weakness improved. Hyperextension with a rotatory component probably resulted in strain in the tectorial membrane and alar ligaments, resulting in avulsion fracture at the sites of attachment, the bilateral occipital condyles and the inferior portion of the clivus. Conservative treatment is probably optimum even for this unusual and severe type of occipital condyle fracture.

    الوصف: A 51-year-old female presented with a rare case of synovial cyst at the cruciate ligament of the odontoid process associated with atlanto-axial dislocation, manifesting as a history of headache and numbness in her left extremities for 5 months, and progressive motor weakness of her left leg. Neuroimaging studies revealed a small cystic lesion behind the dens, which severely compressed the upper cervical cord, and atlanto-axial dislocation. The cyst was successfully removed via the transcondylar approach. C-1 laminectomy and foramen magnum decompression were also performed. Posterior craniocervical fusion was carried out to stabilize the atlanto-axial dislocation. The cyst contained mucinous material. Histological examination detected synovial cells lining the fibrocartilaginous capsule. Synovial cysts of this region do not have typical symptoms or characteristic radiographic features. Careful preoperative evaluation of the symptoms and a less invasive strategy for removal of the cyst are recommended.

    الوصف: A one-year, 9-month-old boy presented with pediatric head trauma associated with unusual accessory cranial sutures. Radiography demonstrated unusual bilateral longitudinal linear bone defects extending from the foramen magnum to the mendosal sutures, and bilateral transverse linear bone defects around the foramen magnum. No swelling or soreness were found in the occipital area, and there was no past history of head trauma. Therefore, the bone defects were considered to be accessory cranial sutures. Complex developmental patterns of the occipital bone and the considerable normal variation of sutures may simulate fractures around the foramen magnum. The present case of accessory cranial sutures is another example.

    الوصف: Tumors of the foramen magnum are rare and the incidence is between 1.6% and 2.4% of all spinal tumors; therefore the personal experiences of one neurosurgeon with this type of tumor are necessarily limited. This unfamiliarity, combined with the curious presentation of some of the lesions due to the complex anatomy of this region, has made the diagnosis of foramen magnum tumors difficult. Unless this is kept in mind, one might fail to consider these lesions in the differential diagnosis, causing a delay in appropriate treatment until permanent damage results. The purpose of this paper is to describe the symptomatology and diagnosis, as well as the surgical treatment, of 21 cases of foramen magnum tumor encountered at our clinic during the last 10 years.

    الوصف: The authors report the incidental discovery of a meningioma during surgery for an acute infratentorial subdural hematoma. The patient was a 46-year-old female admitted for severe head trauma. A plain skull X-ray showed a sharp linear fracture in the left posterior region, extending from the parietal area to the foramen magnum. An initial computed tomographic scan demonstrated a high-density intracranial mass in the left posterior fossa. Three hours after onset, the patient underwent emergency craniectomy and a subdural hematoma weighing 30 g was aspirated. During this procedure, an infratentorial tumor 30 mm in diameter was found to be attached to the dura and was totally removed. The tumor was a fibroblastic meningioma. The patient's postoperative course was uneventful. Most incidentally discovered meningiomas have been found in patients with ruptured aneurysms or chronic subdural hematomas. This is the first reported case in which the tumor was found in association with an acute infratentorial subdural hematoma.
